Ms. Reynolds did not ensure that smoking did not occur on the premises of her child care home while children were in care Upon entering Ms. Reynolds’ home, there was a smell of smoke. When I asked if she smokes on the premises during child care hours, she stated that she smoked a cigarette on the front porch about an hour before I arrived for the on-site inspection at 2:00 p.m., while children were in care.

Ms. Reynolds did not provide a clean, safe and hazard free outdoor play area. During the on-site inspection I observed an ash tray with cigarettes in the outdoor play area next to an adult lounge chair. I also observed a pet waste scooper with two garbage bags of pet waste on the wooden patio platform in the outdoor child play area. There was a rusty metal hanger sticking off the garage within the outdoor child play area. The access door to the garage had rusting and chipping paint. The trim and siding of the garage had wood rot and peeling paint within the outdoor child play area. On the northwest wide of the outdoor play area there is a hazardous cement cylinder in the ground uncovered, this poses a tripping hazard. Ms. Reynolds states that this is a retired cistern that has been mostly filled in.

Ms. Reynolds’ home was not maintained in a clean, safe and comfortable condition. During the on-site inspection there was a broken window on her porch at the front of the house to the left of the front door.

Ms. Reynolds did not have dangerous and hazardous materials out of reach of children. During the on-site inspection, an ashtray was within the reach of children on the front porch where children enter the home. In the main dining/living room area, there was a pack of cigarettes on the table within the reach of children. In the kitchen, right outside of the children's play room, a push button lighter was found on the floor. The bottom drawer of bathroom contained numerous aerosol spray cans of personal hygiene products. In the hallway outside the child care bathroom, a small dresser had multiple drawers containing scissors, serrated tape dispensers, a hammer, multiple screwdrivers, wire cutters, button batteries, and assorted hardware. In an unapproved room where a child was sleeping, assorted hardware was not stored out of the reach of children.
