On 2/16/22, I reviewed the incident report sent to the department form the center. According to the incident report, on 2/15/22 Child A sustained a nursemaid's elbow injury while in care at Ida Kids Center. When Child A was getting up from nap, child care staff member Ms, Carrie Brooks reached down to help Child A up from her nap. Child A was holding Ms. Brook's hands when she helped her up. Child A begun to cry. Child A was comforted and monitored by the child care staff members. The center also immediately notified Child A's Mother of the injury. Child A's Mother picked up Child A from care and had her examined by Toledo Emergency Department. According to the x-rays taken, Child A was diagnosed with nursemaid's elbow. Child A remained home from care on 2/16/22. She returned to care the following day on 2/17/22. On 02/22/22, I conducted an unannounced onsite inspection to the center and interviewed Ms. Nancy Hunter, program director and licensee designee. I informed Ms. Hunter off the allegation. Ms. Hunter confirmed that on 2/15/22 around 2:30p.m. Child A was awakened from her nap/rest time. Child care staff members, Ms. Carrie Brooks and Ms. Alison Turner were working in the toddler room on 2/15/22. According to Ms. Hunter, when Child A awoke from her nap Ms. Brooks reached down with her hands to hep Child A up from her nap. Child A put her arms out and held Ms. Brooks hands as Ms. Brooks lifted her from the cot. Child A begun to cry as Ms. Brooks lifted her up. Ms. Brooks wasn't sure if Child A was hurt as sometimes Child A takes a minute to wake up and collect herself from naptime. The child care staff members took the children to the bathroom for a diaper change. During that time, Ms. Hunter said Child A was favoring her left hand/arm. At this time, both Ms. Brooks and Ms. Turner asked Child A if her arm or hand hurt. They examined her hand and arm while comforting Child A. Ms. Hunter was also informed that Child A may have injured her hand or arm as she was favoring her left hand/arm. Ms. Hunter assisted Ms. Brooks and Ms. Turner with bathroom break. Ms. Hunter asked Child A if she wanted to walk with her to her office. Child A went to grab Ms. Hunters hand to walk toward the office and immediately pulled her hand back. Ms. Hunter notified Child A's Mother of Child A favoring her hand/arm and said that she may have injured it. Child A's Mother picked up Child A from care and took her to Toledo Hospital Emergency room to have her arm/hand examined. Child A's hand/arm was x-rayed. Child A was diagnosed with nursemaid's elbow. According to Child A's Mother, the doctor reset her arm and she is recovering fine. Child A returned to care at the center on 2/17/22. Ms. Hunter said Child A must have sustained the injury when Ms. Brooks picked her up by her arm after nap/rest time. Ms. Hunter informed both child care staff members, Ms. Brooks and Ms. Turner of the injury. Ms. Hunter said both child care staff members felt bad for Child A. Specifically, Ms. Brooks felt responsible as she should have lifted Child A up from the floor by picking her up from under her arm pits versus lifting her from her arms. Although it was an accidental injury, Ms. Hunter said she 3 reminded all of the child care staff members and posted guidelines in every classroom informing them of the importance to assure they are picking up the children the correct way, under the arm pits versus lifting them by their arms, to avoid an injury from occurring. She reminded all of the child care staff members that once a nursemaid elbow injury has occurred, there is a high likelihood of reoccurrence. For this reason, as well as to prevent an initial occurrence, it is important to lift children using the appropriate guidelines: • To safely lift a child, grasp gently under the child's arms • Do not lift a child by holding the child's hands or arms. • And lastly, avoid tugging or pulling on a child hands or arms. On 10/27/20, I completed an unannounced onsite inspection to the center and interviewed Ms. Nancy Hunter, program director/licensee designee, regarding the complaint. Ms. Hunter denied the allegations indicating that her staff members, to include Ms. Lisa Johnston, have never left any child in defecated pants. Ms. Hunter informed me that part of their preschool requirements is that the children are potty trained upon enrollment in the preschool program. Ms. Hunter further explained that on occasions, there have been times that children have "accidents" and were immediately changed by the staff members working in the classroom. Ms. Hunter said that she and the staff members work diligently with the families regarding their children's progress while in the program and have requested that the families of children who have just begun the potty training process provide an extra pair of clothes to have in case of an accident happening. Ms. Hunter could not recall Child A ever having an accident while in preschool. She further explained that Child A has not been present in the preschool program since March of 2020. He only attended care a couple of days, two to three days, from 9:00 am until 11:30 am. Ms. Hunter said she, Ms. Johnston, and Ms. Morgan Howe, are the staff members who teach in the preschool room each day. Ms. Hunter also denied ever hearing Ms. Johnston "scream or berate" any child in care. Ms. Hunter said if any staff member were to "scream or berate" any child in care the other teachers would hear as the preschool classroom and child care room are adjacent to each other. Ms. Hunter described Ms. Johnston as an "excellent teacher" whom all the children and children's parents enjoy. Ms. Hunter said Ms. Johnston has been working at the center for 21 years and she has never had a complaint or issue with Ms. Johnston and the care she provides to the children. 3 On 10/27/20, during my unannounced onsite inspection, I was briefly able to observe Ms. Johnston working with the preschool children before their class ended for the day. During my observation time, Ms. Johnston, and all the staff members were caring and nurturing toward the children. None of the children showed any signs of fear or hesitation toward Ms. Johnston or any of the staff members. On 10/27/20, I interviewed Ms. Lisa Johnston, staff member at the center. I informed Ms. Johnston of the allegations. Ms. Johnston denied the allegations indicating that in her 21 years of being a teacher at the center, she has never left any child sit in defecated clothing nor has she "screamed or berated" the child for having an accident and defecating in their pants. Ms. Johnston reiterated Ms. Hunter's statement regarding the children being potty trained before they are enrolled into the preschool program. Ms. Johnston said they work with the children's families with children that are still in the potty-training phase and have the families provide the center with an extra pair of clothing for the children in case of an accident. Ms. Johnston said no child is ever left in a soiled diaper of clothing for any period of time as they are immediately changed if they had an accident or have soiled their clothing. Ms. Johnston acknowledged Child A attending her preschool classroom two to three days a week from 9:00 am until 11:30 am until March of 2020 when the pandemic occurred. Ms. Johnston said she has had no contact with either Child A or Child A's Mother since the graduation pick up in May of 2020. On 11/10/20, I interviewed Ms. Morgan Howe, staff member via telephone. Ms. Howe has been a staff member at the facility for the past three years. Currently, Ms. Howe is a floater and works in all of the classrooms; however, prior to the pandemic, Ms. Howe worked in the preschool classrooms fulltime with Ms. Johnston. I informed Ms. Howe of the allegations. Ms. Howe denied the allegations indicating that she has never seen any child care children, to include Child A, sit in defecated
