I received this complaint through the Child Care Licensing Bureau's on-line complaint system on 05/11/2023. It should be noted that the complaint was self-reported by the center's proposed Licensee Designee, Israil Steen. Ms. Steen also contacted me by telephone to explain that Child A and Child B had undressed themselves in the drop in area of the center. Ms. Steen further explained how the center handled the situation and what safety measures would be taken with Child A and Child B in the future. I attempted to interview Child A's Guardian on 05/12/2023. I left a voice mail requesting a return telephone call. I made an unannounced on-site inspection to the child care center on 05/19/2023 at approximately 1:12 PM. I interviewed proposed Licensee Designee, Israil Steen. Ms. Steen explained the incident occurred on 05/09/2023 at the drop in area of the child care center. Ms. Steen stated that Child A (5-year-old male) and Child B (3- year- old-female) were playing in the dirt during outdoor play time. Child A and Child B were brought back into the child care center to get cleaned up. Child A and Child B begin running around the drop in room area, taking their clothes off. Proposed Program Director, Vanessa Collins-Smith was present and made sure Child A and Child B did not touch each other and remained supervised during this time. Ms. Steen stated she was made aware of the situation and arrived at the drop in area where she witnessed the incident herself. Ms. Steen stated the incident lasted for approximately 15 minutes before Child A and Child B were redressed and deescalated. Ms. Steen stated a safety plan was put into place so that Child A and Child B would be provided with extra supervision. On 06/30/2023, I made an unannounced on-site inspection to the child care center at approximately 3:37 PM. I interviewed proposed Program Director, Vanessa Collins-Smith and transitioning CEO of the YWCA, Lisa Rodriguez. It should be noted that Ms. Rodriguez was interviewed by cell phone during the on-site. Ms. Collins-Smith and Ms. Rodriguez discussed the behavioral challenges associated with Child A. It was explained that the YWCA's behavioral specialist had been consulted and it was decided that if Child Care Staff Members were unable to handle Child A's level of special needs, he would be discharged from the center. I interviewed Child B's Mother, Child C's Mother and Child D, Child E and Child F's Mother on 07/05/2023. Child B's Mother stated she believed the center appropriately handled the incident with her child. There was appropriate supervision and the center informed her of the 3 incident in a timely manner. Child B's Mother stated there have been no new incidents and she is satisfied with the care her child is receiving. Child C's Mother and Child D, Child E and Child F's Mother were satisfied with the care and supervision their children received at the center. I conducted an unannounced on-site inspection on 07/06/2023. I interviewed proposed Program Director, Vanessa Collins-Smith and Senior Director of Improving the Lives of Children, Robert Johnson. Mr. Johnson explained he was not present for the incident between Child A and Child B, as he was hired at the end of June. Mr. Johnson was made aware of the incident and will make sure the center follows through with providing extra supervision for Child A and Child B for the duration of their enrollment. Ms. Collins-Smith explained she was working directly with Child A and Child B on 05/09/2023. She stated Child A and Child B were brought inside of the center from the outdoor play area to get cleaned up after they had been playing in the dirt. Child A and Child B undressed themselves and ran around the drop in area of the center. She made sure Child A and Child B did not touch each other. There have been no other incidents with Child B. Ms. Collins-Smith explained there have been on-going issues with Child A but the center has handled these issues to the best of their abilit

I made an unannounced on-site inspection to the center on 04/26/2023 at approximately 11:16 AM. I interviewed Israil Steen, proposed Licensee Designee. Ms. Steen stated the food service van was used to transport Child A (11-year-old male). Child B (13-year-old male) and Child C (4-year-old male) on two occasions. Child A, Child B and Child C are siblings. She stated that two of the children needed to be transported to school and one of the children needed to be transported to the YWCA's children center in the downtown location. Ms. Steen explained Child A, Child B and Child C had been in the drop-in area of the center the night before and needed transportation in the morning. On the two occasions the children were transported by the center as Child A, Child B and Child C's Mother was unable to provide transportation. She explained that she wanted to assist the family in getting to school. Transportation is usually not provided to the children in care. I inspected the food service van that was parked outside of the center on that same date as my onsite inspection. The van was a seven-passenger minivan. It contained the proper safety belt restraints. I observed the proper insurance documents for the vehicle. The van did not contain a first aid kit or three bidirectional emergency reflective triangles. Ms. Steen stated the van would not be used again for transporting children. I interviewed Child Care Staff Member 1(CCSM1), on 06/13/2023. CCSM1 was the driver for the YWCA downtown location. He confirmed that he transported Child A, Child B and Child C on two occasions and that the van was not used to transport any other children in care. I have reviewed CCSM1's file during the renewal inspection that I conducted at the YWCA's downtown location on 03/16/2023. I found his file to have all the appropriate documentation as the transporter for the YWCA's Children's Center. On that same date, I interviewed Child A, Child B and Child C's Mother. She stated that she has given permission for the center to transport her children. The center helped her get her children to school on two occasions. She did not have any concerns.

