On July 2, 2021, I spoke with Child A's Mother on the telephone. She stated that Child A had gone on a center sponsored roller skating field trip on June 30, 2021. At pick up time she was told by child care staff member, Layla Mohamed, that Child A had fallen twice at the roller-skating rink, had complained of his right arm hurting, and they had given him ice. Child A's Mother said when he came out to the car at 3:30pm, he was crying and had trouble buckling his seat belt. 2 He was in a lot of pain the rest of the evening so she took Child A to the hospital and he was diagnosed with a broken arm. Child A's Mother said she spoke with the program director, Patrice Greene on July 1, 2021. Ms. Greene told her Child A had fallen at the start of the field trip (at approximately 1:30pm) and was given ice for his arm. Child A's Mother stated that Ms. Greene told her Child A's shoulder looked swollen after the first fall. And that he was allowed to skate again and fell a second time. Child A's Mother was concerned that he had been allowed to skate again after hurting his arm and why she had not been called when the accident happened. On July 13, 2021, I made an unannounced on-site inspection at the child care center. I spoke with program director, Patrice Greene and child care staff members Layla Mohamed and Connie Hull. Layla Mohamed stated she was in charge of the group Child A was with during the field trip on June 30, 2021. Ms. Mohamed said she did not see Child A fall but saw him on the floor after he fell. She said this was at approximately 2:00pm or 2:15pm. She went over to check on him and he was crying and told her his "arm hurt." Ms. Mohamed said she had Child A sit down and gave him an ice pack. She asked program director, Ms. Greene to assess Child A. Ms. Mohamed stated that Ms. Greene assessed Child A and told her that she did not see any swelling or bruising. Ms. Mohamed did not assess Child A or check him for swelling or bruising. Child A then sat for a while, had stopped crying, and iced his arm. Ms. Mohamed said pizza was served and Child A got up, served himself, and ate pizza. After he finished eating, he told Ms. Mohamed, "I feel better." She stated that he had rested a total of approximately 30 minutes from the time he fell, was assessed by Ms. Greene, iced his arm, and ate. Child A went to skate again and fell once more. Ms. Mohamed said, after he fell the second time, she offered him ice but he refused it and he sat at tables for the remainder of the field trip. She does not recall noticing if Child A moved any differently after falling. She does not remember seeing Child A favoring one arm over the other. He was sitting most of the time and seemed to be using both hands. She said Child A complained about his arm hurting while they were on the bus. He was holding his right shoulder when he said that. She told him they would get him ice when they arrived at the school. Ms. Mohamed said when the group arrived back to school, Child A's Mother was waiting in her car in the parking lot to pick up Child A. When Child A saw his mother, he began to cry. She heard Child A's Mother call out "what's wrong?". Child A walked over to the car and said, "I fell." She said, Child A's Mother looked at his shoulder and said, "you look fine." Ms. Mohamed heard Child A's Mother say, "go have snack." Ms. Mohamed told Child A's Mother that Child A had fallen twice during the field trip and that he had been given ice. Child A's Mother told her that she did not know they were going roller skating and would have sent in his own skating gear if she had known. Ms. Mohamed said that once they were back in the building Child A had snack and prepared for dismissal. He did not cry further or complain about being in pain. Ms. Mohamed said before leaving, Child A asked her to help him put on his backpack because he could not do it himself. 3 Patrice Greene stated she is the program director and said during the roller-skating field trip on June 30

On July 2, 2021, I spoke with Child A's Mother on the telephone. She stated that she was not aware that the roller-skating field trip was scheduled for Wednesday, June 30, 2021. She said she was given a list of all the field trips for the summer and told they would be on Mondays and Thursdays. She stated the list did not include dates. She expected the roller-skating field trip to be on Thursday. If she had known the roller-skating field trip was on Wednesday, June 30, 2021, she would have made sure to send Child A's skates and pads because Child A "is not a good skater." I asked Child A's Mother to send me a photograph of the list. At the time of writing this report I have not received a photograph. On July 13, 2021, I spoke with program director Patrice Greene and child care staff member Layla Mohamed. Ms. Greene stated that during enrollment for the summer program parents are provided with a list that contains all the field trips for the summer. Each parent signs off on the field trips giving permission for their child to attend. The day before the field trips, Ms. Greene will send out a reminder note with the date of the field trip, where they are going, and any specific notes about the trip. All splash pad field trips are on Mondays and all other field trips are typically on Thursdays. But the roller- skating filed trip was scheduled on a Wednesday, June 30, 2021, due to a scheduling conflict on Thursday, July 1, 2021. Ms. Greene said that Child A's Father completed the enrollment paperwork and he was given the list of field trips. Child A's Father picked up Child A on June 29, 2021. Ms. Greene explained she sent home the reminder slips with the June 30, 2021, date with children on June 29, 2021. Ms. Greene stated she had not been told that Child A's Mother and Father did not reside 5 at the same residence and share custody of Child A until after Child A was injured. Child A's Mother was not aware that the roller-skating field trip was on a Wednesday instead of Thursday because the information about the change had been given to Child A's Father. Ms. Greene provided me with copies of the field trip list, reminder slip dated June 27, 2021, signed permission slip from Child A's file, and attendance records for the week of June 28, 2021. The information provided to parents concerning the field trips include the destination of the field trips, the date of the trip. The field trip permission slip was signed by Child A's Father and dated June 13, 2021. The attendance records document Child A's Father picked up Child A on June 27, 2021. On July 15, 2021, I spoke with Child A's Father on the telephone. He stated that he thought the roller-skating party was going to be on July 1, 2021, but that it had been "re-scheduled" for June 30, 2021. He could not remember if he received the information about the date change before or after the field trip. Child A's Father did not remember if he received a reminder note on June 29, 2021, or if he picked up Child A on June 27, 2021. He did not recall signing the permission slip for the field trips. I informed him I have a copy of the permission slip with his signature and dated June 13, 2021. I have provided technical assistance to Ms. Greene to include dates on the initial list given to parents at the beginning of the summer.
