Child A's Mother did not tell disclose the identityWoRda"re ta"MemMber 1 to Ms. Hunt out of fear that Child Care Staff Member I would try to "get her story straight." During the onsite inspection on May 22, 2023 licensee designee Lisa Hunt stated that Child A was starting to have concerning at the center. Child A is in the ahool class with Child Care Staff Members 2, 3, and 5. Child A was child care staff members. Child A's Mother came into the center and observed Child A in the class. Ms. Hunt stated that due to Child A's Child A's Mother was informed on the morning of May 19, 2023, that Child A was being placed on a and he would be reduced to attending for a half day and if "coninuWe, would be disenrolled. On May 19, 2023, at 5:54 PM Child s Mother texted Ms. Hunt asking if the fee for child care would be reduced if Child A was reduced to half days. Ms. Hunt told Child A's Mother that the fee would be reduced. Child A's Mother asked Ms. Hunt if she needed to give two weeks' notice if she decided to remove Child A from care. Ms. Hunt told Child A's Mother that she would need to give two weeks' notice. If notice was not given Child A's Mother would still be required to pay. At 7:30 PM, Ms. Hunt receive0another text message from Child A's Mother stating that she believes that Child A a child care staff member at the center and that he returning to the center. Ms. Hunt stated called Child A's Mother the following day get re information about the allegation of Child A hild A's Mother wouldn't tell Ms. Hunt who Child A sal because she said that she didn't want the person to 'get their story straight." nt 3 believes that the allegation is being made as a way for Child A's Mother to avoid giving two weeks' notice. On June 14. 2023, a forensic interview of Child A was conducted at the by forensic interviewer with an observed the intervief. by that the interview Child A stated i vas advised during that Child Care Staff Member 1 Child A declined to answer any further questions about this statement and did give any further details. During a second onsite inspection on June 23, 2023, Child Care Staff Member 1 was interviewed, she has worked at the center since October 2021. Child Care Staff Member 1 opens the center at 6:00 AM in the preschool room and remains in the room until 7:50 AM when she drives school-age children to school. Child Care Staff Member 1 returns to the center after dropping children off at school and works as a floater covering breaks. Child Care Staff Member 1 has never been alone with Child A. She believes that Child A is usually dropped off between 8:20 AM and 8:40 AM after she already eft the preschool room. Child Care Staff Member 1 stated that Child A is fulls so she does not need to assist Child A in Child Carhe aStsaf0f ;j assists in with children w o are She of children who are . but she has never ha o elp C ay. Child Care Staff Member 1 stated that she has tried to calm Child A down when he was upset or sat with him at nap time. She

During the onsite inspection on April 19, 2023, Child Care Staff Member 9 stated that licensee designee Lisa Hunt is not the center's program director. She stated that Ms. Hunt has five other centers so she does not work at the center daily. Child Care Staff Member 9 stated that she handles administrative duties at the center in the absence of a program director. Ms. Hunt is conducting interviews to hire a program director. The last program director left her position to go work in a classroom at one of Ms. Hunt's other centers a month ago. Another previous program director resigned in November 2022. Licensee designee Lisa Hunt stated that she should not be listed as a program director because there is no way that she could be present at the center the required six hours per day. She stated that the center's previous program director M was moved to a different location at the beginning of April 2023. is ue return to the center as program director after May 30, 2023, unless a new director is hired. Ms. Hunt stated that she hopes to find a new director because she needs to operate as a regional director for all of her five centers.

Child Care Staff Member 9 stated the lead caregiver in the infant room recently quit so there is no lead caregiver in that room. She stated that Child Care Staff Member 1 is the lead caregiver in the toddler room, Child Care Staff Member 8 is the lead caregiver in the waddler room and Child Care Staff Member 3 is the preschool lead caregiver. Licensee designee Lisa Hunt stated that the lead caregiver in the infant room quit two months ago and they have not hired a new lead caregiver yet. Ms. Hunt stated that Child Care Staff member 1 is the lead caregiver in the toddler room. Child Care Staff Member 8 is the lead caregiver in the waddler room and Child Care Staff Member 3 is the lead caregiver in the preschool room. Licensee designee Lisa Hunt provided documentation to verify that Child Care Staff Members 1, 8 and 3 qualify as lead caregivers in the toddler, waddler, and preschool classrooms.

