R 400.8263(1)(b) · R 400.8263(1)(b) Child accidents and incidents; child and staff illness (1) A center shall develop and implement a written plan for how and when a parent is notified if program staff or volunteers observe any of the following: (b) A child experiences an accident, injury, or i ncident.

The center has a written plan for when a child experiences accidents, injuries or incidents. On 03/09/2026, the center followed their accident reporting policy when Child A fell on the playground.

R 400.8266(4) · R 400.8266(4) Incident, accident, injury, illness, death, fire reporting (4) A licensee, licensee designee, or program administrator shall report to the department, via phone, fax, or email, within 24 hours of notification by a parent that a child received medical treatment or was hospitalized for an injury, accident, or medical condition that occurred while the child was i n care.

On 03/12/2026, the center reported to the department, within 24 hours of notification by Child A's Mother, that Child A received medical treatment for an injury that occurred while in care.
