On 08/04/2022 I conducted an unannounced on-site inspection to the licensed child care facility. I met face to face with Terri Rohrbacher who said she was the new program director for the facility. She then went on to recount significant internal challenges due to staffing changes. The previous program director, Pam VanHoven, abruptly left her position on 07/27/2022 which started the internal issues. Following this announcement, several staff became upset with decision that were made by Tim Quinlan, superintendent and the board of the school. This resulted in Elizabeth VanKoevering, child care staff member, (CCSM) to send an email to all the child care parents expressing her displeasure with the center. This was done without knowledge or approval of the licensee or board. She and several other CCSM's then chose to approach Mr. Quinlan on 07/27/2022 and quit. She then went to her classroom and began packing it up. Mr. QUinllin found her in the classroom and they spoke. Ms. Rohrbacher said she was not present when this took place. She was aware of the hurt feelings and disagreements among staff. She does not have any concerns about Mr. Quinlan or his interactions with staff. Ms. VanKoevering and the other staff have not been rehired. She denied the facility did not maintain the staff to 2 child ratios at any time. The day the staff walked out they were not scheduled to work. She said they have staffing where all rooms are within the required staff to child ratio. I walked through the facility and did not find any classrooms operating out of ratio. I then met face to face with Angie Wall, CCSM, outside the presence of others. She explained prior to 07/27/2022 she was the co-director with Pam VanHoven. When Ms. VanHoven left due to internal staff issues Ms. Wall chose to step away from the co-director position. She remains employed as a CCSM and is assisting Ms. Rohrbacher in training her in the director position. Ms. Wall was aware of the reported concerns. She said on the day of the incident Ms. VanKoevering and others were not scheduled to work and were present in the center for a party. She chose that time to approach Mr. Quinlan with others and said they were quitting. Mr. Quinlan later found Ms. VanKoevering in her classroom, and they spoke. There was no yelling, argument, or aggressive behavior. She has had no concerns with him or his interaction with staff. I then met face to face with Mr. Quinlan outside the presence of others. He said on 07/27/2022 he was in the process of interviewing candidates for a position at the school when Ms. VanKoevering and other staff stopped him in his office. They had demands and said they were going to quit. He said their approach was not appropriate and he was in the middle of interviews. He explained the extensive staff issues he had been navigating with Pam VanHoven leaving abruptly and staff struggling to adjust to the changes. He said when he completed his interviews, he went to find Ms. Van Koevering and she was in her classroom packing it up while children were sleeping. There were other staff who were working and caring for children in the classroom. He asked to speak with her, and they exchanged words, she did not dialogue with him long and he left. He denied there was any argument, raised voices, or aggressiveness. He expressed frustration with the dissension she had caused among staff with no regard for the impact to families and children. She made assumptions about decisions that were made and was making demands that were inappropriate. On 08/08/2022 I made telephone contact with Ms. VanKoevering. She said she ended her employment at the center on 07/27/2022. She acknowledged she was not working on that day and came to the center for an event and to quit her job. She said she caught Mr. Quinlan in the middle of interviews, and he said he would find her when he was done. He found her in her classroom where she was packing up her things. On 02/05/2021 I spoke with program director, Angie Wall. She said she has not received any concerns regarding care for infants and toddlers from child care staff members or parents and provided a list of child care staff members that work with this age group. Ms. Wall explained they recently dismissed Child A from the center due to a variety of reasons. She stated the center does a flex schedule and parents are required to provide a schedule every Thursday, by the end of the day for scheduling that following week. She stated Child A's Mother emailed Child A's schedule on Sunday demanding that Child A needs to have care provided from 7:00AM until 6:00PM all 3 week. Ms. Wall stated she advised Child A's Mother that the center could not accommodate that due to lack of notice, but they could care for Child A during those times three days out of that week. Ms. Wall stated Child A's Mother replied that Child A will be at the center all week regardless because she had advised Child A's caregiver on Friday that Child A would be at the center every day next week. Child A's Mother continued to send those emails and the center had to get the head of schools, Timothy Quinlan, involved. She stated Mr. Quinlan stated the school does not need parents who are threatening staff members. She stated she asked various staff members if Child A's Mother provided a schedule for Child A and they denied being told this. Ms. Wall stated Child A's Mother never brought up concerns regarding Child A's care. On 02/17/2021 I made an unannounced visit to the center. I spoke with program director, Pamela Van Hoven. She explained she is in and out of the