During the onsite inspection on April 12, 2023, Child Care Staff Member 9 stated that Child A (male, ) has been enrolled at the center since he was old. Child Care Staff Member 9 stated that Child A is Child Care Staff Member 9 stated a a child care staff member has to be next o Child A at all times. Child A injures other children in care sometimes two times a day, sometimes once a week or three times a week depending on his mood or if he is otherwise engaged. Child Care Staff Member 9 stated that she would need to check incident reports posted on the Bright Wheel online application used to notify parents about incidents or accidents. On April 11, 2023, Child Child B (male, ) on the . Child B's Mother was upset because this was not the first injury that Child B sustained at Child A. Child Care Staff Member 9 told Child B's Mother that sheth wlouu ld try her best to limit Child A and Child B's time together. Child A's name was not disclosed to Child B's Mother. In order to limit Child A and Child B's time together Child A was moved from the toddler room to the preschool room on April 11. 2023, where he will have one-on-one care. Child Care Staff Member 9 stated that she spoke with Child A's Mother about the incidents of Child A children. Child Care Staff Member 9 suggested that Child A's Mother have Child A but Child A's Mother was not receptive to that. Child Care Staff Member 9 stated that whenever there is an incident with Child A child care staff members notify her and she notifies Child A's Mother and the parent of the other child by using the Bright Wheel online application. Child Care Staff Member 9 spoke with licensee designee Lisa Hunt about Child A's behaviors. Ms. Hunt has implemented a plan for Child A that includes Child A being reduced to half days if he has another Child A is currently in care for 12 hours per day. If there is another incident of Child A after that he will be disenrolled from the center. Child Care Staff Member 9 sated that she hopes that the one-on-one care will prevent Child A from-"„"- Child Care Staff Member 1 stated that she has been employed at the center since September 2022. Child Care Staff Member 1 works in the toddler room with eight children ages ; Child Care Staff Member 1 always works with a second child care staff person but it varies day by day. Child Care Staff Member 1 stated that children often hurt other children, and nothing is done about it. Child A is and every second of the day he is Child A will also "come for""ci care staff Ememmembers en theyn ervene between Child A and . Child A likes to Child B and Child E but he also as well. Child Care Staff [1 Member 1 stated that on average Child A other R hild Care Staff Member 1 stated that i d A's behaviors existed Re egaCn working at the center in September 2022 but things have gotten worse over the last month. Child Care Staff Member 1 was by Child A last Thursday, April 6. 2023. The was so on Child Care Staff Member 1' still existed on the date of onsito inspection on April 12, 2023. Child Care Staff Member 1 stated that Child A was child so she intervened, and Child A Child Care Staff Member 1 spoke with Child Care Staff Member 9 about Child A's behavior and requested another child care staff member in the room. Child Care Staff Member 9 told Child Care Staff Member 1 that she does not need another child care staff member in the room but that she needs to be a "buddy" to stick close by Child A throughout the day. Child Care Staff Member 1 stated that she tries as much as possible to keep an eye on Child A but there are seven other children in the classroom who need care. Child Care Staff Member 1 does not feel that center is the appropriate place for Child A because h

Child Care Staff Member 9 stated that she has not had any issues with child care staff members being on their cell phones for personal use. She stated that the center uses the Brightwheel application to document incidents, to message parents and to input daily record requirements such as diaper changes and meals. Each child care staff member has the Brightwheel "app" on their cell phones, so someone may think that they are using the phone for personal use when they are actually on Brightwheel. Licensee designee Lisa Hunt stated that child care staff members are only allowed to use their cell phones to use the Brightwheel application to communicate with parents. Child Care Staff Members 1, 2, 3, 4, 5, 6 and 7 all state that they only use their cell phones to input information on the Brightwheel application. Child Care Staff 11 Members take pictures of children and send them to parents, notify parents of incidents or accidents, record diaper changes, meals/bottles and naps all using the Brightwheel application. Child A, B, C, D, E and F's Mothers stated that they all receive messages from child care staff members using the Brightwheel application. None of the parents report having issues with child care staff members being on their phones and not supervising children appropriately.

