    R 400.8176 · R 400.8176 Sleeping equipment. (18) All occupied cribs, porta-cribs, cots, and mats must be placed in such a manner that there is a free and direct means of egress and must be spaced as follows: (a) Cribs and porta-cribs must be at least 2 feet apart. Cribs or porta-cribs with solid-panel ends may be placed end-to- end.

    On 08/23/2023 I completed an unannounced on-site inspection to the child care facility. I met with Program Director Allison Abbott. After I reviewed the allegations with Allison Abbott, we went into Infant Room 1. I was able to view and verify the child care center is using approved porta cribs. These cribs have solid, see through ends. Because the ends are solid and do not have any slats or openings, they may be placed end to end against each other. I viewed that there was a row of four cribs that were touching end to end. On the floor was a line of tape. This tape was placed as a reference point to be two feet apart from the next row four of cribs. spaced out appropriately by the tape on the ground at two feet. The Infant 1 room had the appropriate crib set up. Each crib was in good repair. There were no blankets in any of the cribs or any other objects that would pose a safe sleep concern. The Infant 1 room was staffed with CCSM1 and four infants in attendance during the time I was present. CCSM1 stated the cribs are lined up end to end and spaced according to the tape. CCSM1 stated the cribs are checked regularly throughout the day to make sure they continue to stay spaced appropriately. CCSM1 2 stated she has never been in the classroom where she has seen the cribs not set up according to the end-to-end method and spaced according to the tape on the floor. I went into Infant 2 room and viewed the crib setup. There were six cribs in the room. These were the porta cribs with the solid see through surface on the end with no slats or spaces. There was a row of four cribs spaced end to end. Across from these cribs was another row of two cribs placed end to end. These cribs were separated by tape on the ground that measured two feet across. The cribs were placed in the same manner as the Infant 1 room. In attendance at the time of my visit were three infants and CCSM2. CCSM2 stated the cribs are set up and monitored on a daily basis to maintain compliance with them being end to end and spaced appropriately. CCSM2 stated throughout the day, staff will maintain the spacing and make sure the cribs are not moved. She has not seen the cribs set up any other way. I met with Alison Abbott who indicated the way the cribs are set up in both rooms is the way they continue to keep them. Staff monitor their placement throughout the day and make sure the cribs are properly spaced by following the tape that is placed on the ground. The tape helps keep an appropriate visual, so they are constantly maintained. On 08/21/2023 and 08/23/2023 I attempted to contact the complainant for further clarification. At the time of this report, the complainant did not return my contact attempts.

    On 09/8/2022, I conducted an unannounced on-site inspection. I spoke with the licensee designee of Solid Rock Child Care, Angela Loreth. She stated that the person who she believes filed this complaint is a disgruntled employee named, Anya Hammatova. She knows that Anya Hammatova poke to Center Director, Allison (Ally) Abbott about Child Care Staff Member Maya Sands but she doesn't know the details of the conversation. She did however state that the Anya Hammatova is no longer employed at the facility. She walked off the job on 08/29/22. The day she walked off an infant spit up on her. She asked if she could go home to change and never came back: they never heard from her again. After she 2 left, the next day she found the parents on Facebook and started messaging bad things about staff and the center. After Child A's parent made them aware of what she was doing, they contacted the police. The police reached out to the Anya Hammatova and that's why she believes she submitted a complaint against the center. On 09/08/2022 I spoke with Child Care Staff Member Maya Sands regarding the complaint. She stated that she has never "ripped" any child out of anyone's hands, and she has never "jerked" any child's neck. She knows who said that. There was an child care staff member, Anya Hammatova whom she had to have a conversation with a about holding Child A too much. The Ms. Hammatova didn't like that she talked to her about holding