I completed an onsite inspection on 1/31/2023. I interviewed program director Tamra Bronson and child care staff member 1 regarding the bottle labeling. Ms. Bronson reported that the parents bring formula containers in multi-day supply and the child care staff members write down the child's name on the lid of the container. Upon inspection, there were four multi-day containers in the infant room. Two were labeled with the child's first name only, and two had the first initial and last name of the child. All containers were missing the date of opening. Child care staff member 1 reported that the bottles are labeled with the child's first and last name, but they did not realize that the date of opening had to be on the multi-day containers. On 2/1/2023 I spoke to Child A's mother who has a child in the infant classroom. She reported that her child's bottle is labeled with Child A's name and the center asked her to do this but sometimes it comes off and she has to put a new label on it. She reported that she will write her child's first name on the formula container when she drops those off as well.

During my onsite inspection I observed the preschool room and interviewed child care staff member 2 and child care staff member 3. The room was separated into two distinct use areas. On one side, child care staff member 2 reported that she has five children in her care ages 2-3 years old. Ms. Bronson accompanied me to the room and recalled that two of the children were under 30 months of age. Ms. Bronson was reminded that when combining age groups, the ratio in the room changes based on the youngest member of the group, making the ratio in this room 1:4, not 1:8. Child care staff member 2 reported that her classroom is frequently out of ratio. Child care staff member three had two children This side of the room was within ratio. Child care staff member 3 reported that her side of the room is typically within ratio, but she knows that the other side of the room is usually out of ratio due to children moving over from the infant room at age 24 months instead of 30. Child A's mother reported that there are many times when it appears as though the child care staff members are having a hard time with the amount of children they are caring for and it can be very loud in the center. She has a child in the infant room and preschool room. She has never had any safety concerns or issues and is happy with the care her children receive. On 2/3/23 I spoke to Child B's mother and Child C's mother. Child B's mother reported that they have not noticed too many kids in the preschool classroom, but she does not stay long and look around because the child care staff members always have Child B ready to go when she arrives for pick up. When she drops off there are not many children in attendance yet. Child C's mother reported that she does not know how many children are supposed to be in each room, but she has never noticed there being a lot of kids at once. She said it can be loud from older children at times, but she has no concerns for her child's safety.

I completed an onsite inspection on 3/16/2021 and completed interviews with the child care staff in the infant room, Kendra Towers and Stephanie Marshall as well as the Program Director, Jenilee Griffiths. Ms. Towers reported that last week Child A's mother believed that they had not changed Child A's diapers until 3:24 p.m. when he woke up from a nap. Ms. Towers said they had informed Child A's mother that they were concerned because he had not had a wet diaper from 6:45 a.m. until 3:24 p.m. Child A's mother believed this to be untrue and accused the child care staff of not checking and changing Child A's diaper all day. Ms. Towers provided the daily diaper logs for Child A from the past two weeks and highlighted the day they brought it to Child A's mothers attention that Child A had not soiled his diaper all day. The day was 3/2/21 and there were recordings of checking Child A's diaper every 1.5 to 2 hours and it was documented as dry. Child A had a wet diaper at 6:45 a.m. and then was dry until 3:24 p.m. after Child A had woke up from nap. Ms. Towers said she and Ms. Marshall check the children's diapers every 1.5 to 2 hours and they each share the responsibility by each checking half of the kids, while the other person checks the other half. Ms. Marshall reported that she and Ms. Towers will change a diaper immediately when they notice it has become soiled, or every 2 hours whichever comes first. She provided an example that when a child has defecated it is obvious and they change it immediately, and for urination they routinely check. However, many of the diapers provided by the parents have color changing technology so it makes it easier to see if a child has urinated even a little and they change the diaper right away. Ms. Marshall confirmed that they were concerned on 3/2/21 that Child A had not had a wet diaper throughout the day and thought Child A's mother should know to keep an eye on his fluid intake. Ms. Marshall said that Child A does not drink a lot at snack and lunch time. Program Director Jenilee Griffiths reported that she has never had any concerns with the child care staff members not changing diapers, and has never had to 3 discipline or talk to a child care staff member about this topic. She believes they are checking and changing diapers as needed. She often observes staff throughout the day and have seen them change diapers. On 3/24/2021 I spoke to Child B's mother. She reported that she gets a copy of Child B's diaper log and has never had any concerns related to diapering. She believes that the diapers are changed even more than she changes them at home. On 3/25/2021 I spoke to Child C's mother. She reported she has no concerns regarding diapering of Child C and has never noticed any rashes or problems. She provides the diapers for Child C and feels the amount that she provides is consistent with what Child C uses at home. On 4/27/2021 I spoke to Child D's mother. She reported that Child D is almost ready to transition into the toddler room and does not have as many diapering needs as an infant. She has no concerns regarding the diapering of Child D. Child D has attended since infancy and she feels the child care staff in the infant room always do a great job caring for Child D. I attempted several phone calls with Child A's mother that were not returned before this investigation was concluded.

During my onsite inspection on 3/16/21 I did not observe any ants in the hallway, bathrooms or any part of the facility. Mrs. Griffiths acknowledged that once in a while after a heavy rain they have ants in the center. They do a pesticide treatment and set traps in non-child use areas, where the children have no access to them. Ms. Griffiths reported that they do not have an insect problem at all, and simply take those measures as any facility would. Ms. Marshall reported that she has never seen any ants in the hallways or bathrooms. She and Ms. Towers clean the bathroom in her classroom twice a day, once at nap time and then again at the end of the day. Ms. Towers stated that she has seen one ant in the hallway before after a hard rain and knows that Ms. Griffiths has ant traps in the employee area where children do not have access to them. She does not feel that the center has a pest control issue and maintains that the center is clean and in good condition. Child B's mother reported that she has never seen an ant in the center and part of the reason Child B is enrolled in the center is because it is always clean and well kept. Child C's mother stated that she has not observed any ants in the hallways and said she has never been in the bathrooms. She does not feel that the center has a pest issue and is always happy with how clean and well kept it is. Child D's mother reported that ants are not an issue that she has ever noticed, and she has always been impressed with how clean the facility is.
