I made an unscheduled onsite inspection at Small World Learning Center on 2/4/2021. I interviewed program director Chelsea Beebe and child care staff member Paige Strate. Ms. Strate assists the director when needed. Ms. Strate has never witnessed a child drink from another child's cup. Ms. Beebe informed me that children in the wobbler room have attempted to take a cup that did not belong to them. If successful, staff take the cup away and either wash, rinse and sanitize the cup or provide the child a new cup. I spoke on the telephone with toddler lead caregiver Molly Berry. Ms. Berry informed me that she was employed at Small World Learning Center for approximately two years. She just recently discontinued employment there. Ms. Berry stated that children did not drink out of each other's cups. If a child grabbed the wrong cup, staff would intervene and take the cup away. The cup would then be cleaned prior to returning to the child. I interviewed Child A's Mother and Child B and Child C's Mother on 2/17/2021 and Child D and Child E's Mother on 2/18/2021. None of the parents have ever witnessed children drinking out of the wrong cup.

Paige Strate informed me that staff always have their eyes on the children. They may talk with one another but are often separated and engaged. Four staff are typically positioned in each corner of the outdoor play area. Chelsea Beebe stated that staff are engaged with children and separated from one another. The center requires that only the lead caregiver (or their replacement) have their phone with them while outside. Staff cannot be within 6 ft. of one another for more than 15 minutes. 3 Molly Berry said that there is typically four or five staff members outside with the wobbler and toddler children. Staff sometimes talk but they are typically positioned at each corner of the playground and circulate the area. None of the parents expressed any concerns regarding the care and supervision provided at the child care center. Child A's Mother and Child B and Child C's Mother informed me that child care staff members pay attention and are engaged with the children.
Children drink from each other's cups. Child care staff congregate while children are playing outside. They are not engaged or supervising the children. Child care staff yell at children. Child care staff do not clean items used by sick children. Child care staff and children are not excluded from the center due to illness. Temperatures are taken outside. The thermometer does not work in cold weather. Child care staff do not wash toys. They just spray disinfectant around the room. Toys that have been in children's mouths are left out to be used by other children.

Paige Strate, Chelsea Beebe and Molly Berry all informed me that child care staff members may raise their voice to get a child's attention if there is a safety concern, but none of them had ever heard staff yell at children. None of the parents have ever heard staff yell at children. Child A's Mother described staff as both caring and compassionate.

None of the parents expressed concern regarding the cleanliness of the center. Child A's Mother informed me that it is the center's policy to isolate sick children until picked up and that the room is cleaned. She has faith that they are following through with this protocol as everyone is staying healthy. Child B and Child C's Mother informed me that she does not have first-hand knowledge that staff are cleaning toys and equipment used by sick children but assume this is being done.

Paige Strate informed me that children's temperatures are taken by Ms. Beebe in the mornings. She has no knowledge of the thermometer not working. I was provided a copy of the center's exclusion policy and COVID response plan. The center's exclusion policy and COVID response plan state that children and staff cannot attend the child care center if they have any COVID related symptoms or a fever over 100.4 F. Chelsea Beebe stated that daily temperature checks are done on both staff and children every morning. No one is permitted in the building if they have a temperature over 100.4 F. If a child becomes ill, they are sent home and must be symptom free for 24 hours. Staff with COVID symptoms were tested and stayed home pending results. Ms. Beebe takes children's temperature outside. It sometimes reads low if it is really cold outside. When this happens, the child will be brought inside the building where another temperature check will be administered under the arm. 5 Molly Berry informed me that both staff and children submit to temperature screening daily. The thermometer used outside will sometimes register low. When this happens, children are brought into the entryway where another temperature check will be administered under the arm. Sick children are isolated and sent home. Staff that are not feeling well must attempt to find someone to cover their shift. The director will cover the shift if a replacement cannot be found. If staff have COVID related symptoms or have been exposed to COVID, they must have a negative test prior to returning to the center. All parents stated that the center follows their exclusion policy and send children home if they have a fever, diarrhea etc. Child A's Mother informed me that staff take children's temperature upon arrival. If the temperature is not accurate, staff will take the children inside the building to get a better read. Child B and Child C's Mother stated that she takes her children's temperature at home. The thermometer used at the center appears to work properly as it reads the same temperature as the home thermometer.

Paige Strate, Chelsea Beebe and Molly Berry informed me that there is a cleaning checklist that staff use to clean and sanitize toys daily. Classrooms are sanitized at the end of each day and a deep clean is conducted every Friday. Toys that have been in children's mouths are placed in a dirty bin and are not returned until cleaned and sanitized. None of the parents expressed concern regarding the cleanliness of the child care center and were provided a copy of the center's cleaning/sanitizing protocol. Child B A and Child C's Mother informed me that she has observed staff cleaning toys at the end of the day.