Child Care Staff Member 9 stated that she has never heard any complaints about staff and children not washing hands at appropriate times. Hand washing signs are posted at every hand washing sink and child care staff members wash hands at the beginning of each day, before handling food and after diapering children or using the bathroom. Children in care also wash their hands before eating, after being diapered and after using the bathroom. Licensee designee Lisa Hunt stated that child care staff members wash their hands before their shift starts, after diapering, using the bathroom, blowing their nose. They also wash their hands before food preparation. Ms. Hunt stated that child care staff members are given a quiz about handwashing to ensure that they know the rules. Child care staff members also ensure that children wash their hands before eating, after diaper changes and after using the bathroom. Child Care Staff Members 1, 2, 3, 4, 5, 6, and 7 all stated that they wash their hands at the start of the day. They also wash their hands after diapering or using the bathroom and prior to feeding children or handling food. Hands are also washed if children's bodily fluids are handled. Child Care Staff Members 1, 2, 3, 4, 5, 6, and 7 ensure that children wash their hands after a diaper change or using the bathroom 12 and before eating. Children also wash their hands if they have their hands in their noses or down their pants.

Child Care Staff Member 9 stated that diapers are changed every two hours or as needed. The only time that children would be soiled is during nap time but children are changed when they wake up. If the child is dry when they are checked after two hours, they are then checked every 30 minutes. Licensee designee Lisa Hunt stated that diapers are changed every two hours or sooner if they are wet. Ms. Hunt stated that diaper changes are noted in the Brightwheel application. Ms. Hunt has not heard any concerns from parents regarding children's diapers not being changed. Child Care Staff Member 1 stated that diaper changes are done regularly and documented using the Brightwheel application. Child Care Staff Member 2 stated that diapers are checked every two hours or changed sooner if needed. Child Care Staff Member 3 stated that children are not left in soiled diapers. They are checked often and changed as needed. Child Care Staff Member 4 stated that when she enters a room as a floater, she checks the Brightwheel application to see if diapers are up to date. Child Care Staff Member 4 checks children's diapers every hour but they are told to check every two hours. Child Care Staff Member 4 stated that she has never noticed that children's diapers are being changed appropriately. Child Care Staff Member 5 stated that she notes diaper changes in Brightwheel. Children are never left soiled. Child Care Staff Member 6 stated that children are appropriately diapered and are never left soiled. Child Care Staff Member 7 stated that children's diapers are changed at least every two hours or sooner if needed. She stated that children are never left in soiled diapers. Child C's Mother stated that on occasion Child C will end up with because she was left sitting in a wet diaper for too long at the center. Child C's Mother gets notifications of diaper changes through the Brightwheel application but sometimes it only shows two or three diaper changes in a day and Child C is at the 14 center for 9 hours. Child C's Mother has not spoken with anyone at the center about Child C's being left in a wet diaper. Child F's Mother stated that she will be removing Child F from the center at the end of April. Child F has had a three different times with the last time being that Child F=s Mother almost took Child F to the hospital. Child F's Mother stated that she gets notifications in Brightwheel of dia er changes, but she believes that she was left soiled for which During the onsite inspection I reviewed records from the Brightwheel application which does not show diaper changes every two hours for Child C and Child F. Child A, B, D and E's Mothers do not report any issues with diaper changes and stated that they are notified in Brightwheel when diapers have been changed.

