I made an unscheduled onsite inspection at the Learning Village child care center on 9/9/2020. Program directors Ann Brown and Nesha Nesbitt informed me that Child A began their program on 9/2/2020. Child A had difficulty adjusting so he was held and coddled much of the time. Child A arrived at the center without shoes on 9/2/2020 and ill-fitting shoes on 9/3/2020. Child A is approximately 16 months old and fell a lot in those two days. Child A was dropped off by his grandmother on 9/3/2020 at 11:45 a.m. Ms. Nesbitt observed a small red mark on Child A's face when he arrived. Ms. Nesbitt did not inquire about the red mark as she thought it was possibly from sleeping. A bruise in that area appeared later in the day. While walking barefoot in the courtyard on 9/3/2020, Child A fell and scraped his pinky toe. It is the child care center's policy to provide parents with an accident report for them to sign when they arrive to pick up their child. Child care staff members forgot to inform Child A's Mother of the injury or provide her a copy of the accident report when she arrived to pick him up on 9/3/2020. Child care staff member Abagail Tuttle provided care for Child A in the toddler room on 9/3/2020. Ms. Tuttle did not observe any injuries on Child A when he arrived that day. He tripped and scraped his pinkie toe while walking barefoot in the courtyard. Ms. Tuttle brought Child A inside after the fall to stop the bleeding and put on a Band-Aid. The Band-Aid later fell off and she had to replace it. Both times, Ms. Tuttle recalls seeing Child A's toenail intact. Ms. Tuttle noticed a bruise on Child A's face later in the day. She did not witness anything that day that would have caused that injury. Child A's face never hit the ground when he fell in the courtyard. Ms. Tuttle was not present when Child A's Mother came to pick Child A up. She is not sure if Child A's Mother was informed of the injury. Ms. Tuttle completed the accident report on 9/4/2020. I spoke on the telephone with Child A's Mother on 10/6/2020. Child A's Mother informed me that child care staff did not mention anything about the injuries when she picked Child A up on 9/3/2020. Child A's Mother did not notice the injuries until after she arrived home. She tried calling the child care center, however, they had already closed for the day. Child A's Mother arrived at the center the following day to discuss the injuries. The child care center acknowledged that staff forgot to provide her with an accident report. They did not have an explanation for the bruise under his right eye. Child A's Mother denied that the injury occurred at either her house or when Child A was with his Grandmother. I spoke on the telephone with Child B's Mother on 9/28/2020. Child B's Mother did not report any supervision concerns. Child B has received minor injuries while in 3 their care. She sometimes receives accident reports notifying her of the injuries, other times Child B's Mother has to ask staff about the injuries the following day. I spoke on the telephone with Child C and Child D's Mother on 9/28/2020. Child C and Child D's Mother did not report any concerns regarding the care and supervision her children receive and has been provided accident reports for past injuries.

On 9/3/2020, Child A had a bruise under his right eye and his pinky toe was bloody and the nail was missing. Child A's Mother was not notified of the injuries by child care staff members.

Appropriate care and supervision were provided on 9/3/2020. Child A fell and scraped his toe while playing outside. The child was brought inside to treat the injury. The cause of injury to Child A's face is unknown. Program director Nesha Nesbitt reported that Child A arrived at the center with a red mark on his face that later turned into a bruise. Child A's Mother stated that Child A was injured while attending the child care center. I
