On 04/20/2022, I received an incident report from 04/13/2022, stating that Child A's Mother arrived at the center the previous day and informed program director Ms. Alexis DeWitt that the night prior, she found a bump on Child A's (male, 16 months) head with a small scratch that was bleeding. Child A's Mother took Child A to the doctor, where he was diagnosed with a concussion. I interviewed Ms. DeWitt. SHe reported that, on April 12, 2022, Child A's Mother came in to the center and asked if Ms. DeWitt knew anything about Child A getting hurt the day prior. Child A's Mother saw a mark on Tuesday morning before bringing Child A to care. Ms. DeWitt observed a red mark with a scratch and bump on his 2 head. Ms. DeWitt denied that she knew how Child A got the mark. Ms. DeWitt reported that, on April 11, 2022, she left around 4:50 P.M. and Child A left at 5:10 P.M. from the playground. Ms. DeWitt interviewed the child care staff members that were on the playground that afternoon and none of them witnessed him fall. They stated that he was playing on the grass prior to leaving. Ms. DeWitt reported that, on the day in question, she went inside around 4:20 P.M. to clean up. She denied that an incident would have gone unnoticed while she was there because she makes sure that her staff are spaced out on the playground and she would have heard Child A cry. After Ms. DeWitt left the playground, child care staff member Ms. Jordan Warner was on the preschool playground adjacent to the toddler playground. Child care staff members Ms. Cassie Kelly and Ms. Kaylee Darling were on the infant/toddler playground with Child A and other children. There was no lead caregiver present at the time, but Ms. Kelly was in charge of the children from the infant room. Ms. DeWitt reported that Child A was sliding down the hill and denied that she ever heard him cry. She reported that Child A's Mother left him in care on the morning of April 12, 2022 and went to work, where her supervisor told her she should take Child A to the doctor. When she did, she was informed that he had a mild concussion. Ms. DeWitt denied knowing what time on Monday Child A's Mother saw the injury. Child A's Mother removed him from the child care. Ms. DeWitt reported that she has had previous active supervision issues with Ms. Kelly and Ms. Darling. She had Ms. Kelly complete an active supervision training in the past and she seemed to benefit from training, but Ms. DeWitt did not know how Ms. Kelly supervised when she was not around. The center also recently completed an active supervision training facilitated by the Executive Director, Ms. Michelle Stark. Ms. DeWitt denied that the center has ever had concerns with Child A or Child A's Mother. Child A's Mother has come to her prior with concerns and she denied that Child A's Mother is difficult to work with. Ms. Kelly reported that she has been an assistant, mainly in the infant room, since 09/21/2021. Children in that room are ages birth to 2 years. There are normally five children present with two child care staff members. Ms. Kelly reported that, on 04/12/2022, she was in charge of the infants and toddlers on the playground. She was with Child A everywhere he went because he was climbing the hill. He played on the hill, the bikes, and in the sandbox. She denied that she ever saw him fall and denied that he cried all day. According to daily attendance records, there were eight infants and toddlers present on the playground with Ms. Kelly and Ms. Darling. According to Ms. Kelly, Child A was "a super happy baby all day with us." Ms. Kelly went inside at 4:50 P.M. and left the center through the playground at 5:03 P.M. She stated that he was happy when she left, said goodbye to her, and blew her kisses. Ms. Darling and Ms. Warner were still out on the playground with the other children. Ms. Darling has worked at the center for approximately three months as a floater. Ms. Darling reported that she was in the toddler room on 04/12/2022
