On 05/20/2021 the department received an incident report from the facility stating that Child A was diagnosed with a severe concussion due to an incident that occurred while in care. The report stated that on 05/18/2021 near the end of the day, Child A and Child B were transitioning from the Pratt classroom into the Terance classroom while waiting for parent pickup. There had been an incident prior where Child B pushed Child A but no reports of punching. The report identified that when Child A's Mother arrived at the center, she noticed bruising on Child A's forehead. Child A and his brother Child C (male, age 9) told their mother that Child B had punched Child A multiple times. The report indicated that Child A's Mother contacted the facility on 05/19/2021 and stated that she was taking Child A to Urgent Care due to a headache, fainting/dizziness, bruising, and vomiting. Following the Urgent Care visit Child A's Mother disclosed that Child A had been diagnosed with a severe concussion. On 05/20/2021 I contacted Child A and C's Mother by phone. She stated that when she picked up Child A and Child C from child care she noticed bruising on Child A's head. She stated Child A told her that Child B punched him in the head and chest resulting in bruises. She identified that she was told by Child A that the child care staff member (CCSM) in the classroom was aware of the situation because she told Child B to stop punching Child A. Child A's Mother was not sure of the CCSM's name. She was not aware of what led up to Child B punching Child A. She denied there being any issues with Appletree prior to this incident. Child A and C's Mother stated they only attend before and after school care and school lets out at 3:05pm and she typically picks up between 5:30pm and 5:45pm. She was unsure when they arrive to Appletree because she was not sure how long the bus ride is. Once she picked up Child A and Child C on 5/18/2021, she observed marks on Child A and was told by Child A and Child that Child B had punched Child A multiple times. She went up to the front of the building and spoke to program director Dawn Ulberg and assistant director Ashely Cariello about the situation. Child A's Mother identified that both Ms. Ulberg and Ms. Cariello were not aware of any situations that occurred with 3 Child A and Child B on this day. She stated that the center staff appeared concerned and she felt like they handled it appropriately. Child A's Mother stated the center kept her well informed on the follow up to the incident and she is still very happy with the center. On 05/20/2021 Child A's Mother sent photos of Child A by text message. I observed the photos. There appeared to be some pinkness on Child A's forehead. I was unable to see any puffiness or bruising on the photos. A photo of Child A's leg was also sent and a small scratch on his shin was observed. On 5/25/2021 I completed an unannounced onsite inspection at the facility and spoke to Ms. Ulberg and CCSM's Najilah Word and Gabrielle Bauer. Ms. Ulberg identified Child A and Child B were in the Andre classroom for after school care and there were no documented incidents that occurred. Towards the end of the day the Andre classroom transitioned into the Pratt classroom for a short period of time. There was an incident where Child B was observed shoving Child A while in line. Ms. Ulberg believed this was handled in the moment. The classroom then transitioned to the Terance classroom as the numbers at the end of the day continued to go down. Child A and Child B transitioned to the Terance classroom at around 5:20pm and Child A and Child C were picked up by their mother at 5:32pm. During the time of these transitions there were no more documented incidents between Child A and Child B. I spoke to CCSM Najilah Word who is the Pratt assistant. She was working in the Pratt classroom alone during this time. She stated that she believed Child A and Child B arrived at the center between 3:45pm and 4:00pm
