    R 400.8330 · 1 R 400.8330 Food services and nutrition generally. (4) A center shall ensure a child who has special dietary needs is provided with snacks and meals in accordance with the child's needs and with the instructions of the child's parent or licensed health care provider.

    On 05/02/2023, I received a phone call from the Program Director of Gilden Woods; Heidi Emerson, stating that Child A had eaten a veggie burger for lunch, and she and the staff were unaware that the burger had egg in it. Child A is allergic to egg and had an allergic reaction. She stated that Child A initially got rosy cheeks and then about 45 minutes later he vomited. CCSM 1 called Child A's parents, and it was determined that Child A needed his Epi-Pen and for 911 to be called. Ms. Emerson administered the Epi-Pen to Child A, and when the ambulance arrived, the medical staff were able to evaluate Child A where they 2 checked his vitals. Child A's parents arrived at the center, and Child A left with them to go home as they declined to go to the hospital. Ms. Emerson stated that she will send me the incident report. On 05/02/2023, I received the incident report regarding Child A from Ms. Emerson. The incident report stated that Child A was eating lunch when he consumed a veggie burger that had egg in it. At 12:00 PM, Child A was given Benadryl to help with the reaction but then vomited. At 1:05 PM Child A was injected with the Epi- Pen and the ambulance was called. On 05/10/2023, I conducted an onside inspection at CT DBA Gilden Woods and interviewed Program Director Heidi Emerson and CCSM 1. Ms. Emerson and CCSM 1 were interviewed together. CCSM 1 stated on Tuesday 05/02/2023, she had 12 children in care (staff to child ratio: 8)). She had one CCSM 2; however, CCSM 2 does not help assist with lunches as that is something specifically CCSM 1 does due to the allergies in her classroom. She stated that Child A has an egg allergy, and it is known throughout the center. CCSM 1 stated that Child A's mother provides a weekly lunch menu with highlighted items of what Child A cannot have or what needs to be reviewed to confirm the ingredients. She stated every time she receives the menu, she posts it and lets the office and kitchen staff know for each meal what Child A can or cannot have. On Tuesday, 05/02/2023; the menu listed veggie burger for lunch. There were no highlighted marks stating Child A could not have the burger or to review the ingredients. She stated that they have had veggie burger in the past and felt that Child A had eaten it, so it did not register with her to double check. She stated the packaging on the burger was not different, and no one was informed that it was a substitution from previously. CCSM 1 stated immediately after lunch, Child A developed a rash, and she called Child A's mother who told her to give Child A Benadryl. Child A fell asleep on his mat, and about 45 minutes later, Child A woke up and vomited on his mat two times. Ms. Emerson stated she contacted Child A's mother immediately, who advised her to give Child A an Epi-Pen and call 911. Ms. Emerson stayed on the phone with Child A's mother the entire time it took her to arrive at the center. Ms. Emerson stated that the ambulance arrived and checked Child A's vitals where he was stable at this time. Child A did not go in the ambulance and left with his parents. Ms. Emerson stated Child A's mother called back later in the day and stated that they were heading to the hospital due to a recommendation from Child A's doctor. At the hospital there were no concerns, and he was released that day. CCSM 1 stated Child A returned the next day. CCSM 1 stated that she now double- checks all ingredients every day to confirm there is not egg in any products. Child 3 A's mother now only highlights foods Child A cannot have, and everything else is reviewed. At this time, CCSM 1 and Ms. Emerson provided me with a copy of the lunch menu that Child A's mother wrote on for the week of 05/01/2023. The menu has multiple days highlighted with items he cannot have and items that need to be reviewed. On the day veggie burger is listed, it does not have any highlighted marks. On 05/16/2023, I spoke to Child A's mother via phone. She stated that veggie burger had been review