, contacted me through e-mail on explained "Se received a new investigation for YWCA Edison and wanted to set up a date and time to conduct a joint unannounced on-site inspection, had already interviewed Child A's Mother and stated she would share the interview notes with me. I later that day and told her that I could meet her at the centere omn 0a1il/2e5d/2W0223 a round 1P M. I conducted on unannounced on-site inspection at the center with on 01/25/2023 at approximately 1PM. I interviewed Program fRre!1oR,!ynnn VanDyk. Child Care Staff Member (CCSM) 1, CCSM 2, CCSM 3 and CCSM 4. Program Director, LynAnn VanDyk, stated there have recently been times when she had to fill in as a child care staff member in a classroom in order to maintain ratios. There is no concern with the lack of supervision due to staff shortages. She stated they are currently hiring additional CCSMs. CCSM 1 stated that there have been some issues with some of the children in care trying to leave their individual classrooms. CCSM 1 stated that none of the children have been successful in getting out of their class, as child care staff members are always able to catch the children before they are able to leave the classrooms. CCSM 2 stated that she was under the impression the center was fully staffed. She explained there have been times when one of the classrooms has been out of ratio. If that is the case, a floater will be assigned to that classroom to maintain ratios. CCSM 2 did not have any concerns with supervision. There have been times when the children in care have attempted to leave the classrooms and either go into other classrooms, into the common space area or outside to the courtyard. The staff will intervene before the children in care can get out of their assigned space, however there is a concern with the way the half door leading to the common space is so easy for the children in care to open. This issue has been brought to the attention of Program Director, LynAnn VanDyl however there have not been any steps made to resolve the issue. CCSM 3 stated she does not believe there are any concerns with staffing or with supervision. CCSM 3 is a floater for the center. On that same day, Child B's Guardian, Child C's Mother and Child D's Mother were interviewed over the telephone. Neither parent had concerns with staffing shortages or supervision at the child care center. Child E's Mother was also called but she did not answer the telephone. A telephone message was left requesting a return call. I received an email from on 02/10/2023. report regarding theMen PerwMasa tMtac ed. I reviewed report on 2023. Including in the report were the interviews conducted with Child A's Mother and Child E's Mother. Child A's Mother stated that she did have some concerns regarding supervision and staff shortages at the center. Child A's Mother stated there was one occasion that a child in care was out of their classroom and was trying to open the door leading to the foyer. She stated there was a CCSM in close proximity to the child but found it inappropriate for the child to be attempting to open the door. Child E's Mother did not have any concerns with supervision and staff shortages. did not find that child abuse/ neglect occurred during this investigation.

