On September 13, 2022, I made telephone contact with Child A's mother. She has observed eight to nine children in care regularly when Jamie Lynch is the only staff member. She realizes Ms. Lynch received a variance to have seven children and one of the children is Ms. Lynch's son who is 18 months old, but if Ms. Lynch's husband is there, he is not the one providing care to their son. Child A's mother said she has removed Child A from Ms. Lynch's care. On September 15, 2022, I completed an unannounced onsite inspection at Ms. Lynch's child care. At the time of my arrival Ms. Lynch was caring for ten children. One of the children was her 18-month-old son. Her husband was home and took their son from the child care area while I was present. Ms. Lynch reported that she only has seven child care children in care, but has two neighbor children that she does not normally have, but is helping her neighbor out. The neighbor's children are both three years old. She reported she does not normally go over her capacity of seven children. I asked Ms. Lynch for her attendance records to review, and she reported that she could not find them. I made telephone contact with Child B's mother on October 10, 2022. She reported she generally sees a total of five to six children in care. On October 11, 2022, I made telephone contact with Child C and Child D's mother and Child E, Child F, and Child G's mother. Child C and Child D's mother said there are normally six to seven children in care counting Ms. Lynch's son, but her husband is usually there as well. Child E, Child F and Child G's mother said she generally only sees one or two other children in care besides her children. I made telephone contact with Child H's mother and Child I's mother on October 12, 2022. Child H's mother said her husband drops off in the morning, but when she picks up there are usually only three children in care. Child I's mother said she normally sees five to six children in care.

Child A's mother reported that while picking her child up one day she observed an infant sleeping on the floor in the living room on a boppy pillow. Child A's mother was texting with Jamie Lynch that evening to address some concerns. While texting Ms. Lynch addressed the child sleeping on the floor, stating "Child H was on a blanket when you came in because we were reading, and he fell sleep (I do tummy time with the babies). No big kids were in the living room and the gate was shut. He wasn't left alone and was with me and there was nothing that could hurt him. I never left him unattended." During my interview with Ms. Lynch she stated that Child H is five months old, and he fell asleep during story time. She reported he has only slept on the boppy pillow one other time because they were outside on the deck, and he fell asleep. Child G's mother stated that she has not seen Child G sleeping on the floor, but she has seen Child G sleeping on the boppy pillow in the crib. She reported that Ms. Lynch uses the boppy pillow in the crib when Child G is congested because it helps him sleep. Child B's mother, Child C and D's mother, Child E, Child F, and Child G's mother, and Child I's mother denied ever seeing an infant sleeping on a boppy pillow.

Jamie Lynch reported that she washes the sheets and blankets every Friday, and she has two cribs so that the infants have their own sleeping space. She denied having children share the cribs. I observed two cribs in the living room while onsite. Child A's mother believes the infants share the cribs and sheets are not changed in between uses. Child B's mother reported she has a pillow and blanket that she takes on Mondays and brings home to wash on Fridays. Child C and Child D's mother said she is not sure what happens with the sheets and blankets, but she believes Ms. Lynch washes them. Child E, Child F, and Child G's mother said she has not seen children using the same sheets and blankets. Child H's mother said she has only ever seen her son in one of the cribs and Ms. Lynch told her she washes the sheet every Friday. Child I's mother said she believes Ms. Lynch washes the sheets and blankets.

Child C and Child D's mother and Child E, Child F, and Child G's mother denied seeing any bottle propping. Child H's mother reported that Ms. Lynch has told her that she has propped Child H's bottle with a boppy pillow, but she was not aware it was happening regularly.
