Based on my observation during the on-site inspection on 08/27/2025, there is sufficient evidence to support that Licensee was out of ratio on 08/27/2025. When I arrived at the child care family home, there were six unrelated children and three of Licensee’s minor children, who were under the age of six years old, inside the home with Licensee. It cannot be determined if Licensee left CCSM 4 alone with twelve children for four hours. There were no records detailing the arrival and departure times for CCSM 1, CCSM 2, and CCSM 4 on 07/29/2025. Licensee stated that CCSM 1 and CCSM 2 were caring for the children on 07/29/2025. There was documentation that three children were being cared for in the home on 07/29/2025.