I emailed later CI that day and told her that I could meet her at the center on 01/25/2023 at around 1P M. I conducted on unannounced on-site inspection at the center with on 01/25/2023 at approximately 1PM. I interviewed "rogram= erector, LynAnn VanDyk, Child Care Staff Member 1, CCSM 2, and CCSM 3. Program Director, LynAnn VanDyk, stated that CCSM 1 started working at the center on 06/10/2022. There have been no complaints or disciplinary action against CCSM 1. Ms. VanDyk did not have any concerns that CCSM 1 was using inappropriate discipline with any of the children in the room. CCSM 1 is the lead teacher for the Outstanding Owls Room. The age range in that room is infant to 3 years of age. CCSM 1 denied the allegation that she used any form of physical discipline on Child A. CCSM 1 stated that Child A will often claim that one of the other children the classroom is hitting her, but it is usually Child A who is hitting the other children. The only form of discipline used in the child care center is redirecting. There is no form of physical discipline nor time outs being used. CCSM 2, denied the allegation that CCSM 1 used physical discipline on Child A or on any of the other children in the classroom. CCSM 2 stated no parent has ever complained about CCSM 1 and that she had no concerns. CCSM 3 works in the center as a floater. CCSM 3 stated she has no concerns that CCSM 1 or any other CCSM in the center uses inappropriate discipline. CCSM 3 stated that there have been no complaints from parents regarding CCSM 1. On that same date, Child B's Guardian, Child C's Mother and Child D's Mother were interviewed over the telephone. Child B's Guardian, Child C's Mother and Child D's Mother did not have any concerns that CCSM 1 or any other CCSM was using physical discipline on the children in care. I received a report from on 02/10/2023. Including in the report were in interviews from Child A's Mother and Child E's Mother. Child A's Mother stated that Child A ) told her that CCSM 1 had hit her on the Child A only had child related injuries located on her shins. There were no injuries on her . Child A's Mother did not have any other concerns or information regarding this allegation. Child A is still attending the center. Child E's Mother did not have any concerns that CCSM 1 or any other CCSM has used physical discipline on the children in care. 5 advised that her investigation resulted in no findings and that she would "ec ing her case.

I interviewed Senior Director of Improving Lives of Children, Patricia Ryan over the telephone on 02/24/2022. Ms. Ryan stated that she did not observe child care staff member, Clarisse Kaulismbuko, fall asleep while holding a sleeping infant. Ms. Ryan stated that Human Resources Manager, Gloria Wilkerson, observed the incident and reported the incident to Program Director, Melissa Goldsberry. Ms. Goldsberry spoke to Ms. Kaulismbuko regarding the incident. It should be noted there are five classrooms in the main child care area. The age range in these classrooms is birth to three years of age. There is also a 24-hour- area separate from the main classrooms. This area is used for drop-in 24-hour care. I made an on-site inspection at the facility on 03/01/2022 at approximately 9:45 AM. interviewed Senior Director of Improving Lives of Children, Patricia Ryan, Program Director, Melissa Goldsberry, Program Coordinator, Bianca Young and Vice President of Programs, Melissa Dunn. None of the above individuals observed child care staff member, Clarisse Kaulismbuko, asleep while holding an infant. Ms. Ryan stated the incident was reported by Human Resources Manager, Gloria Wilkerson. Ms. Wilkerson observed the incident and reported it to Program Director, Melissa Goldsberry. Ms. Goldsberry stated that once the incident with child care staff member, Clarisse Kaulismbuko, was brought to her attention, she addressed the concerns with Kaulismbuko. 3 During the unannounced on-site inspection, I interviewed child care staff member, Zaria Taylor. Ms. Taylor is also Child A's Mother. Ms. Taylor stated she did not witness Clarisse Kaulsimbuko, asleep while holding an infant. However, Ms. Taylor stated that she was aware that the incident occurred. On 04/12/2022 I interviewed child care staff member, Ashley Taylor and former child care staff member, Genesis Griffen over the telephone. Ms. Taylor stated that she is assigned to the Outstanding Owls room. Ms. Taylor stated that a few child care staff members have been caught sleeping while holding infants. She stated that Human Resources Manager, Gloria Wilkerson, observed Clarisse Kaulismbuko sleeping while holding a sleeping infant. Ms. Taylor could not remember which infant Ms. Kaulismbuko was holding but did know that the child was under a year old. Ms. Griffen stated she left employment with the facility on 03/11/2022. She was assigned to the Fearless Foxes