Of the twelve children enrolled Ms. Orr only had eight child information cards from the previous center.

Throughout the investigation there were numerous times the child care home was found to be over capacity, for which was verified through observation and reviewing attendance logs. On 04/09/2026, during an unannounced on-site inspection, I observed three children in the living room of the home, which is not an approved space by the department for children. Two children were lying on the floor, and one child was asleep on the couch.
