It should be noted that on 12/04/2020 I spoke with licensee designee, Linda Owens. Ms. Owens reported that the Child Development Center (CDC) Borgess would be closing their license effective 12/31/2020 and were in the process of transitioning staff and children to other CDC sites. She also indicated that program director, Malia DePriest, was leaving her position. On 12/08/2020 I received an incident report from program director, Ms. DePriest, that Child A fell down the steps outside, resulting in her tooth being knocked out. 2 According to the incident report, the incident occurred around 3:30PM on 12/07/2020 during outdoor play. The Tikes classroom of children were on their way to go outside. Child care staff member, Klaudia Ignaciuk, was trying to get the children out the door leading directly outside, and had one child in her arms, and was trying to grab Child A but Child A kept walking and fell down the porch steps. Ms. Ignaciuk immediately set the other child down and jumped off the steps after Child A. Child A got up and continued to go play. Ms. Ignaciuk noticed Child A's mouth bleeding and tried to control the blood. Child A was given a frozen teether and the tooth was found on the steps. Child A's mother was called at approximately 3:40PM after first aid was given,. On 12/09/2020 I conducted an unannounced onsite inspection to the CDC Borgess. I observed each classroom of children eating lunch, with the exception of the Caterpillars classroom (infants), where some of the children were sleeping in their crib, while others were still being fed. I observed Child A present in the Tikes classroom. I obtained child information cards, attendance records, the supervision policy for the center, and a health report of the incident signed by the parent. I interviewed program director, Ms. DePriest, licensee designee, Ms. Owens, and child care staff members, Ms. Ignaciuk, Katy Celinske, and Michaela Brown. Ms. DePriest confirmed with me that she was leaving her position and her last day would be that Friday, 12/11/2020, and that the center still intended to close their license at the end of the month. She was not present on 12/07/2020 when the incident occurred as she had to step out of the center briefly for an emergency. Ms. DePriest verified that Child A is in the Tikes classroom (toddler) and on the day/time of the incident Ms. Ignaciuk had four children present with her. She reported that she did complete a health report and it was signed by Child A's Mother. She also told Child A's Mother that the incident was reported to child care licensing and the licensee designee, Ms. Owens. Ms. Owens contact information was given to Child A's Mother as well. Ms. DePriest did not have concerns with Ms. Ignaciuk and her ability to care and supervise children. Ms. Owens was not present when the incident occurred as she works from the main office for the CDC's. She was made aware of the incident and did not report concerns with any of the child care staff at CDC Borgess. Ms. Owens indicated Ms. Ignaciuk would be working as the interim program director for the remaining few weeks the center was open. Ms. Ignaciuk has been employed with the center approximately a year and works in the Tikes classroom. She confirmed the information on the incident report. She reported that the incident happened during outdoor play time. Ms. Ignaciuk indicated they usually bring the children out to the patio and the children sit down. Each child is then lifted down the steps and placed on the ground. Some of the older toddlers are able to climb without help but the younger children are always lifted. At the time the incident occurred, Ms. Ignaciuk had one child in her arms to put down in the play area and Child A was by her side. She went to grab Child A but Child A kept walking 3 and fell down the step hitting her face. Ms. Ignaciuk immediately jumped down the steps to make sure Child A was ok.
