On 09/30/2022, I completed an unannounced onsite inspection at the Cardinal Center Day Care. I met with Assistant, Melissa Delong who reported the program director Justin Diegel was not present today. I was taken to the infant room and requested all child information cards for those in the infant and preschool room. On 09/30/2022, I met with and conducted an interview with child care staff member 1, Katie Tobey, who is the lead teacher for the infant room. Ms. Tobey has worked at the center for two and a half years. She is the lead and has been the lead her entire time employed. She has different assistants who come into the classroom. She has not had any incidents where children are being hurt or injured with marks or bruises. If a child does have an incident, they fill out the incident report and provide it to the family. On 09/30/2022, I conducted an interview with child care staff member 2, Debra Erbacher. She is an assistant and has worked at the center since August 2022. She has previously worked in a different child care center. She was provided training regarding how to close for preschool room, afterschool room, diaper changes, and Brightwheel. They do a lot of on-the-job training. She normally starts in the infant room in the morning and will sometimes cover breaks in the preschool and GSRP room, also covering their lunches. In the preschool room they work with the children to keep their hands to themselves. There is one child in particular who is a little bit more of a challenge. Child E has a difficult time with sharing and tends to be in the middle of arguments over things. He has come a long way and is a work in progress but has made great strides. She believes the child care staff members are appropriately supervising the room and ensuring no child is being hurt. They spend a lot of time helping children use their words and talk through any conflict that happens. On 09/30/2022, I conducted an interview of child care staff member 4, Jaclynn Mead who is the lead in the preschool room. There is a few children in the preschool room who could be having issues. If someone gets hurt, they fill out incident reports, if there is not a mark or injury, they do not normally fill out an incident report. If this had come in last year, she could see it being about Child E, he is a pretty big two and half year old. He had a lot of issues last year, but this year they have seen 4 improvement in his behavior and issues at school. She believes he probably needs an evaluation by a doctor for possible autism or ADHD, but that is not up to her. They try to be supportive to all parents and give advice or seek advice on how to handle children in the classroom when they are having issues. The more they know about a child and their challenges the more they can help the child. There are other children who get excited easily and have to be reminded to not be so touchy and to keep their hands to themselves. She believes throughout the week they might have 10 incident reports, spread out between the children. She is not sure if that is a lot or not, this is the only daycare she has worked at. They spend a lot of time redirecting children in this age group. She believes the high number of children and low number of staff can be difficult as well. Most of the children enrolled in the program have a parent who is employed by the school district. Of the 16 total enrolled, 13 have parents who are employed by the school district. They have one child, Child B, who is also accident prone, she is one of the youngest in the classroom, her parent is not employed by the school district. She feels like they address all the issues in the classroom regardless of whether a parent works for the school district or not. On 09/30/2022, I met with and conducted an interview with child care staff member 1, Katie Tobey, who is the lead teacher for the infant room. There were a total of eight infants today, and 12 enrolled in the infant room, they utilize a white board in the room to denote primary caregiving duties. They use Brightwheel application for parent notifications and daily updates. Ms. Tobey allowed me to utilize her phone to review diaper changes for the infants in the current week as they do not have a classroom tablet for use. I reviewed all 12 of the infants' profiles in Brightwheel regarding their diaper changes in the week of September 26, 2022, through September 30th, 2022. Of the 12 infant records reviewed on the phone all had at least one if not more episodes of more than two hours between a documented diaper change and half had periods of three to four hours between documented diaper changes. Ms. Tobey has worked at the center for two and a half years. She is the lead and has been the lead her entire time employed. She has different assistants who come into the classroom. She was trained in regard to diaper changes and knows what the licensing book requires as well as the centers policy regarding diaper changes. She was a nanny for many years and has worked in day cares as well prior to this. She works 6:45 AM until 3:00 PM, she has a break 7 between 11-11:30 AM. When she returns, she always checks in with where the diaper changes are. She has reviewed Brightwheel during the day and if she notices it does not look like diaper changes were completed, she asks the workers if they were and has them document them. She has at time had to speak to workers regarding not changing the children enough, this has been done in verbal conversation only. They try to keep a schedule going, after breakfast, they change them, then after snack they change them, after lunch they change them, when they wake up, they check and change them. The staff have been trained on how to use Brightwheel and it is expected they use it. Ms. Tobey acknowledges they might not document all of the changes in the Brightwheel application but believes they are changing diapers every two hours. Sometimes one person changes, another enters into the phone and things may get missed. Discussed returning to primary caregiver changing and entering the diaper change in Brightwheel so it is more accurate. She has not had any parents talk to her about their child having diaper rashes believed to be from lack of changing. There are only a few children right now with diaper rashes and they are being changed and having diaper cream applied. She knows the process for changing diapers, they keep track of it on Brightwheel, when checked and changed, and they are supposed to enter it right away. The children are taken to the diaper change area, they are changed, they have bins with all their stuff (diapers, wipes, creams). They stay right next to the diaper changing area.