Child Care Staff Member 9 stated that she has never witnessed or heard about child care staff members hitting children too hard on their backs to make them stay on their cots. Child Care Staff Member 9 stated that child care staff members are very nurturing with children. Licensee designee Lisa Hunt stated that the center's philosophy is to be nurturing to children. She has never seen or heard of any child care staff member being mean or abusive toward children. She stated that hitting children too hard to get them to stay on their cots would result in automatic termination. Child Care Staff Members 1, 2, 3, 4, 5, 6 and 7 all stated that they have never hit children hard on their backs to get them to remain on their cots. None of the child care staff members have heard or seen children being hit hard on the back during nap time. Child A, B, C, D, E and F's Mothers do not report any concerns regarding Child A, Child B, Child C, Child D, Child E and Child F being hit too hard on their backs during nap time.

Child Care Staff Member 9 stated that she has never heard child care staff members scream at children or tell them that they do not like them. Child Care Staff Member 9 stated that when child care staff members start to become frustrated, they are told to take a 10-minute break so that they do not scream at children. In the past if child care staff members were heard yelling at children, they were sent home for the day. Licensee designee Lisa Hunt stated that child care staff members are very nurturing and sweet people. She has never heard of child care staff members screaming at children. She has never had a complaint from any other child care staff members or from parents. Child Care Staff Members 1, 2, 3, 4, 5, 6 and 7 stated that they have never screamed at children or told children that they do not like them. None of the child care staff members have ever heard any other child care staff member screaming at children. Child Care Staff Member 4 stated that child care staff members are very comforting toward children. Child Care Staff Member 5 stated that child care staff members are really good with children and children love to come to the center. Child Care Staff Member 6 stated that child care staff members are nurturing and have a connection with the children in care. Child Care Staff Member 6 stated that parents always tell her and other child care staff members that the children love them. Child A's Mother stated that she has never had concerns about the care provided to Child A at the center. Child B's Mother did not report any concerns with how child care staff members treat Child B. Child C's Mother stated that she believes that Child C has been treated nicely by child care staff members. Child D's Mother stated that she has had two other children enrolled at the center prior to Child D. She has had a good experience with the center and she has no concerns about the care that Child D receives. Child E's Mother stated that her experience with the center has been good. She stated that she never hears children crying or child care staff members yelling at children. Child F's Mother stated that Child F's most recent child care staff member is great. Child F's Mother stated that she does not know the names of the child care staff 17 members because there has been a lot of turn over but she has not had issues with any of them.

Child Care Staff Member 9 stated that the center uses shade canopies to provide shade for children. The canopies are taken down every winter because snow accumulates on the canopies and destroys them. Child Care Staff Member 9 stated that there are also trees on the play area that also provide shade. The shade canopies have not been put up yet because it has not been very warm or sunny yet. Licensee designee Lisa Hunt stated that the shade canopies which were taken down over the winter will be put back in place over the weekend. Ms. Hunt stated that there are also trees on the playground that provide shade. During the onsite inspection on April 19, 2023, I observed the outdoor play area. I saw trees on the playground that provide shade to children in care. I also saw the post for the shade canopies but the canopy was not in place.

During the onsite inspection on April 19, 2023, I observed the outdoor play area where there were three holes in the dirt that could pose a tripping hazard. There was a slide on top of a hill that had turf on each side. The turf was ripped and hanging from one side of the slide also posing a tripping a hazard. Child Care Staff Member 9 stated that she was not aware of the holes on the playground or the ripping turf on the slide but stated that she would have the holes filled and the turf removed. Licensee designee Lisa Hunt stated that the center receives a delivery of mulch every spring to fill in any holes on the play area. She stated the turf was put in place on the side of slide to keep children cleaner as they went down the slide on the top of the hill. She stated that she was not aware that the turf was ripped and hanging off but stated that it would be removed immediately.

