On 10/19/2022, I spoke to vviiaa phone, and she stated that she visited Take Flight I DevelopmentCenter and put a safety plan in place for child care staff member to not work unsupervised until the investigation is com e. stated she saw Child B who did not have any marks or bruises or at this time. On and I made an onsite inspection at Take Flight Child I Development Cente and interviewed Program Director Brianna Put1na0m/ a2s0 w/e2ll 0as2 a2s,Ta mem ers working. We first interviewed Ms. Putnam who stated she has never seen Child B's any type of reason. She stated she has seen RP"shield" Child B's mouth to stop from spitting. This looks like putting her . M front of Child B's mouth but not a ually touching Ms. Putnam stated that the children in the infant/toddler classroom have been spitting lately and Child B does this often. She stated a few weeks ago (date not remembered), Child B had a and she instantly put this information in Bright Wheel for Child B's mother to see, and Child B's mother stated that she had noticed it the day before and it was unknown how it happened. Ms. Putnam stated it did not come from or any other staff member CChhiilldd B'~ Child B's mother worried about it and agreed that it could ave come from home as Child B always has bumps and bruises because is a very active child. Next, we interviewed and she stated she usually works in the Preschool classroom and sometimes works in the Infant/Toddler classroom with Child B. works primarily in the preschool classroom, however, will 3 help out in the infant/toddler classrooms during lunch breaks as well as in the morning and evening when the classrooms are combined. She stated for discipline, she will talk to the children and remove them from the situation by redirection. ■ denied using time out or physical discipline with any of the children and stated she always sits with the children and talks to them. stated she has never been physical with any child stating, l don't get physical with my own children; I would never do that with someone else's child". When asked if she Child B , she stated 'l didn't per se". She continued with saying that Child B keptspitting and she asked o stop. Child B then continued to do it over and over and then spit in her face. hen Child B did this she stated that Child B did try to get away when she reause oes not like■ face touched, even to wi a it off if it's mess . nce d B tried to move away, she stated she let go of stated she told Ms. Putnam about this incident. She stated she told Ms. Putnam about the incident on the day it happened but she did not know the specific time or day. She stated she did not leave a Child B's., and she did not ; she just . stated that she was provided a copy of the discipline policy when Re was first hired. She stated that she knows the policy is to redirect and talk to children. and I asked Ms. Putnam if she was informed by that - Child B had spit on her, and she . Ms. Putnam stated that she was told that Child B spit on her and thatshe covered mouth to prevent ■ from spitting, but she did not Child e did not know the exact B's_ day this occurred. and I then interviewed ~, child care staff member in the Winan o ler classroom, ae she works with a couple times a week in thFRnanKdler classroom and has seen and Child B interact often. Involving the incident with Child B, she stated a B was spitting and ask to stop. Once Child B spit again, Child B's used and stated this was how Child B's She stated that only did this for a couple seconds. Child B then got upset and pulled away. is unaware of any marks or injuries on Child B from this incident. She stated she has only seen do this the one time and has never seen her put her hands on a child in the past. did not recall the date that it happened. We then interviewed , child care staff member. She stated she works with every ay in the preschool classroom and has seen Child B and interacting but has never seen any concerns or seer 4 get physical with Child

On 10/19/2022, l spoke to via phone, and she stated that she went to Take Flight Child Development Center and put a safety plan in place for child care staff member to not work unsupervised until the investigation is complete. On 1011912022, l spoke to via phone, and she stated she has spoken to Child A's mother and Child A no longer attends the center. On 10/20/2022, and I made an onsite inspection at Take Flight Child Development Center. At this time, we interviewed Program Director Brianna Putnam, and child care staff members Our first interview was with Ms. Putnam who stated that Child A attended from 0610112022 to 1010312022 and was in the infant/toddler room. Child A "over and over again". She states children in one PouRr. SW. uPtunam spoke with Child A's mother about it; and they would need to figure something out for Child A due to Child A's being at the center. Ms. Putnam advised Child A's mother that Child A woulad nno"te bae to attend the center if Ms. Putnam stated that she has never seen any staff members get aggressive with A or throw in a crib; she stated if this happened, she would know about it. A's would work in the infant/toddler~ ~hild A so she would be the Child A, if needed. i Ms. Putnam stated for discipline, staff are to remove the child from the area if the child is putting themselves or others in danger. She stated staff are to use redirection and never use time out in the infant/toddler room. In the preschool classroom, she will have children sit in a chair and talk to a staff member for discipline, but never left alone. Ms. Putnam stated that if a child is or hurting other children in the infantltoddler classroom, staff members will put the child in a crib to attend to the - child. The child will only be in the crib for a couple of minutes and are always 6 provided with toys to play with. She stated that another child had a problem as well as Child B, and they followed their and Child B Ms. Putnam stated that Child A did not have a behavioral plan witi but she did follow her= policy and that is why Child A was removed from the program. Ms. Putnam stated she will provide a copy of the centers discipline policy and policy later. Next, and I interviewed who stated that for disci line, she will talk to the child and remove e c "ome situation (redirection). denied using time out and stated that she will sit with the child and lust talk to them. stated the infant/toddler classroom did have a child everyone. Child A would be put in a crib only when the staff members were changing diapers or getting ready. Child A came four days a week and would behavior was worse when worked in the c assroom; eventually Child A's in a different classroom. stated that Child A would only be in the crib for 5-10 minutes and would be provided activities. Child A was only placed in a crib to separate Child A from the other children and so the staff could make sure the other children were okay. stated she does not know how many times Child AOwas put in a crib daily or for how long as she does not work in this room all day. stated the policy for the center is that after- a child is Come, and I then interviewed child care staff member who stated that she works with Child A in the Infant/Toddler classroom, stated that Child A used to attend the center but was removed due to She Ea stated that when Child A would child in the classroom, she would put Child A in a crib or at the table in the eats. stated that this would happen about once a day and then if a ain would be sent home.
