I received a complaint on 06/21/2021 that Child A was playing with some crayons and started throwing them. Child Care Staff Member, Abigail E E Rabin, sat behind Child A and told several times that$ should not throw crayons. Ms. Rabin put Child A onto er lap, while restraining to sit down. Child A struggled then successfully got out of Ms. Rabin's grasp. Ms. abin attempted to grab Child A's midsection but was only able to grab the back of shirt. Child A continued to try and get awa from Ms. Rabin and ended up falling face down causing lip to bleed and to get stained with blood. shirt On 06/21/2021 3 I e-mailed Program Director, Kathy McNinch on that same day to schedule the onsite. Ms. McNinch expressed some concern that the appointment was a week away when there were allegations of abuse. Program Director. Kathy McNinch, stated she did not see the incident with Child A, as she was in her office. Ms. McNinch stated she saw Child A after the incident but did not see anything concerning about behavior or appearance. Ms. McNinch was told that Child A fell face first, injuring $ lip. Assistant Program Director. Heidi CI Albertson completed an incident/accident report and Child A's Mother signed the report on that same day. Child A has been back to the center since the incident. Ms. "McNin0ch sta"ted that she was told that Child A slipped on a piece of paper in the bike room and fell. Ms. McNinch stated the Infant/ Toddler Lead Teacher in the room, Dashaya ard and two volunteers from the were also present with Child Care Staff Member, Abigail Rabin occurred. Abigail Rabin was then interviewed regarding the allegations. Ms. Rabin explained that she is going to . Ms. Rabin stated that her background check has not been cleared yet, as they are waiting for the clearance from Ms. Rabin is never left unsupervised with any of the children =. in care. Ms. Rabin explained that she was on the floor with some of the children and the were coloring on construction paper. She observed Child A putting crayons in mouth and she told no. While Ms. Rabin was trying to get the crayons out o E Child A's hands, slipped on a piece of paper and fell halfway onto her lap. She explained that the ront part of Child A's body landed on the floor where hit lip and the back half body landed in her lap. Ms. Rabin stated she believed th Child A had bit lip. Ms. Ward took Child A into the Infant/Toddler Room, cleaned ■ up and changed shirt. She stated that Child A cried when fell but quickly ■ stopped crying. She stated that the volunteers from the "E were also present. Ms. Rabin stated a she does remember she pulled on Child A's shirt or not. She has accidently pulled on a child's shirt if she was reaching for them. Infant/Toddler Lead Teacher Dashaya Ward was then interviewed regarding the allegations. Child A slipped on a piece of paper and fell. Ms. Ward stated she picked Child A up, took ■ to the changing table located in the Infant/Toddler Room and administered first aid on U .
