On September 1, 2022, Kristen Dill, program director submitted an incident report. Ms. Dill stated that Child A was unattended in the hallway for approximately 1-2 minutes. On September 7, 2022, I completed an unannounced on-site inspection. I individually interviewed program director, Kristen Dill and child care staff member, Hannah Farley. Ms. Dill stated that on August 30, 2022, at approximately 5:00 p.m., as she walked down the hallway to relieve a child care staff member, she was informed by child care staff member Cali Rameriz-Lopez that when she returned from the playground with her classroom, she observed Child A in the hallway unattended and brought her into her classroom. Ms. Dill stated that Ms. Rameriz- Lopez is a child care staff member in Toddler 2 and Child A was enrolled in the Toddler 1 classroom. The Toddler 1 and Toddler 2 classrooms are adjoining classrooms separated by a safety gate. Ms. Dill individually met with Toddler 1 child care staff members Megan Kula and Hannah Farley. Ms. Dill stated that she met with Megan Kula first, as she was the primary caregiver for Child A. When she asked Ms. Kula to explain what happened, Ms. Kula appeared to be "nonchalant" and stated, "I don't know what happened, she got left in the hallway." Ms. Dill told Ms. Kula that this was unacceptable, and she needed to walk her through what happened. Ms. Kula then stated that she and child care staff member Hannah Farley put the six children from the Toddler 1 classroom in wagons to transport them from the gymnasium to the classroom. There were two wagons with three children in each one. When they arrived at the classroom and began to take the children out of the wagon and line them up at the door some of the children ran down the hallway. Ms. Kula and Ms. Farley ran after the children and brought them back to the classroom door. They took the children into the classroom and closed the gate. Ms. Kula stated that as she and Ms. Farley were changing the children's diapers, she observed Child A in the Toddler 2 classroom with child care staff member Cali Rameriz-Lopez. Ms. Rameriz-Lopez returned Child A to the Toddler 1 classroom. Ms. Kula stated that she was surprised to see Child A in the Toddler 2 classroom and said, "Child A how did you end up in there?" Ms. Rameriz- Lopez then told Ms. Kula that Child A was in the hallway. Ms. Kula stated that Child A was unattended for approximately 1-2 minutes, as this was the approximate time they were in the classroom after returning from outside. Ms. Dill stated that child care staff member Hannah Farley told her that she and Ms. Kula transported six children in wagons to the Toddler 1 classroom as they returned from outside. When they arrived at the classroom and took the children out of the wagons some of the children began to run down the hallway. Ms. Farley and Ms. Kula ran after the children and returned them into the classroom. Once inside the classroom Ms. Farley observed Child A in the Toddler 2 classroom with child care 3 staff member Cali Rameriz-Lopez. Ms. Rameriz-Lopez then returned Child A to the Toddler 1 classroom. Ms. Dill stated that Ms. Kula and Ms. Farley both stated that Child A was unattended for approximately 1-2 minutes, as this was the approximate time they were in the classroom after returning from outside. Ms. Dill stated that she informed Child A's Mother of the incident, as she arrived approximately 20 minutes after the incident occurred. Child A was not injured or observed to be in distress, as she was observed to be smiling and playing. Ms. Dill also stated that she met with Ms. Kula on the same evening and terminated her employment effective immediately due to "her lack of regard", as well as other previous issues. She also explained to her that child care staff members should have counted the children as they were walking through the classroom door, as well as counting again once everyone was inside the classroom.