Child A. Ms. Sands said that she decided to inform the director about her conversation with the Ms. Hammatova. The director talked to her about how she handled explaining to Ms. Hammatova her concerns and she was proud of her. Ms. Sands denies that she ever "yelled" at Child A but there were occasions when she had to speak to him in a "stern" voice, though not in a harsh or demeaning way. She talks to all of the children the same. She doesn't put him in "timeout" but she would have him take a break. A break would be her sitting with him until he calmed down. She would use that time to talk to him about his behavior. I asked her if she yelled at him for taking his shoes off. She said that Child A never really took his shoes off. She never yelled at him if he did take them off. She also denies that she grabs children aggressively by their arms or pushes them down. I spoke with Child Care Staff Member, Elizabeth Watson regarding the complaint. She said that Ms. Hammatova raised concerns that Maya Sands was a little aggressive with Child A. Anya Hammatova, Maya Sands and Program director, Alley Abbott all sat down to talk about those concerns. She was not a part of the meeting. She has no other firsthand knowledge of the situation. However, she has never had any concerns with Maya Sands: she and Maya Sands used to work together in a classroom. Maya Sands was always gentle and kind. Ms. Watson said she knows that about two months ago they talked to Ms. Sands about her voice tone. She said Ms. Sands tone would show that she was frustrated. Her tone wasn't really harsh or mean just more short and direct, basically you could hear the frustration in her voice. Since their meeting she hasn't been showing her frustration in her voice. She had not had concerns of Maya Sands being mean or aggressive with Child A or any other children. On 09/13/2022, I spoke with Allison Abbott who stated that she is the Program Director. She stated she doesn't know anything about Ms. Sands ripping Child A out of Anya Hammatova's arms. that information was never disclosed to her and she doesn't know about allegations that Ms. Sands jerking child A's neck or her constantly yelling at him. The only thing she knows about is former employee Anya Hammatova coming to her upset that Ms. Sands told her that Child A couldn't sit in 3 her lap during circle time. She had Ms. Sands and Ms. Hammatova talk about it in front of her so that there wouldn't be any he said, she said. Ms. Sands was concerned that Child A was being held constantly and favored

    On March 10, 2022, I spoke with Child L's Father on the telephone. Child L's Father stated that on the way home from the center on March 8, 2022, Child L told him "Miss Tiffany threw me on the bed and I bumped my noggin." "Miss Tiffany" is Tiffany Henley. Child L's Father said he asked where on his "noggin" he was hit, Child L pointed to the back of his head. He asked Child L if Ms. Henley had been upset or mad at the time this happened. Child L did not respond to this question. He asked Child L if he was upset or frustrated. Child L responded, "Yes, I was and Miss Tiffany was also." He looked at Child L's head and did not see any bumps or bruises. Child L's Father stated that Child L typically naps every day at home but not regularly at the center. On March 9, 2022, he and Child L spoke with licensee, Angela Loreth at the center. He had Child L repeat what he had told him the previous day. Child L told Ms. Loreth that Ms. Henley had pushed him down the cot and he hit his head. Child L's Father stated that this is the first time he has heard any concerns about the staff in Child L's room. Typically, Miss Henley and Alexis VanWuyckhuyse are the staff in that room. He has no concerns about the supervision Ms. VanWuyckhuyse provides. He mentioned that Child L does not use the bathroom at the center. He has been told that Child L will "fight staff at the center" when asked to go to the bathroom. He is not sure if he is afraid or if there is an issue with some staff. He said that Child L will use the bathroom at home and wears a pull-up at night. He mentioned that Child L asks for privacy when he uses the bathroom at home. On March 10, 2022, I left a voicemail for Child L's Mother. At the time of writing this report, I have received a phone call in return. On March 11, 2022, I made an on-site inspection at the child care center. I spoke with licensee Angela Loreth, and child care staff members (CCSM) Alexis VanWuyckhuyse, Andrew