room. Ms. Griffen confirmed that child care staff member, Clarisse Kaulismbuko fell asleep while holding an infant. Ms. Griffen has observed Ms. Kaulismbuko asleep while holding an infant on more than one occasion. Ms. Griffen could not confirm how many times Ms. Kaulismbuko has fallen asleep with an infant. She stated that when Ms. Kaulismbuko falls asleep while holding an infant, other child care staff members wake her up. Ms. Griffen explained that Human Resources Manager, Gloria Wilkerson, observed Clarisse Kaulismbuko asleep while holding an infant and reported the incident to Program Director, Melissa Goldsberry. I called child care staff member, Sadia Faisal. Ms. Faisal was not available, and I left a telephone message requesting a return telephone call. I have not heard from her to date. I interviewed Program Coordinator, Bianca Young, Child B's Mother, Child C's Mother and Child D's Father over the telephone on 04/15/2022. Ms. Young stated she was aware that Clarisse Kaulismbuko fell asleep while holding an infant. She did not observe the incident herself but knew that Human Resources Manager, Gloria Wilkerson, witnessed and reported the incident to Program Director, Melissa Goldsberry. Child B's Mother, Child C's Mother and Child D's Mother did not have any concerns. I left a telephone message with Human Services Manager, Gloria Wilkerson, on that same date requesting a return telephone call. On 04/19/2022 I called Human Services Manager, Gloria Wilkerson. I interviewed Patricia Ryan, on 02/24/2022. Ms. Ryan stated that it was reported to her that Child A (17-month-old, female) was not supervised properly and as a result she went into the bathroom and took the spray bottle which contains a bleach solution. Ms. Ryan stated that it was unclear how long the Child A had the spray bottle before a child care staff member took it away from her. Ms. Ryan believed it was only a minute or two. Child A did was not injured from the incident. On 03/01/2022 during the on-site inspection I interviewed Senior Director of Improving Lives of Children, Patricia Ryan, Program Director, Melissa Goldsberry, Program Coordinator, Bianca Young, Vice President of Programs, Meagan Dunn and child care staff member, Zaria Taylor. Ms. Ryan, Ms. Goldsberry, Ms. Young and Ms. Dunn did not witness the incident and did not have any firsthand knowledge; however, they were all aware that the incident occurred. Child care staff member, Zaria Taylor, stated that child care staff members Ashley Taylor and Genesis Griffen saw Child A with the spray bottle containing bleach/ water solution and in turn reported the incident to her. Ms. Taylor stated she was not sure where the child care staff members who were supposed to be in that room were, but she was concerned that Child A was able to get a spray bottle with a bleach/water solution. Ms. Taylor was not sure how long Child A had the spray bottle. She stated that Child A was not injured from the incident. On 04/12/2022 I interviewed child care staff member Ashley Taylor. Ms. Taylor reported that she is assigned to the Outstanding Owls room. She explained her room is adjacent to the Responsible Rabbits room and they share a bathroom. Ms. Taylor stated approximately two or three months ago she observed Child A in the bathroom unsupervised with a spray bottle that contained a bleach/ water solution. The spray bottle was taken away from Child A and she believed Child A only had the spray bottle for a couple of minutes. Ms. Taylor stated that Child A was not injured from the incident. It did not appear that Child A ingested any of the bleach water. I interviewed former child care staff member, Genesis Griffen. Ms. Griffen stated she used to work in the Fearless Foxes room. She was present when Child A was found with a spray bottle with a bleach/ water solution in it. Ms. Griffen stated that Ashley Taylor found Child A in the bathroom with the spray bottle, but she was present to witness the incident. Ms. Griffen stated she was unsure where the child care staff member was who was supposed to be watching Child A. Ms. Griffen stated that Child A was not injured from the incident. 7 I attempted to call child care staff member, Sadia Faisal, but she was unavailable. I left a voice mail but have not heard from her to date. On 04/15/2022 I interviewed Child B's Mother, Child C's Mother and Child D's Father over the telephone. They did not have any concerns. I interviewed Human Resources Manager, Gloria Wilkerson and Lead Caregiver, Imani Systema. Ms. Systema stated she did not witness the incident in which Child A found a spray bottle with bleach/ water solution. Ms. Wilkerson also did not have any direct knowledge of the incident.

Child A was not being properly supervised and was able to get a spray bottle with bleach/ water solution. The spray bottle was within reach of Child A, putting Child A at risk.
