    On March 12, 2021 I received a voicemail from program director, Julie Moes concerning an incident that occurred during the after-school program. Child A was using playground equipment and fell resulting in a broken collarbone. On March 15, 2021 I received an email from Ms. Moes with the incident report. After receiving the incident report, I reviewed the center's playground inspection that was completed on November 07, 2016. Upon review, I noticed that some of the equipment was marked "not approved" on the report. And that some of the structures were approved for children between the ages of 2 to 12 years or 5 to 12 years. On March 16, 2021 I spoke with child care staff member, Hannah DeYoung on the telephone. She stated that she and two other staff were on the playground with 23 children: Ms. Erika Dykstra and Ms. Crawford. Ms. Erika Dykstra was not involved with the accident. She supervised the other children while she and Ms. Crawford helped Child A. Ms. DeYoung stated that she did not see Child A fall but heard some girls "crying" and went over to see if they "were ok." The girls were pretending to cry. She heard more crying and looked in the direction of the cries and saw Child A walking toward the school crying. Ms. Crawford went over to Child A. Child A told her that Child B pushed her down the pole. Ms. Crawford assessed Child A and did not notice any bruising or swelling. Child A informed Ms. Crawford, "her neck hurt." She had Child A sit down to rest. Ms. Crawford prepared to call Child A's Mother when she arrived to pick up Child A. Ms. Crawford let her know what happened. On March 16, 2021 I made an on-site inspection at the child care center to verify which structure Child A was using. Ms. DeYoung was present at the center. I asked her if she could show me the structure from which Child A fell. I inspected the playground structure. The structure did not contain any labeling with age restrictions. I took a photograph of the structure. These photographs are on file with this report in the center file. On March 16, 2021 I spoke with child care staff member Rachel Crawford on the telephone. She stated she and two other staff members were on the playground with 25 children at around 4:00pm. She said she saw Child A and Child B were on one of the playground structures. They were taking turns on bars that are meant to slide down. She did not see Child A fall. She noticed that Child A was hurt when another staff member, Hannah DeYoung, went over to Child A who was crying. Ms. Crawford went over to where Ms. DeYoung and Child A were and noticed that she was hurt. Child A let her know that she fell from the top of the bars and that Child B 3 pushed her. She called Ms. Moes to let her know of the accident. Child A's Mother arrived a few minutes after the accident. On March 17, 2021 I emailed program director, Julie Moes to inform her that I had completed an on-site inspection and that I had taken photographs of the playground structure. Since there was no labeling on the structure, I let her know that I would have to review the inspection report to try to determine if the structure used was approved or not. On March 17, 2021 Ms. Moes emailed me a photograph of the name imprinted on a piece of the playground structure in question. On March 19, 2021 I emailed Ms. Moes letting her know the playground inspection documented the playground structure Child A was using was not approved. The report documented the structure is approved for ages 5 to 12 years. I asked her if she had a more recent inspection report on file at the center. On March 19, 2021 I spoke with Ms. Moes on the telephone. She informed me that she did not realize some equipment on the playground was not approved. She did not have any updated inspection reports on file. She had contacted the inspector who completed the inspection in 2016 and the inspector did not have any records of an updated inspection report either.