Child Care Staff Member 9 stated that it is not true that the center is dirty. She stated that the center is cleaned and sanitized constantly using the three-step method of washing with soap and water, rinsing with water, and sanitizing with a combination of bleach and water. Child care staff members clean classrooms and toys and equipment during down time or nap time. Licensee designee Lisa Hunt denied that the center is dirty. Ms. Hunt stated that she opened the center because she saw other centers that were dirty. Ms. Hunt takes pride in keeping a clean center. Ms. Hunt stated that Child Care Staff Member 6 is the closing child care staff member and she has a lot of cleaning duties. Ms. Hunt stated that she and her husband also come into the center after hours to clean. Child Care Staff Member 6 stated that she is the closing supervisor so she gives breaks and cleans the center. She stated that center is never dirty. During the three onsite inspections on April 13, 2023, April 19, 2023, and May 2, 2023, I observed the center to be maintained in a clean and safe condition.

On 02/15/2022 I made a phone call to the complainant requesting a return call. On 02/15/2022 I made an announced visit with we interviewed Lisa Hunt, program director. Ms. Hunt said the center goes outside in cold weather as this is routine to do so, even if it is only for a few minutes. She stated the center has plenty of extra clothing to in case parents forgot to bring a needed item for outdoor wear. She said child care children are always appropriately dressed for the weather and there has never been a time a child sustained frost bite while playing outside in negative 10-degree weather nor are children told to put their hands in their pockets if they do not have gloves. Ms. Hunt stated no parent has 3 ever complained about their child receiving frost bite either. The center provides gloves if needed. I interviewed Sierra Adkins, child care staff member from the preschool She stated she works Monday through Friday typically between 1:30PM and 6:00PM. Ms. Adkins stated the children typically get ready to play outside between 3:00PM and 3:30PM, so the preschool children can be outside at the same time the school-aged children are being dropped off. If it is "super cold" and cold enough to where children "aren't having fun, we won't go out". If it is warm enough, the children will play outside for roughly 15 minutes. The children use snow pants, boots, hats, gloves, coats and the center has extra outdoor clothes for any child that does not have weather appropriate outdoor clothing. Ms. Adkins stated there has never been a time a child didn't have gloves and she is not aware of a child getting frostbite. Kelsey Hildebrandt, child care staff member in the preschool classroom, was interviewed. She said she works Tuesday through Friday from 1:30PM until 6:00PM. She stated after snack time, the child care staff members help the child care children get dressed for outside play. For example, today the preschool classroom went outside for 15 minutes around 3:30PM. The child care staff members read the what the outdoor temperature is, and if it is around 12 or 15 degrees, then the preschool classroom will not go outside because it is too cold. She said the children should have winter boots, snow pants, coats and water proof gloves. Not all children have the water proof gloves and some children wear scarfs. The center has extra clothing for any child who does not have the needed equipment to go outside. Extra outdoor clothing was purchased by Ms. Hunt and includes spare boots, gloves, coats, etc. If a child is missing anything, the center provides it. Ms. Hildebrandt has never heard of a child sustaining frostbite while at the center. Jennifer Ward, lead infant child care staff member was interviewed. She said the center provides extra outdoor clothing for use if needed. She said the infant room does not go outside if the temperature is below 40 degrees as this is too cold for the infants. Ms. Ward has never heard of a child sustaining frostbite at the center and she has never seen a child playing outside that was not dressed appropriately for the weather. Kristen Jonas, administrative assistant and lead Great Start Readiness Program (GSRP) child care staff member, was interviewed. She has been working at the center for roughly seven and a half years and works Monday through Friday from 8:00AM until 4:00PM or 5:00PM. The GSRP classroom goes outside roughly three times a day, weather permitting. Children are dressed in snow pants, boots, coats, hats, gloves and some come with scarves. The center supplies additional outdoor clothing that children can borrow for the day or week. Once the children are done 4 borrowing whatever they need, the outdoor clothing is washed. Ms. Jonas has never heard of a child sustaining frostbite. P I discussed the need to interview other child care staff members and Rompete another onsite inspection on 02/16/2022 at 9:15AM. She stated the center uses an app