On 04/09/2021, a complaint was received stating that on the evening of 4/8/2021 an email was sent from the center informing parents that some of the child care children are expected to quarantine as a result of a COVID-19 positive staff member at their childcare facility. The complainant stated that after they further discussed this with other families and members of the midway team it was believed that the child care staff member was allowed to work on Monday and Tuesday after someone in their home was not feeling well and they themselves started to become symptomatic because they thought it was "just allergies". This child care staff member is known to float between rooms thus exposing the entire center. Additionally, it was reported that the child care staff member has been observed without a properly fitted mask and her entire nose and majority of her mouth were exposed. On 04/09/2021 I called the complainant and did not receive a call back that day. I again called the complainant on 04/13/2021 and left a voicemail. The complainant returned my call on 04/14/2021 and left me a voicemail. I again called the complainant on 04/27/2021a nd left another voicemail. The complainant has not returned my call as of the date of this report. On 04/14/2021, I completed an unannounced inspection at the child care center. During this inspection I interviewed Licensee Designee, Heather Crandall. I informed Ms. Crandall of the allegations and she denied that the center knowingly allowed a child care staff member to work with COVID-19. Ms. Crandall confirmed that on 04/08/2021, an email was sent to families informing them that a person affiliated with the center tested positive for COVID-19. Ms. Crandall stated that she was not made aware of the staff members exposure until Tuesday, 04/06/2021. She stated that Child Care Staff Member 1 (CCSM1) came to work on Monday, 04/05/201 at 7:30a.m. and completed the required health screen and temperature check and there was no indication that CCSM1 was ill. At that time, CCSM1 reported a "little congestion" on one side of her nose but did not exhibit any symptoms that are commonly associated with COVID-19 such as cough, fever, or fatigue. On Tuesday, 04/06/2021, CCSM1 arrived at 7:30a.m., completed the health screening and temperature check with no symptoms other than minor 3 sinus congestion. Ms. Crandall stated that it was not until 9:50a.m. that CCSM1 checked her temperature and realized that her temperature had risen. CCSM1 immediately informed the school secretary, Sam Merritt, and explained that her temperature was higher and CCSM1 was immediately sent home. On Wednesday, 04/07/2021, CCSM1 was tested for COVID-19 and received a positive test result on Thursday, 04/08/2021. Ms. Crandall was unsure if CCSM1 had ill family members and CCSM1 had not reported any exposures to COVID-19 positive individuals. Ms. Crandall stated that after learning of the positive test result, she and Ms. Phinney went two days back into CCSM1's schedule to identify which children were directly exposed. Ultimately, seven children were quarantined. She added that there had been no other reports of anyone testing positive for COVID-19 after their exposure to CCSM1. Ms. Crandall stated that the child care center has a strict COVID-19 screening procedure for all child care staff members and children. The procedure includes everyone completing a health questionnaire along with having their temperature taken upon arrival at the child care center. Mrs. Crandall immediately notified parents of potential exposures by sending a building wide email. On 04/27/2021, I conducted a phone interview with Program Director, Jennifer Phinney. Ms. Phinney denied allowing a child or a child care staff member to work at the facility while ill with COVID-19. She added that she is a parent, and she would not want to potentially expose her child or any other child. Ms. Phinney stated that on Monday, 04/05/2021, she was working from home when she