Demers, Emma Rountree, and program director, Tiffany Henley. Ms. Loreth stated that she has not heard complaints from either staff or parents concerning Ms. Henley's interactions with children. She described Ms. Henley as "very kind, gentle, and loving." She said that Ms. Henley is the lead caregiver and Ms. VanWuyckhuyse is the assistant in the Preschool 2 room. Typically, there are two staff members in the room but on March 8, 2022, Ms. VanWuyckhuyse was in another room. Ms. Loreth said that on March 9, 2022, Child L's Father and Child L spoke to her at the center. Child L told her he was "sad" at nap time because "[Ms. Henley] was "mean"? She asked Child L why Ms. Henley was mean? He told her that Ms. Henley "threw (him) on (his) cot" and he "bumped his head". Ms. VanWuyckhuyse said she works with Ms. Henley in the Preschool 2 Room. The children assigned to that room are between the ages of 3 and 4years old. She said 3 that on March 8, 2022, she was not in the Preschool 2 room the whole day because she was helping in another room. I asked her to explain what rest time looks like in the Preschool 2 room. She explained that after lunch children will be asked to lay on their cots. They are allowed to look at books first. Some of the children fall asleep; those that do not nap are allowed to continue looking at books or playing with something quiet on their cots. She stated that children are not forced to stay on their cots. If a child is "wiggly" she will remind them to stay on their cots or may "help move their body" back onto the cot, but she does this gently. Staff will pat children's back to help them relax and rest. She stated that Child L will jump on his cot and sometimes will get upset and will "throw a fit." If this happens there is the possibility that he would hit his head on the cot. She stated Child L likes to have his pacifier and blanket with him during nap and if he doesn't have it, he will "cry and scream." Typically, Child L naps everyday as long as he has his pacifier and blanket. Ms. Van

    I conducted an unannounced onsite inspection on 1/13/2022. I interviewed Angela Loreth regarding the allegation. She stated that new child care staff members receive training upon hire which includes but isn't limited to health and safety training through the MlRegistry, training regarding the center's policies, sanitization, and an orientation of the administrative rules for child care centers. I interviewed Alexis Vanwuyckhuy regarding the allegation. She stated that she has worked for the center for approximately 2 weeks, and she currently works in the Infant 1 classroom. She reported receiving training regarding scheduling, diapering, record keeping, feeding, staff to child ratio, and capacity. Ms. Vanwuyckhuy stated that she took training on the MlRegistry, but she couldn't recall the names of the 2 training. Ms. Vanwuyckhuy stated that she knew what she was doing from prior babysitting experience, and she felt that she received proper guidance and instruction from the center. Ms. Vanwuyckhuy's hire date with the center is 12/20/2021. Per the MlRegistry, Ms. Vanwuyckhuy completed all of the required training for a child care staff member on 12/20/2021. I interviewed Maya Sands regarding the allegation. She reported working at the center for four days. Ms. Sands currently works in the Toddler 2 room. She stated that she took infant safe sleep training on line, but she had not received training regarding shaken baby syndrome, abusive head trauma, or child maltreatment. Ms. Sands explained that she had not received training regarding child discipline at the center. She would discipline children however their parents requested her to. I provided technical assistance to Ms. Sands regarding the administrative rules related to discipline. She stated that she feels comfortable working alone. She explained that she has only worked alone for one hour at a time. Ms. Sands stated that she receives adequate guidance and direction at the center. Ms. Sand's date of hire is 1/10/2022. Per the MlRegistry, Ms. Sands did not complete training regarding sudden infant death syndrome, shaken baby syndrome, abusive head trauma, recognition and reporting of child abuse and neglect, and the prevention and control of infectious diseases and immunizations until 1/13/2021. I interviewed Kaylinn Joynes regarding the allegation on 1/3/2022. Her date of hire is 1/3/2022. She explained that she began working in the Infant 2 room. She received "hands on" training at the center. Ms. Joynes explained that on her first day of employment she was alone with children for two hours. This occurred toward the end of the day, and she was alone with three children. Ms. Joynes explained that she became a little nervous as parents arrived for pick up as it was her first day on the job. Ms. Joynes stated that the parents were very supportive, and she could call the director if she needed help. Ms. Joynes stated that she received orientation at the center which covered the licensing rules for child care. However, she did not receive training regarding the discipline of children. Ms. Joynes received no training on the MlRegistry and she currently works in the Toddler 2 classroom. I asked Ms. Loreth to verify whether Ms. Joynes had taken the required training needed prior to caring for children. Ms. Loreth verified that Ms. Joynes was not connected on the MlRegistry and she had not taken the required training needed prior to working with children. I also checked the MlRegistry on 3/4/2022. Ms. Joynes did not complete infant safe sleep, shaken baby syndrome, abusive head trauma, child maltreatment, abusive head trauma, recognition and reporting of child abuse and neglect, and the prevention and control of infectious diseases and immunizations until 1/13/2022 and 1/14/2022 though she began caring for children on 1/3/2022.     On 10/05/2021 I completed an unannounced visit to Solid Rock Child Care center. I interviewed Program Director Sarah VanDeWege. Ms. VanDeWege stated she was working in the preschool 3 classroom with staff member Kayla Koeller on 9/23/2021. The children were ready to go outside on the playground. The children were counted before they went out on the playground and all the children were counted. Ms. VanDeWege stated the children were counted again when they were on the playground, and she noticed that there was a shortage of one child. Ms. VanDeWege stated she went inside and found Child H in the hallway between the two classrooms. Child H was in the hallway unsupervised for approximately 5 minutes. Ms. VanDeWege stated this incident was not reported to licensing. I interviewed Licensee Designee Angela Loreth. Ms. Loreth stated on 9/23/2021 the child care staff members were outside with the preschool classroom and there was a miscount. There was a child that was left in the hallway unsupervised. Child H was found in the hallway after being alone for approximately 5 minutes. Ms. Loreth stated staff were doing counting between transitions and are now doing name to face sheets for all transitions. She stated this policy has already been put into place. Ms. Loreth stated this incident was not reported to licensing. I explained to Ms. Loreth that this type of incident should be reported to licensing just like an accident. Ms., Loreth stated she understood and would make sure to speak with her licensing consultant if an incident like this occurs. 3 I interviewed staff member Kayla Koeller. Ms. Koeller stated she was working with Ms. VanDeWege on 9/23/2021. She stated she was working in the preschool room and the class was about to go outside. She and Ms. VanDeWege counted the children and then walked to the playground. After everyone got on the playground, they counted the children again and they were missing one child. Ms. VanDeWege then went and found Child H in the hallway. Child H was alone for approximately 5 minutes. Ms. Koeller stated she did not fill out an incident report or contact licensing because she thought Ms. VanDeWege would do it. Ms. Koeller stated the staff are all now using name to face sheets to make sure everyone is always counted during transitions On 10/21/2021 I contacted Child H's Mother. Child H's Mother stated Child H is no longer attending the center. She stated the center did not have good communication at all. She stated she removed her child from care because of her concerns On 10/21/2021 I contacted Child I and J's Mother. Child I and J's Mother stated her children are no longer attending the center, but she did not have any concerns with supervision. On 10/21/2021 I contacted Child K's Mother and left a message for a return call. As of the date of this report, I have not received a return telephone call. On 10/21/2021 I contacted Child C's Mother, Child D's Mother and Child E's Mother. They all stated they do not have any older children enrolled in care and have no direct knowledge of concerns with allegations in the preschool rooms.