called Bright Wheel to document diaper changes and parents have not complained about their child's diaper not being changed. Ms. Adkins, child care staff member from the preschool classroom was interviewed. She stated she does not recall a time a child was left in a soiled diaper. She said when the center first began using Bright Wheel, it took the child care staff members and parents time to learn the app and not all parents had access to the app right away. Therefore, this caused confusion for parents as child care staff members were not recording diaper changes promptly. Currently, the app is working well, and parents have access. Diaper changes and potty times are documented using the app and there are no concerns with child care staff members not changing the children's diapers. Ms. Hildebrandt, preschool child care staff member, was interviewed. She said the child care staff members check the children's diapers every two hours and the diaper changes are entered on the Bright Wheel app, including if a child attempted to use the toilet. There is also a whiteboard in the classroom that is used as a visual to remind child care staff members when to check the children's diapers again. The whiteboard is marked with each child's initials, the times they were changed and the outcome of the diaper change. Ms. Hildebrandt said at the end of the night, she takes a picture of the whiteboard and sends the picture to Ms. Hunt to show the children's diapers were adequately changed. She stated there was never a time a child was left in a soiled diaper. If she "smells something" in the classroom or if she 7 notices that a diaper is sagging down, then the children are changed immediately. She clarified that diaper checks start as soon as the children arrive in the morning. Although Ms. Hildebrandt is not present during arrival times, the whiteboard is used to show the last time diaper changes occurred. Jennifer Ward, lead infant child care staff member was interviewed. She said she typically works Monday through Friday from 1:00PM until 6:00PM but works other hours to help open if needed. She usually arrives to the center after lunch time and starts with diaper changes. Diapers are checked every two hours and logged into the Bright Wheel app. A whiteboard is also used in the infant classroom that includes the children's names, the time they are changed and the outcome of the diaper change; the child care staff members also write the times for when the two-hour mark is up so everyone knows what time to check the diapers again. She stated no one has complained to her about diapers not being changed. Ms. Ward stated there was a time someone missed a diaper change as she noticed a child had a bowel movement, but she quickly changed them. Sometimes the children have bowel movements right after a diaper change, so she can understand how those might get overlooked. This does not happen regularly or often. When child care staff members notice a soiled diaper, they will promptly change the child's diaper. Kristen Jonas, administrative assistant and lead GSRP child care staff member was interviewed. She said the children's diapers are changed every two hours unless the child care staff members notice that a child needs to be changed sooner. There is a whiteboard in the classrooms with each child's initials, the time the diaper was changed, and the diaper change is also recorded in Bright Wheel. She stated she isn't sure if the child care staff members continue to use the whiteboard daily, but they always log in the diaper changes in Bright Wheel. Ms. Hunt was not at the center when called, so she called Shakeay Watts and asked her to go into the preschool classroom and speak with . Roughly a half an hour later Ms. Hunt texted Ms. Watts to see ho conversation went. Ms. Watts responded that she was able to speak with , but "there is nothing the center can do please her." Ms. Hunt said this is the sixth or seventh complaint from about "random things." Ms. Hunt believes _ has a higher standard or what a high-quality program can offer. For example, does not understand why the children have to go outside. Ms. Hunt said on Thursday, 02/10/2022 she had a sit-down meeting with Ms. Hildebrandt regarding the allegations made. This was documented as a written warning and Ms. Hildebrandt is required to attend additional trainings. Prior to this, neither Ms. Hildebrandt nor Ms. Adkins had any disciplinary records on file. Ms. Hildebrandt received a written warning because Ms. Hunt wanted documentation of the allegation and how the center handled the allegation. said Ms. Hildebrandt "yanked" Child A and Ms. Hildebrandt denied ever doing so. Ms. Hildebrandt said she took Child A's shoulder and guided him around. Ms. Adkins was asked what happened as well and she responded she witnessed W yell at Ms. Hildebrandt saying, "Don't touch again" andFs.