While onsite on 10/07/2025 the staff training packet was reviewed to ensure that all requirements were met per R 400.8216 Orientation and R 400.8216a Professional Development requirements were met. The early childhood center's MiRegistry account was also reviewed to ensure that current staff were up to date on required trainings.

Teacher 2 found Child A (2 years) and Child B (2 years) alone on the south end of the playground at approximately 9:46am. Program staff did not observe the children leave the classroom. The Program Administrator did not see the children outside at approximately 9:35am, consequently the children were outside unattended for approximately 11 minutes.