    On 10/05/2021 I interviewed Program Director Sarah VanDeWege. Ms. VanDeWege stated infants are fed and changed on a regular and recurrent basis. She stated the only time that a child might not be fed or changed regularly, is because they are sleeping. She stated they check for diapers at least every 2 hours or sooner when needed. Ms. VanDeWege stated infants are fed on demand and they will speak with parents about feeding schedules. Ms. VanDeWege stated they will send diapering's and feedings to parents through the procare app the center uses. Ms. Loreth stated infants are fed on demand and according to their schedules made up by parents. She stated staff keep up on this and they also change diapers at least every 2 hours. She stated they will enter the diaper change in the procare app the center uses. Ms. Loreth stated there could be a time when a child misses a diaper change while sleeping. They will not wake the child up to change. Ms. Koeller stated she has worked in the infant room before. She stated that infants are fed on demand. She stated that infants must be changed at least every 2 hours. She stated sometimes they are not changed because they are sleeping, but they will get changed as soon as they wake up. Ms. Koeller stated everything is entered into the procare app the center uses to notify parents. On 10/21/2021 I contacted former staff member Kaitlyn Houtsma. Ms. Houtsma stated while working in the infant room, she was not able to change children's diapers every 2 hours. She stated they were so short staffed that she could not keep up. She stated this also caused diaper changes to not be done on time or entered into the app. Ms. Houtsma stated they just need more help in the infant room. Ms. Houtsma also stated she was not able to keep up with feeding babies sometimes. She stated the children would cry and she would do her best to keep feeding the children and also changing the children. She stated she could not always keep up and that she was overwhelmed with keeping track. 5 On 10/21/2021 I contacted Child C's Mother. Child C's Mother stated she has no concerns with diapering or feeding of her child. On 10/21/2021 I contacted Child D's Mother. Child D's Mother stated she has no concerns with diapering or feeding of her child On 10/21/2021 I contacted Child H's Mother and Child I and J's Mother. They stated they do not have infants in care and do not have any direct knowledge of allegations regarding infant care.

    On 10/13/2021 I completed an unannounced visit to the child care center. I met with Licensee Designee Angela Loreth. Ms. Loreth stated on 10/08/2021 the only rooms that were operating were the preschool 2 room and the infant room. She stated the infant room had received their 4th child and a 5►h child 5th was on the way. While the child was being dropped off, Program Director Sarah VanDeWege was walking down the hallway to assist in the room. Ms. Loreth stated the room maintained the staff member to child ratio. Ms. Loreth stated the preschool 2 room only had 6 children age 3 years old the entire day as she was in the room working. They were not out of ratio. I interviewed staff member Sarah VanDeWege. Ms. VanDeWege stated staff member Chloe Jarka was in the infant room and was at her limit with 4 children. While she was at her limit, she noticed another parent walking down the hallway. Ms. Jarka called the office to let them know another parent was walking in. Ms. VanDeWege was walking in the infant class to assist in maintaining the staff member to child ratio. Ms. VanDeWege stated the room was not out of ratio. She stated the only rooms open on 10/08/2021 were the infant 1 room and the preschool 2 room. The preschool 2 room only had 6 children for the entire day and Mrs. Loreth was working in that room. I interviewed staff member Chloe Jarka. Ms. Jarka stated she was in the infant room on 10/8/2021 for her first day of work. She stated she was with 4 children and noticed another mom walking down the hallway with another child. She stated she contacted the office and Ms. VanDeWege was on her way to help. As the parent was leaving the classroom, Ms. VanDeWege was in the room. The room was not out of ratio. Ms. Jarka stated she was not working in the preschool 2 room but did see that room was very low in numbers. She stated they did not have more than 6 kids and were not out of ratio. Chloe Jarka did not have any information about any other allegations about children in any other rooms, any other ages, or any other situations because this was her first day. Because it was her first day, on 10/08/2021 she would not know about anything else that happened at the center before when she stated working. I was provided with attendance records and employee records for 10/08/2021. There was no time that infant 1 or the preschool 2 room were out of staff member to child ratio. On 10/21/2021 I contacted Child C's Mother. Child C's Mother stated she has no concerns with the infant room being out of ratio. She stated there are always enough staff in the room. 7 On 10/21/2021 I contacted Child D's Mother. Child D's Mother stated she has no concerns with the infant room being out of ratio. She stated she has never seen too many children in the room. On 10/21/2021 I contacted Child E's Mother. I left a message for a return call. On 10/21/2021 I contacted Child H's Mother and Child I and J's Mother. They stated they do not have infants in care and do not have any direct knowledge of allegations regarding infant care.

