There is no evidence that Child A's teeth were chipped while in care on 5/1/2025. When Child A fell off of the children's sized picnic table on 3/28/2025, child care staff were appropriately supervising him, they were just unable to reach him in time to stop the fall.

On 04/13/2021, I spoke to Program Director, Tricia Oostveen regarding the complaint. Ms. Oostveen stated this never, never, never happens. The infants are laying in the back and are never hanging over the sides of a stroller to 2 her knowledge. She stated the infants are in the back the older infants, five months until able to walk are in the front of the stroller. They don't have any ties. Over six months ago they had a walking rope, but it was for the older (3 children not '/2-4 1/2) the younger ones; however, it was so rarely used. I interviewed Child Care Staff Member Rita Langerhorst regarding the complaint. She stated that she has been employed at the center for almost 4 years in the infant room. Ms. Langerhosrt denied that anyone stopped her during a walk to ask her to straighten an infant's head. She said they usually sit the infants that cannot sit up on their own in the back of the stroller and lay them down. The older infants that can sit up go in the front. They never had anyone stop them to tell them to fix any of the children. They never tied anyone to the strollers. The children would sometimes hold onto the stroller. They have walking rope that the children put their hands through a lope to hold onto the rope; however, they don't use it very often. They normally have enough staff that go on the walks which allows the children to hold the staff member's hand and/or onto the sides of the stroller. They are never dragging them at all. They go at a slow pace because they know that the children can't walk fast. If they know they have a child that can't keep up they put them in a stroller. She denies that anyone spoke to them two years ago about dragging the children. She doesn't know where this is coming from, but she doesn't remember anything like this happening. I interview Child Care Staff Member Adriana (Marisol) Martinez regarding the complaint. She stated she has been working at the center for two years now. She stated that this allegation is very upsetting because they have never been stopped regarding the children at all. They always make sure the infants are laying down in the back. They always pull over the check on them to make sure they are safe and secure and haven't moved. She said she is really confused about the allegation. She stated that it is mainly her and Rita Langerhorst in the infant room but sometimes Morgan Devries, Chasity Zimmer and occasionally Laura may step for breaks. She said she is not sure what this person is talking about because most of the last year they were at Little Munchkins 2, the view pond location. When they did have older children here, they always had enough staff to go on the walk. The children would either hold onto the side of the stroller or a staff member's hand. They never tied any children to the stroller. She wishes this person who made this allegation would have a picture or something. She also does not recall using the walking rope or anyone stopping to talk to them two years ago regarding dragging the older kids. I interviewed the complainant about her concerns. She stated one incident happened on 04/08/21 in the morning right before 11:00am. There was a woman with four infants in a stroller. The woman had blond hair with kind of a messy bun. This person looked to be in her late 30's early 40's. She spoke to the person. She asked her to straighten the infants head and she did. The complainant also explained that two summers ago she observed two women walking: each woman had a quad stroller with 4 infant/toddlers inside and the older children were trailing behind holding onto a walking rope. The two women were talking to each other, and 3 the older children were struggling to keep up. She recalls a little girl in particular who was upset and nearly falling over. She stopped them and told them to slow down. On 07/30/2021, I spoke with Child Care Staff Member, Chastity Zimmer.
