On 04/21/2021, I spoke to the complainant via telephone, who stated that on 04/08/2021, Child Care Staff Member A had symptoms of Covid-19 but only told a select few other staff members (unknown staff member names) and program director Mary Wolner. Child Care Staff Member A did not go home but continued working for the rest of the day. The complainant did not state the exact symptoms that Child Care Staff Member A had as she did not tell them anything. The complainant stated that on 04/12/2021, Child Care Staff Member A could not taste anything and wanted to finish the day out working but was told by Ms. Wolner to go get tested. The next day the test came back positive. Child Care Staff Member A works at the front desk but covers lunch breaks and bathroom breaks for all the classrooms; however, none of the rooms shut down and no one has had to 2 quarantine. The complainant stated that Child Care Staff Member A is back to work today and was not in quarantine for the required 14 days as their policy states she is supposed to be. The complainant stated they have not spoken to Ms. Wolner or Child Care Staff Member A about the incident personally; however, the complainant has received information from staff members when they talk about it. The complainant stated that they are concerned due to no one being quarantined in any classrooms, staff or children, that could have been exposed. On 04/22/2021 I spoke to Mary Wolner via telephone and requested a copy of the centers Covid-19 policy. I advised her of the allegations, and she stated that on 04/08/2021 Child Care Staff Member A only covered breaks for two classrooms and did not go in any other classrooms the remainder of the week. She stated sometimes Child Care Staff Member A covers bathroom breaks but is not in the room or around children/staff for more than 15 minutes. She stated that on 04/12/2021, Child Care Staff Member A was sent home to be tested for Covid-19 , and on 04/16/2021 she received the test back that stated she was positive for Covid- 19. Child Care Staff Member A came back to work on 04/21/2021 due to the health department telling her she was able to return to work. Ms. Wolner stated she spoke to her regional manager Melissa Wood, who advised her that no one needed to quarantine in any of the rooms and no rooms would need to shut down. On 04/27/2021 I made an onsite inspection at Gilden Woods Preschool and interviewed Ms. Wolner, Ms. Wood and Child Care Staff Member A. I first spoke to Ms. Wolner, who stated that Child Care Staff Member A had worked her normal schedule on 04/05/2021-04/09/2021. On the 04/06/2021 she covered lunch breaks for the 3-year-old classroom and the 4-year-old classroom. Child Care Staff Member A did not have any complaints or concerns of feeling ill during this week. I obtained the name to face sheets for all classrooms for 04/05/2021-04/09/2021, and Child Care Staff Member A was signed in at the 4-year-old room sheet from 9:30 AM — 10:30 AM and then signed in on the 3-year-old room sheet from 10:30 AM — 12:30 PM. Ms. Wolner stated that on 04/08/2021, Child Care Staff Member A came to work fine with no issues, and by mid-morning she started getting a headache. This is not uncommon for her as she is someone who experiences migraines often. She took some Motrin and felt a little better but did leave a little early that day; 4:00 PM instead of closing. On 04/09/2021, Child Care Staff Member A came into work; she did not try to call in, so it was Ms. Wolner's impression she had felt better. Ms. Wolner stated she did not personally work on the 04/09/2021, so she did not have firsthand knowledge of what Child Care Staff Member A looked like or felt like when she arrived to work. She stated Child Care Staff Member A worked all day and no one tried to call her or call Ms. Wood. 3 On Monday, 04/12/2021, Child Care Staff Member A arrived at work at 9:00 AM, and around 11:30 AM she put on hand sanitizer at the front desk where she was working, and i
