Teacher 1 observed Child A on the tricycle when his head dropped, monitored Child A and stayed close to him. Video footage showed Child A did not hit his head prior to the seizure. Both Teacher 1 and Teacher 3 indicated that Teacher 1 informed Teacher 3 of the incident as soon as she arrived for her shift on 09/24/2025. Teacher 3 submitted an incident report to Child A's Mother quickly and Child A was taken to the emergency room to seek medical attention.

On 3/13/25, I reviewed video camera footage from 3/12/25. The video indicated that Child A ran off from the group into the stairwell. On 3/12/25 CCSM 1, CCSM 2, CCSM 3, and Witness, confirmed that Child A ran from the group and was unattended for at least one minute or more. Witness confirmed Child A was found alone, away from her assigned group and brought back into the gym. CCSM 1 and CCSM 3 confirmed Witness brought Child A back into the gym. On 3/20/2025, Supervised Volunteer indicated that Child A ran from the group, but thought she ran to the classroom. When Supervised Volunteer arrived at the classroom, Child A was not there. Supervised volunteer went to the gym to get Child A.