toddler classrooms multiple times each day and she has never seen a staff member mistreat a child. She explained there was a previous investigation where a child care staff member had pinched a child care child and she fired that staff member immediately. She stated other child care staff members have not reported concerns on anyone and if a child was being mistreated, someone would report it. Ms. Van Hoven explained child care staff members take child care children from their parents' arms, but she has never seen this done in an aggressive manner or in a forceful manner. She stated she has never observed staff members raise or snap at children to stop crying. She explained when she takes Child A from Child A's Mother, Child A's Mother will hold Child A for a while and she would ask "Okay, are you ready?" and Child A's Mother would lean Child A towards her, so she believed Child A was ready to be picked up and taken to the classroom for the day. She stated Child A's Mother had never complained about how Child A was picked up and she would never take a child away from a parent that was not ready to. Ms. Van Hoven stated the staff members who have worked with Child A are Kimberly Omans and Christiana Omans. She has never had any complaints with these staff members, and they come to the center dressed appropriately. Child A's Mother told Ms. Van Hoven that she provided Kimberly Omans and Christiana Omans Child A's schedule, however Ms. Van Hoven stated this was not true as these staff members would have reported the schedule and they did not because they never received the schedule for Child A. Ms. Van Hoven explained parents need to have their flex schedule turned in every Thursday in order secure placement for the following week. She stated parents also bring in diapers for their children, however Child A's Mother continues to not bring them in, so the center has been supplying them. She stated there was a time when Child A's Mother had brought Child A into the center completely soaked from a soiled diaper. Staff had to change Child A's clothes and washed them at the center before returning them to Child A's Mother. She explained Child A's Mother is being dismissed from the center due to lack of communication, not showing up for assigned care, not communicating her schedule timely and for being threatening when

On 02/05/2021 I spoke with program director Ms. Wall. When asked about any issues regarding diaper changes, Ms. Wall said she had a parent complain that their child always had soiled diapers when he was picked up. She said this has happened with other children as well and explained that child care staff members were changing diapers after nap time, but some children were not receiving another diaper change before pick up time. Ms. Walls stated she addressed this concern with staff the first time at the last staff meeting, which was less than a month ago. She stated yesterday, she had sent a reminder email to all staff members to remind staff about diaper changes during pick up time. She stated she also went into the toddler classrooms as this seemed to be where most of the concerns were and spoke with the lead caregivers and staff members present. She said she advised those staff members to pass along the message regarding diaper changes as well. Ms. Wall explained towards the end of the day, classrooms will combine with others for staff and child ratio reasons. Staff came up with a plan that when classrooms combine, diapers will be placed into a basket that belong to specific children as a reminder to give every child a diaper change when combining. On 02/17/2021 I made an unannounced visit to the center and spoke with program director Ms. Van Hoven. Ms. Van Hoven reported that staff members change diapers every two hours, but they had one child with a diaper rash. Child B's Father had told staff members about the diaper rash and when she spoke with staff about this, she was told staff had been applying cream to Child B's rash. Ms. Van Hoven stated she spoke again with Child B's Father and they discussed a possible yeast infection for Child B as he does have sensitive skin. Child B's Father asked if staff would be able to let the Child B air dry before putting a diaper back on. She said she spoke with staff and staff would clean Child B and pat his bottom with a towel to eliminate excess moisture. Ms. Van Hoven stated she has not heard from Child B's Father or other staff members that this was still an issue or that it continued to be of concern. I spoke with child care staff member, Lindsey Terbeek. She explained soiled diapers are not ignored and staff change them either every two hours or when they are soiled. She stated diaper changes have never been an issue in the classroom and she has never had concerns with Child A's diaper changes. I spoke with lead child care staff member, Erika Webster. She explained that Child A seemed "scared" to have her diaper changed after a bowel movement. She stated despite this challenge, she would always be changed and there was never a time rj she was left in a soiled diaper. Ms. Webster stated staff generally change diapers every two hours unless soiled. She explained Child A has not had any diaper rashes that she is aware of. On 03/09/2021 I spoke with child care staff member Cassandra Pankratz. She explained she has never seen a diaper rash on Child A that was not been taken care of. She stated all toddlers are changed every two hours and all staff are "good about getting that done every two hours." Ms. Pankratz has no concerns regarding diaper changes and