On 01/28/2021 I received an incident report from licensee, Jessica Truman. The incident report stated Child A "slid/fell off a 19-inch-high step/platform onto the floor and was unconscious for approximately 30 seconds. Child care staff member Brooke Brandt saw the incident happen and said Child A did not hit her head. She appeared to be sleeping and not very responsive. Licensee, Ms. Truman tapped on Child A's cheek; she woke up and started to cry. Child A then pointed to the high chair and wanted to eat. She ate her lunch "as if nothing had happened". Ms. Truman contacted Child A's Mother and continued to monitor Child A until Child A's Mother arrived to take Child A to the hospital. Child A's Mother texted Ms. Truman from the hospital to let her know the doctor thought Child A had a seizure and is referring Child A to a neurologist. On 01/29/2021 I interviewed Child A's Mother. On 01/28/2021 she received a call from Ms. Truman saying Child A was very tired, walked over to the step and sat down. Child A fell off the step onto her butt and then laid down as if she was sleeping. Child A never hit her head, it was like she was unconscious/ sleeping for 30 seconds. Child A's Mother took Child A to Mid-Michigan Hospital in Alpena. The 2 doctor who evaluated Child A said it sounds like Child A had a seizure and has referred Child A to a neurologist. On 02/02/2021 I completed an onsite inspection at the Truman group child care home. Licensee, Ms. Truman was present with child care staff member Brooke Brandt and nine children in care. Ms. Brandt stated on 01/28/2021 Child A went and sat down on the step/platform. She was sitting backwards and fell off onto her butt. Child A then laid down. Ms. Truman stated Child A was tired that day. Child A's Mother reported Child A did not sleep well the night before. Child A has been referred to a neurologist because the doctor felt it may have been a seizure. There have been no issues with Child A since 01/28/2021. I observed the platform/step area. The top of the platform is 19 inches high. There is on small step before getting to the top of the platform. There was nothing that would cause the step/ platform to be slippery and the total rise was under 24 inches, so a hand railing is not required.

There is no evidence that Ms. Truman did not ensure the step and elevated structure was protected to prevent falls.