= e randt appeared scared. Ms. Adkins denied seeing Ms. Hildebrandt "yank" Child A. Ms. Hunt stated she has never had any prior complaints on Ms. Hildebrandt or Ms. Adkins. Ms. Hunt has no concerns about Ms. Hildebrandt or Ms. Adkins and has not observed Ms. Adkins yelling at children. Ms. Hunt denied ever blaming a child for a child care staff member 's reaction to something. Sierra Adkins was interviewed. She was aware a complaint was made because Ms. Hunt called her yesterday regarding the complaint. Ms. Adkins stated when the preschool classroom came in from outside play, the children were directed to sit in the carpet area to do a painting project. Child A went to play by himself in the kitchen area instead. Ms. Hildebrandt walked over to him, helped him put a toy away and said, "this area is closed" and she was going to turn him around to look at the carpet area when Child A's Mother walked in. Child A then walked towards his mother. Ms. Adkins said she saw this happen through the mirror in the classroom, and it looked as if Ms. Hildebrandt was going to move his body. Ms. Hildebrandt did not have to jump over any children when addressing Child A, nor did she yell or grab Child A. 10 Ms. Adkins stated she does not yell at the children, when she needs to pick up a child, she does so from under their arms, so she is not pulling on their arms. If she raises her voice, it is to get the children's attention which may sound like yelling further away as the classroom echoes, but she is not yelling. She stated it is the center's policy to pick up children underneath their arms. Kelsay Hildebrandt, child care staff member in the preschool classroom was interviewed. She stated last week, either Tuesday or Wednesday afternoon, the preschool children were coming back inside the classroom after outdoor play. The children were taking off their winter "gear" to get ready for the next activity. She noticed Child A playing in the kitchen area, she then gently used an open hand on Child A's back to redirect him towards the carpet area as this is where the children needed to be for the next activity. She said only her fingertips touched his back in order to show Child A the direction they were headed. The only thing she said to Child A was this area is closed, then came towards her and said, "I don't like the way you are speaking tohim." s. On 02/15/2022 I made an announced visit to the center and interviewed s. Hunt. Ms. Hunt"sai in regard to feedings, the center follows a strict meal schedule, and all classrooms receive breakfast, lunch and snacks. Infants and toddlers eat on demand and all feedings/meals are recorded through the Bright Wheel app. She said no parents have reported concerns regarding meals. If a sippy cup was not taken out of a child's bag, this means it was not used but does not mean they were not fed. When children are 12 months of age and older, the center provides food. If the children are 12 months of age and younger, the parents provide the food. There was never a time child care staff members refused to feed a child. Sierra Adkins, child care staff member from the preschool classroom, was interviewed. She said she works Monday through Friday typically between 1:30PM and 6:00PM. When she arrives at the center, children are typically sleeping and start waking up around 2:30PM. Afternoon snack starts around the same time once children put their blankets away. Nap time usually begins after lunch, around 12:40PM. Once more children are awake, Ms. Adkins usually helps with snack time while Ms. Hildebrandt completes the diaper changes. The center uses an app called Bright Wheel to document meals and snacks for every child, this app is maintained and up to date for parents. She said she cannot recall a time when a child care staff member refused to feed a child. The children are offered either water or milk during snack and meal times. If a child brings in their own sippy cup, it is labeled 14 appropriately with names and dates and typically rinsed at the end of the day so parents can wash it at home. Kelsey Hildebrandt, child care staff member in the preschool classroom, was interviewed. She stated snack time starts at 2:30PM which is after nap time. Snack is charted in the Bright Wheel app for parents. No parents have ever complained about their child not being fed. If a child refuses to eat, child care staff members will offer the food and notify the parents of the child's refusal. She described a time when a child in her class complained his mouth hurt and would hardly eat, so she notified the parent of this. She stated the Bright Wheel app shows what was offered during meal times and what they ate, but not how much of it they ate. Jennifer Ward, lead infant child care staff member was interviewed. She said she isn't usually present in the classroom during lunch time as she starts her shift at 1:00PM. However, the schedule for infant feedings is to feed the younger infants before the older infants as the older infants eat the center's food. The infant classroom tries to ensure the younger ones are fed before diverting attention to feeding the older infants. Ms. Ward has never had a