children are changed when they are soiled. She stated she does not recall a time when Child A was sent home in a soiled diaper as all staff change diapers right away when they notice that a diaper is soiled. She stated she has no concerns with any staff members and no concerns with the center. On 03/09/2021 I spoke with program director Ms. Wall. Ms. Wall stated to her knowledge, Child A has never been sent home in a soiled diaper, but she has arrived at the center on several occasions, with a soiled diaper. She stated if Child A's Mother told Ms. Wall and Ms. Van Hoven about concerns of Child A having a fever, then Ms. Wall and Ms. Van Hoven would have told herself and other staff members in that classroom, however no concerns were brought to her attention regarding Child A having a fever. On 03/09/2021 I spoke with program director Ms. Wall. Ms. Wall stated she has no concerns regarding staff addressing fevers for child care children. She stated she does remember a time when Child A had a fever and believes Child A had a high fever so she called Child A's Mother and it took Child A's Mother more than an hour to get to the center to pick up Child A. She explained typically, the center directors will contact parents when a child is ill, but if the child care staff members feel comfortable, then they will reach out to the parents. If a child care child has a high fever, then either Ms. Wall or Ms. Van Hoven will stay with that child until the parents arrive. I requested documentation of any fevers or illness related to Child A. Ms. Wall stated she will email these documents. On 03/09/2021 I spoke with program director, Ms. Van Hoven. She stated there has never been a time where Child A had a fever and it was not reported. She explained typically parents are told they need to pick up within an hour if a child has a fever. She stated there was a time when Child A had a fever and the center contacted Child A's Mother. On 02/17/2021 I made an unannounced visit to the center and spoke with program director Ms. Van Hoven. Ms. Van Hoven explained to her knowledge, all children are fed during lunch and snack times and no one is ever denied food. She explained child care staff members use an app to record meal times, if a parent were to check the app during lunch time, that meal may not be recorded as staff members may log this in later in the day. This may cause confusion or look as if their child did not eat when looking at the app. I spoke with child care staff member, Lindsey Terbeek. Ms. Terbeek explained there was never a time she can recall where Child A was not offered food or denied food. She stated Child A usually ate most of her lunch, but sometimes her lunch would be sent home with a few items still packed. I spoke with child care staff member, Christiana Newton. Ms. Newton explained she has never seen staff deny Child A food as staff make sure all children are offered food. If Child A does not want an item in her lunch box, then staff will leave it in her lunch box. Child A can sometimes be picky and there are times when she does not want food in her lunch box or if an item contains nuts, then it cannot be offered due to the center's no nut policy and this applies to all children. I spoke with lead child care staff member, Erika Webster. Ms. Webster explained sometimes Child A refused food that Child A's Mother had packed. For example, Child A's Mother packed spaghetti in her lunch box and Child A did not want to eat it. However, Child A ate the other items packed in her lunch box such as yogurt, juice, or candy. She explained Child A's Mother packs homemade lunches and sometimes Child A will eat and other times she will not, but she is always offered food at lunch time. She stated there has never been a time where a child care child was not offered food. On 03/10/2021 I spoke with Child C's Mother. She stated she has no concerns with Child C's food intake while at the center. She explained the center uses an app to record daily feedings and this will show how much Child C has eaten. Sometimes he will eat some of his food and sometimes he will eat all of it, but there are no concerns with staff not offering food to him. She stated Child C has never come back from the center overly hungry and he has never said that he has not eaten that day. She explained in his lunch box, if he shows no interest in any prepackaged foods like granola bars, that the center will leave it in the lunch box, however the staff clean the dishes for any unpackaged items like sandwiches. Therefore, it is difficult to gauge how much of something Child C ate. On 03/10/2021 I spoke with Child B's Mother. She stated Child B's Father primarily does the drop off's and pick up's, but she does not have any concerns regarding 13 staff members feeding Child B. She stated Child B is in the infant classroom and consumes solid foods. She explained the center provides snacks, but she provides the lunches. She stated the child care staff members document on the app how much Child B has eaten and it typically reports that he has eaten all of his lunch. She explained she was impressed with the child care staff members back in October when they conducted parent teacher conferences and the staff members had told her that Child B was not eating solid foods like the other children his age were and was a little behind in this. She stated staff members did a great job in partnering with her to help Child B increase his solid food intake. She stated Child B's doctor also wrote a plan for Child B regarding food intake that the staff members followed well.