    On 10/13/2021 I interviewed staff member Chloe Jarka. Ms. Jarka is the current primary caregiver in the infant room. Because of short staffing, she is a primary caregiver with Program Director Sarah VanDeWege. They are looking to hire more staff soon. I interviewed Program Director Sarah VanDeWege. Ms. VanDeWege stated staff member Chloe Jarka is the primary caregiver in the infant room now. She stated she is helping out at this time until more staff are hired. The two primary caregivers now are Ms. Jarka and Ms. VanDeWege. I interviewed Licensee Designee Angela Loreth. Ms. Loreth stated staff member Chloe Jarka is a primary caregiver. She stated Ms. VanDeWege is also a primary caregiver at this time, until they hire more staff. Ms. VanDeWege stated a new hire will be starting in a week. She stated they are working hard to get as many new staff as they can. During this timeframe, there were only 4 infants attending at the center due to staff shortages and COVID-19 cases.

    On 9/29/2021 I eted an unannounced visit to Solid Rock child care. I met with 41It*KsIiL~ members insiae me cniia care racmrues and I interviewed Licensee Designee Angela Loreth. Mrs. Loreth stated on 9/27/2021, Child F was dropped off by Child F's Mother at 7:54am. When Child F was dropped off, Child F did not have a fever as staff members are checking all children when they come into the facility. When Child F was dropped off at the center in the morning, Child F's Mother talked to staff member Mariel Palen and they talked about how Child F did the previous night, what he ate recently and how he was feeling. During this conversation to staff member Mariel Palen, Child F's Mother pointed that Child F's neck had some marks on them. Staff member Mariel Palen contacted the office and spoke with Mrs. Loreth about Child F having a temperature of over 100 degrees Fahrenheit. Mrs. Loreth came into the room and took Child F's temperature again. While taking Child F's temperature, staff member Mariel Palen pointed out the marks on Child F's neck. While taking Child F's temperature again, staff member Mariel Palen had reached out to Child F's mom to let her know Child F was running a temperature and that Child F needed to be picked up. Mrs. Loreth stated Child F appeared to have faint marks on his neck that appeared to be older. Mrs. Loreth stated Child F's mother came to the facility to pick her child up and talked to her about the marks. Mrs. Loreth stated Child F's mom did not speak to her directly about the marks. Mrs. Loreth stated she did not have any concerns with Child F's mother. She stated she did not have any concerns with any staff members being rough with any children. Mrs. Loreth stated they did not contact Child Protective Services regarding the marks on Child F. We interviewed staff member Mariel Palen. Ms. Palen stated she is the lead caregiver in the infant room. She stated she was assigned as the lead caregiver in August 2021. Ms. Palen stated Child F was at the center on Monday 9/27/2021 from about 7am until about 5:30pm. When Child F was dropped off, Child F's Mother did not talk to her about the bruising; however discussed what Child F ate, and when the last diaper was. After Child F's Mother left the center, Ms. Palen noticed bruising on Child F's left ear as well as bruising on Child F's neck that looked yellow and older. . After Ms. Palen noticed the bruising and marks, she contacted office staff and the assistant in the room, Sarah VanDeWege, to look at Child F. Ms. Palen stated Child F's Mother came back to pick Child F up because Child F was reading a temperature and could have had a fever. Ms. Palen stated when Child F's Mother came back to pick Child F up, she stated the marks on Child F's face must have been from her squeezing Child F's face too hard and pinching Child F's ear. Ms. Palen did not ask Child F's Mother why she said she squeezed her child's face or pinched her ears. Ms. Palen stated she did not think that Child F's Mother would do anything to harm her child. Ms. Palen stated she felt the statement was strange, but just figured it was an explanation to what happened. Ms. Palen stated the other staff member working in the room, Sarah VanDeWege, documented the marks and bruises on a daily sheet that was in the room, and she figured she took care of it. Ms. Palen did not contact child protective services. Ms. Palen stated she also did not call Child F's Mother to come pick him up. Although Ms. Palen stated she did not call Child F's Mother to pick the child up, it was found that Ms. Palen did contact her through the Child Care Center's automated application program. Ms. Palen asked her to pick Child F up because Child F had a fever. Ms. Palen stated she does not have any concerns with staff in the center being rough with any children. Ms. Palen stated she did not cause the marks on Child F. We interviewed Program Director Sarah VanDeWege. Mrs. VanDeWege stated on 9/27/2021 Ms. Palen came to her a

    On 9/27/2021 Child F was dropped off at Solid Rock Child Care Center with marks and bruises on his face. Ms. Palen stated she noticed the bruises and that the other staff member working in the room, Sarah VanDeWege, documented the marks and bruises on a daily sheet. I was provided with the facilities mandated reporting policy. The policy states "Our staff is required by law to report any suspected abuse or neglect, including substance abuse, to child protective services. A report must be made immediately to the central department of human services state office and followed by written documentation.': Staff at Solid Rock Child Care did not follow their policies on reporting injuries to child protective services. Although all staff at Solid Rock Child Care signed statements knowing how and when to follow mandated reporting guidelines, they did not follow these policies.

