I interviewed Child A's Mother. She stated that approximately six weeks ago, CCSM 1 told her that Child A "flashed" another child. Child A's Mother believes that this is normal behavior based on his age. She was told that he would be placed under increased supervision and that the room would be rearranged to assure for better supervision of the children. On 5/4/23, she was notified to pick up Child A after he was observed in the outside playhouse with another child with their pants down. On 5/8/23, she was notified that Child A tried to unbutton another child's pants, despite Child A not knowing how to unbutton pants yet. Child A was disenrolled from the center. She believes his behaviors are normal for his age, saying that he should have been redirected away from the other children. She is concerned that the children are not appropriately supervised as the kids are "all over the place and the staff just sit there." She also questioned the mental capabilities of one of the CCSMs but did not provide a name. Child A does not have any current medical diagnosis, but she is concerned that he may have autism. Child A's Mother stated that she did not report any previous concerns as she believed the center was trying to handle the situation. I completed an unannounced on-site inspection to the center on 5/12/23. I observed the preschool room, which is approved for up to 18 children. The room cares for children ages 33 months (with written parental permission) and older to operate within a 1 CCSM to 10 children ratio requirements. During the operation there were two Child Care Staff Members (CCSMs) caring for the children. The children were playing in an open fenced area near the garden planter boxes. The children were appropriately supervised. The CCSMs were spread out to provide for better observation of the children. I interviewed Ms. Laeder, program director. She began working at the center the end of March 2023. During the interview Ms. Mentink, licensee joined the interview. They both acknowledged that three incidents occurred with Child A. Ms. Mentink 3 was not present for any of the three incidents. CCSM 1 and CCSM 2 were present for the first incident. CCSM 1 was present for the second incident. CCSM 3 and Ms. Laeder were present for the third and final incident. All the parents were notified. CCSM 1 and CCSM 2 are the main CCSMs in Child A's room. Ms. Laeder and CCSM 3 also frequently work in the room. I reviewed the incident reports regarding Child A. According to the incident reports: • On 3/31/23, CCSM 1 observed out of the corner of her eye Child A (age 4 years, male) and Child B (age 3 years, male) pulling their pants back up having just flashed each other while standing a few feet apart. She told them to pull up their pants while walking a few feet over to them. No inappropriate touching occurred. CCSM 2 spoke with the boys about not pulling their pants down at preschool. Both boys said that Child A asked Child B to pull his pants down. When asked why, Child A indicated a reason that was not center related. This is the first time this behavior as occurred from either child. Both parents were contacted. A safety plan will be put in place when Child A returns to care. • On 5/4/23, Child A and Child C (age 4 years, male) were playing on the playground. CCSM 1 was walking around when she noticed the boys were in the playhouse. She walked over and noticed Child A starting to pull Child C's pants down. CCSM 1 placed her hand on Child A and said, "No thank you." CCSM 1 removed Child A from the playhouse and had him sit on the bench. CCSM 1 made sure that Child C was able to get his pants up. No other acts where made. Both parents were notified. • On 5/8/23, Child A and Child B were outside playing. Child B yelled "No" then came up to Ms. Laeder saying that Child A unbuttoned his pants and wanted to pull them down. Ms. Laeder had been keeping a close eye on Child A throughout the day. I interviewed Child A's Mother. She stated in the Winter of 2022, Child A tried to follow her back to her car when she dropped him off in the morning. She entered and left through the center's front door. When she came out the front door, she turned around to see Child A inside at the front door. She went back into the center and gave him to CCSM 1. She was told by another parent that there was another incident at lunch time or outdoor play time, but she was not there. She was told by that parent that Child A left the playground and got into the parking lot. She was not sure of the date, nor would she provide me with any additional parent contact information. The last incident occurred maybe the week of March 20 — 24, 2023. Child A's Mother left the center after dropping off Child A. She turned, and again Child A was standing at the front door. He may have been outside the door. rj She again took him back to CCSM 1 who told him that he cannot do that. After this incident, Child A's Mother decided to wait until the children began circle time before she left. She stated that he did not report the incidents when they happened as she believed that the center was trying to handle the situation. I observed the front door to the center. The front door is located inside Child A's Room. The door to the outside leads directly into a fenced in play area. The fenced in play area has a gate that can be opened leading to the front parking lot. I interviewed Ms. Laeder. She was told that Child A had a history of wanting to run to the front door which is located inside his classroom. The door to the outside leads directly into a fenced in area to provide for better protection of the children when they first go outside. Child A was watched closely when he was near the door. Child A has not tried to run out the door since she began employment on 3/31/23. Ms. Laeder has never heard that Child A was ever outside unsupervised or that he went into the parking lot. She has no concerns regarding the CCSMs ability to provide care and supervision for the children. I interviewed Ms. Mentink. She was not aware of Child A getting outside without being supervised. She has not heard that he was in the parking lot. She heard that he would run to the front door, which is in Child A's room, but that he was not successful in leaving. Ms. Mentink stated that she had the play area gate extended to fully enclose the front door to add an additional level of protection when going outside. She also has an alarm on the doors. She has no concerns regarding the CCSMs ability to provide care and supervision for the children. CCSM 1 stated that beginning November or December 2022, that Child A started wanting to give other children hugs goodbye near the front door which is in the room. Child A never went outside unsupervised, although he would try to run out. Child A thought it was funny. On one occasion, he took two steps outside the door onto the porch landing as another parent was leaving, but she immediately redirected him back inside as she was standing right at the open door. Child A never went into the parking lot. There is always a CCSM right next to the door as children are coming and going. Child A was told that if he wanted to say goodbye to the other children that he needed to do so further inside the room. CCSM 1 denied that Child A's Mother ever brought Child A back inside the room or said anything regarding Child A being at the door either inside or outside the center. CCSM 1 stated that Child A went near the door when his mother was leaving, but he did not leave and was redirected away from the door. The children receive good care and supervision. CCSM 2 stated that she has not witnessed Child A leave the center unsupervised or get into the parking lot. Child A used to try to give children hugs goodbye next to the door in the room. On November 3, 2022, and I spoke with Amanda Renee Hanson, assistant program director, Ms. Hanson stated on October 27, 2022, at 9:48 a.m. she received a text message from that she had just 4 observed Child A. Child 's Mother was informed of the allegations, the two different reports of what happened, and that this incident was being reported to licensing to allow the professionals to handle it. Ms. Hanson stated there have been recurring issues between Child A and frustration level is concerning. There has been no formal disciplinary action. Ms. Hanson stated they had not told Child A's Father about the incident because Child A's Mother asked them not to tell Child A's Father anything regardin-M care. I explained bath parents are part of a collaborative team and should be informed of concerns, , and anything that is of concern regarding their child. Ms. Hanson stated ere is a training at the next scheduled staff meeting for all child care staff members to be trained on mandated reporting. Moving forward, child care staff members are aware of the expectation they report any suspected abuse or neglect to management and contact On November 3 2022 and I spoke with child care staff member stated she has worked there a few months and she is a floater and works in all classrooms.
