    On Tuesday, 03/23/2021 I received a telephone call from Licensing Consultant, Timika Haley informing me that she sent me an email. The email consisted of an email and attachment that was sent to her on Friday, 03/19/2021 from Starfish Family Services. The document sent to me was a letter that Starfish Family Services sent to the families of the facility notifying them that an individual in the center tested positive for COVID-19. On Wednesday, 03/24/2021, I completed an onsite visit to Starfish Family Services, located at 30000 Hiveley in Inkster, MI to conduct an interview. I interviewed Krystle Dionise, Program Director, Kecia Rorie, Operations Director, and Rachael Druyor, Compliance and Risk Manager. Ms. Rechael Druyor was conferenced-in via telephone speaker phone. During the interview, Kecia Rorie, Krystle Dionise, and Rachael Druyor provided me with the following information and documents: • 03/07/2021- A staff member of Starfish Family Services tested positive for COVID-19. • 03/08/2021-The staff reported their test results to Human Resource. • 03/09/2021 — Human Resources contacted the Wayne County Health Department. The Health Department offered them no guidance. • 03/09-03/11/2021- The Compliance Team of Human Resources at Starfish Family Services conducted a contact tracing investigation to notify exposed individuals. During this interview, I received a copy of the 12 contract tracing questions used for their investigation. Based on the investigation, there was no exposure to children. • 03/12/2021 — I read an email from Staff 1 to Krystle Dionise informing her that she was exposed to a staff member at Starfish Family Services who tested positive for COVID-19 on 03/07/2021. The letter informed Ms. Dionise that the last day the infected person visited the center was on 03/05/2021. HR informed Ms. Dionise that the critical week had passed; however, according to Starfish's protocols, she is required to isolate herself from her physical work location for a period of 14 days and monitor her symptoms. • 03/13/2021 — I read an email from Staff 2 to staff, informing them that she was informed by Human Resources on Friday, 03/12/2021 after work hours, that she was exposed to someone at work who tested positive for COVID-19. She stated in the communication to the staff, that she went and had a test and will self- isolate. She informed them that Human Resources has identified and notified others through a contact tracing investigation. She reminded them to do their part by wearing their masks, washing their hands, staying in their own bubble, and not congregating with one another. • 03/16/2021 — Kecia Rorie, Operations Director had a meeting/training with the staff, with HR on speaker phone called, "Safe Start COVID Reporting Booster Training." A recording of the training was provided for staff who could not attend. The power point from the training was provided to me. • 03/19/2021- Letter was sent to the parents and guardians of the children who are enrolled at Starfish Family Services.