parent complain about a child not being fed nor was there a time a child was not fed while at the center. Sometimes parents will provide food and staff will help the children eat. Kristen Jonas, administrative assistant and lead GSRP child care staff member, was interviewed. She said she has not received any complaints from parents or staff about children not being fed. All feedings and meals and recorded in the Bright Wheel app. If a child refused lunch, child care staff members will verbally communicate with the parents about this or send them a private message via Bright Wheel to alert them. On 02/16/2022, I made an unannounced visit to the center. We interviewed a ina Wittman, ministrative assistant and classroom floater. She said breakfast is served between 8:45AM and 9:00AM, but it is offered until 9:30AM for any children that arrive late. The children do come to the center hungry, so child care staff members want to ensure children are eating. After breakfast, staff conduct small and large group time. Lunch is served between 11:45AM and noon and brought in to all of the classrooms. An afternoon snack is offered after nap time which usually ends between 2:15P

On 2/4/2021 I completed an unannounced on-site inspection at the child care center. I informed licensee, Lisa Hunt, of the allegations. Ms. Hunt said she was surprised to hear of the allegations. Ms. Hunt has not had any parents express concerns to her about the behaviors of any children in the preschool classroom. Ms. Hunt said the group of parents who currently have children enrolled at the center do not typically have a problem with addressing concerns with her. Ms. Hunt could not immediately identify any children whose behaviors fit the description in the allegations. Ms. Hunt said a short time ago a couple of the teachers described Child A as a "hard" child. Child A liked saying "booty" and has said the "F word" with another child in the class. The teachers did not report Child A's behaviors were sexual in nature. Ms. Hunt spent a week in Child A's classroom to observe. Ms. Hunt said Child A seemed to be "starved for attention." Ms. Hunt encouraged the teachers to engage often with Child A to keep him from seeking out negative attention. Ms. Hunt said if Child A is the child described in the allegations, Ms. Hunt is surprised the behaviors raised to the level of a complaint because Child A "wasn't bad" when Ms. Hunt observed him in class. Child A seemed to require positive attention and re- direction. Ms. Hunt said after she gave the teachers feedback on how to work with Child A, the teachers did not complain about Child A's behaviors anymore. Ms. Hunt said if a child exhibits extreme behaviors, she will first do her best to work with the child's parents and the teachers to develop a behavior plan that works for the child. Ms. Hunt does not immediately withdrawal a child if the child has behavior problems. Ms. Hunt works to find a solution so the child will be successful at the child care center. I interviewed child care staff member Perrise Stewart. Ms. Stewart said she could not identify a child whose behaviors matched those in the allegations. Ms. Stewart said on one occasion a parent expressed concern that her child talked about killing someone. Ms. Stewart realized that this child, along with other boys in the classroom, had been playing a zombie game and may have learned to use the phrase "I'm going to kill you" because of the game. The boys like to say they are going to kill the zombies. Ms. Stewart told the parent she will encourage the boys to use terms like "I'm going to capture you" instead of using violent words. The parent seemed satisfied with Ms. Stewart's response. Ms. Stewart said a lot of the children talk about butts and giggle because they think the word "butt" is funny. Ms. Stewart said when the children play "house" they sometimes act like they are pulling down their pants when pretending to be the baby using the potty. On one occasion Ms. Stewart observed a child attempt to really pull down their pants while playing "house." Ms. Stewart was able to intervene before the child pulled down their pants. Ms. Stewart explained to the children they can only pretend to pull down their pants unless they are in the real bathroom and have to use the real toilet. 3 Ms. Stewart said if a parent has a concern she has no problem addressing the concern with the parent and working to resolve the concern. If Ms. Stewart is unable to resolve a parent's concern on her own, she will request assistance from Ms. Hunt. I interviewed child care staff member Kristen Jonas. Ms. Jonas was unable to identify any children whose behaviors are consistent with the behaviors described in the allegations. Ms. Jonas has not had any parent express concerns regarding the behaviors of a child in the classroom. Ms. Jonas said she has heard children talk about butts and underwear in the classroom. Ms. Jonas said it is common for preschool children to think the words "butt" and "underwear" are funny.