    On 09123/2021 1 contacted Child A & B's Mother who stated there was a staff member, CCSM1 who was diagnosed with and was forced to work. While CCSM1was ill with e was working in the infant room, and she was coughing and upset that hadI to continue to work. Child A & B's Mother stated CCSM1 was telling other parents that she was sick and that was why she was wearing a mask. On 09/23/2021 i contacted Licensee Designee Angela Loreth. Mrs. Loreth stated staff member CCSM1 was diagnosed with and Mrs. Loreth stated qRv~Mr mg with a mas on while Peowlas sick. Mrs. Loreth stated as soon as CCSM1 stated she was not feeling well and she told her to go home and rs. Loreth explained she also sent out emai s to families and closed the classroom CCSM1 was working in as directed by the health department. The email was sent directly from the Ottawa County Health Department's template and indicated "an individual from Solid Rock Childcare was recently diagnosed with The letter lists symptoms and advises parents to contact ea care providers if they see any changes in health of their children. This is the letter template that has been utilized across the county and signed by Dr. Paul A. Heidel. Mrs. Loreth stated she did not tell her to continue working. She stated she sent her home right away. On 09/29/2021 i completed an announced on-site visit. I met with Program Directc Sarah Van DeWege. Mrs. Van DeWege stated CCSMI stated she had and came into work. When she came into work, she stated she was Mrs. Van DeWege stated the Ottawa County Health Department was contacted and the center followed the guidelines recommended. Mrs. Van DeWege stated another staff member, Nicole Linton, stated she was sick. Staff members all jokingly said if you are sick, you must 3 work. Mrs. Van DeWege stated this was said because the center was so short staffed from sick employees, sick children, and following communicable disease protocols. Mrs. Van DeWege stated this was not something serious and they would never make someone work sick. Mrs. Van DeWege stated staff at the center all follow the health protocols set in place. These protocols state when an employee is sick, has a fever and other symptoms, they are to leave the facility and seek medical treatment. I interviewed staff member Nicole Linton. Ms. Linton stated her boyfriend contracted but she was not in close contact with him. Ms. Linton stated she did not test positive for and was only sick with a cold. She explained staff were joking saying to"connue to work because staff were so short in rooms. She stated they had to keep closing rooms because employees were getting sick, and they don't have enough staff as it is. Ms. Linton stated she would never work while sick anyways. She stated the center policy is to report when they are sick and then leave when they have certain symptoms. I received the center's policies on health and illness for both children and employees. Both policies clearly indicate when a child and staff member should leave the center and seek medical treatment. The policies also clearly indicate what policies will be followed if there is a communicable disease found in a child or at the center. On 09/29/2021 I contacted CCSMI . I left a message for a return call. On 10/21/2021 I contacted staff member Kaitlyn Houtsma. Ms. Houtsma stated if staff members are sick, they try to call other staff members to cover their work hours at the center. If they can't find coverage, they talk to management to assess the situation. Ms. Houtsma stated she only works Monday's and was not aware of CCSM1 having and working. Ms. Houtsma stated if a staff member or child get sick with or they send out emails and communications to parents. If someone has a specific communicable disease, they have to call the health department and follow what they say. Ms. Houtsma stated classrooms have been closed because staff or children have been sick in them. On 1012112021 I contacted Child C's Mother. Child C's Mother stat
