Ms. Abbott did not provide a clean, safe, and hazard free outdoor play area. During the on-site inspection I observed broken lawn ornaments with sharp edges, rusting metal lawn ornaments with sharp edges, a dug-out hole leading under the house and a metal garden hoe in the outdoor play area.

Ms. Abbott did not have dangerous and hazardous materials out of reach of children. During the on-site inspection a wax warmer was in use in the child care play room. In the kitchen, there was a container of saran wrap with serrated edges on the chest freezer within reach of children. In the kitchen there was also a broken lock on a drawer that contained numerous kinds of batteries and two drawers with no locks which contained plastic trash bags and plastic ziplock bags. In the living room there was a drawer with no lock which contained more than a dozen loose charging cords. In the living room there were also multiple drawers containing personal care items stating "keep out of reach of children" on the back as well as aerosol cans.

In the child care play room there were numerous toys with flaking and deteriorating paint. Most walls throughout the child care area had paint that was flaking or deteriorating.

At the time of the on-site inspection there were multiple combustible items, such as cans of paint or stain, clothing and cleaning products on a 4-tier shelf stored within 4 feet of the furnace.

While touring the upper level of the home, one room had a thick cloud of smoke and smelled of marijuana. When asked if smoking occurred in the home during hours of operations, Ms. Abbott stated it was an adult household member that was likely smoking in the upper level of the home but she knows it should not be happening on the premises at all due to her 24- hour child care hours.
