On May 5, 2021, at 7:21p m, Ms. Ketcham left a voicemail on this consultant's phone reporting that Child A had been taken to the emergency room. Child A's mother told Ms. Ketcham that Child A had a broken shoulder. Ms. Ketcham stated that Child A arrived at 8:20am on May 5, 2021. Child A appeared to be fine. A few days prior Child A appeared to use ■ left arm more than the right and this seemed odd, but it was assumed that Child A was attempting to figure out which would be the dominant hand. Child A crawls but at the time of this investigation did not walk. On May 5, '2021, Child A was crawling around and not fussy. Child A fell asleep in Ms. Ketcham's arms at approximately 9:30am then ate lunch while sitting in a high chair at approximately 11:45am. When Ms. Ketcham lifted Child A by cupping her hands under Child A's armpits, Child A let out a scream and 2 began to cry. Ms. Ketcham did not notice anything wrong, so she consoled the child I and had her husband lay Child A in a pack-n-play on the main floor of the home. Child A woke up "screaming." Ms. Ketcham checked diaper thinking a full diaper would have made Child A cry, but the diaper was dry. uring the diaper check, Ms. Ketcham looked but did not observe any unusual bruises or marks. When Child A's father, who does not live with Child A's mother, picked Child A up from child 0 care, Ms. Ketcham told him that Child A was ` scrinching" on right side and that something might be wrong. Child A's father stated he would keep an eye on it." That night, Child A's mother called Ms. Ketcham to say that Child A had a fractured shoulder. Mr. Ketcham, Ms. Ketcham's husband and child care assistant, stated that he had not interacted with Child A that day other than putting Child A in a pack-n-play. He was not aware of any incident that may have occurred that would have caused harm to Child A while in care that day. Savannah Ketcham, Ms. Ketcham's daughter and child care assistant, was not aware of any incident that may have occurred during the day that would have caused harm to Child A while in care that day. No child care children were aggressive, nor did a child fall on or throw toys at.. Attendance records were reviewed and the pack-n-play as well as the highchair were inspected while onsite. 3 Rose Rafferty and this consultant were in attendance. It was concluded that the injury was accidental and that there was insufficient evidence to substantiate a violation.
