R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.
On 01/00/2022 I spoke with . He stated Child A's Mother reported the incident and stated Child A did isc ose a name, possibly_ but Child A's Mother could not understand Child A and this made Child A frustrated_ It is believed the other child is= but it is unknown what classroom they were in. On 01/06/2022 1 spoke with Child A's Mother. She explained on 12/29/2021. Child A came home and was using the restroom when she noticed Child A was not wearing any underwear. She asked Child A what happened, Child A responded underwear was in locker and was in the bathroom with in class who touched II Rld A's Mother stated she is not sure if took e underwear off ild A, but she pointed to various body parts to Child A and asked where was touched, Child A confirmed was touched on butt, leg and hip. She explained Child A did say a name and it sounded like a but she couldn't understand and this isn't a name she recognizes. She stated tried to say another Warne ia# sounded like the one Child A was giving, however hild A continued to sa no and became frustrated that Child A's Mother couldn't understand Child A's Mother has not returned to the center since the incident happened. She assktaetde d'. d A if this was a staff member that touched ~or a child and Child A said it was a friend in. class, but ■ is still learning who e people are in ■ class, therefore it is unknown if this happened with a child ■ had interacted with before. Child A's Mother explained the incident occurred during a holiday break so this could have been a child from another school who was at this center for the two-week break. She stated there is a staff member that was present when the incident happened and she is a regular staff member in Child A's classroom, but does not recall her name. She is unsure of any other staff members present this day. but there are three other staff members that work with Child A regularly, unknown names. She stated she tried to ask Child A what time during the day did the incident happen, but Child A did not understand what was being asked of10 She stated the center does have regularly scheduled restroom breaks and she isn t sure how staff members supervise children during these times, but she has seen staff members stand in the door way while children go inside the bathrooms. Child A's Mother stated . She believes Child A's underwear is still on top Rocer. ShePaefs e has no other concerns for the center. ofa On 01114/2022 I made an unannounced visit to the center and spoke with program director, Rebecca VanSomeren. She stated Child A's Mother had previously left her a voicemail that she had concerns and would like a call. She stated Child A's Mother shared Child A described a situation that occurred in the bathroom with and this child tickled ■ tummy. stated ■ does not recall if the other i tickled Child A anywhere else on body N. She stated Child A told■ mother that the other child took off underwear and placed her under-wear on top of 11 locker. Child A's Mother was not able to provide an exact name of the other chi but she tried to provide similar sounding names that she believes Child A was saying, but none of the names matched anyone in ■ classroom that day. She explained she had a meeting with Child A's Mother and Child A on 0111012022. The meeting went well, and Child A's Mother's biggest concern was understanding the situation and how it could have happened. Ms. VanSomeren stated the details of the incident did not match, such as not being able to identify a student name and she spoke with staff members to discuss the incident and believed supervision of Child A was not an issue, but no one saw anything occur. During this meeting the school safety officer was also present, and a safety plan was created so Child A would feel safe at the center. She stated she spoke with Child A in a cautious manner and the names provided did not match any names of the children class. Child A stated could identify the child's locker but then was unablien

(2) A licensee, licensee designee, or program director shall report to the child's parent and the department, directly or via phone, fax, or email, within 24 hours of the occurrence of any of the following: (b) An incident involving an allegation of inappropriate contact.
Ms. VanSomeren did not make a verbal report to the department within 24 hours of learning of an allegation regarding inappropriate touching.
