Upon my arrival, all three licensed classrooms were outside on the playground. There was a total of 45 children, including 9 children from the special education classroom, which exceeded the maximum group size of 30 children. The child care staff members said their outdoor play time is all at the same time and it coincides with the special education classroom's playtime.

On 11/17/2021, I conducted an unannounced on-site inspection. interviewed Michael Beattie (school principal). Mr. Beattie stated that Jodi Sarkella oversees all the school's preschool/PreK programs. Mr. Beattie placed a call to Ms. Sarkella on speaker phone while I sat with him in his office. Ms. Sarkella indicated that she is in the process of submitting her transcripts to the assigned licensing consultant, RaSheeda Mitchell. Ms. Sarkella stated that at this time there is no approved program director. I spoke with Ms. Sarkella and reviewed 2 all program director qualifications and responsibilities, including that the program director needs to be on-site at least 50% of the time that children are in care and not less than a total of 6 hours per day. Ms. Sarkella said that she will work on getting Ms. Mitchell the required documents to become the approved program director. On 01/07/2022, I spoke with Ms. Mitchell via telephone. Ms. Mitchell stated that she never received the required documents from Ms. Sarkella. Ms. Mitchell also said the program recently appointed a new licensee designee, Jennifer Herbstreit. On 01/07/2022, I spoke with Ms. Herbstreit via telephone. Ms. Herbstreit informed me that Ms. Sarkella resigned and is no longer with the school district. Ms. Herbstreit stated that she is in the process of interviewing candidates to fill the vacant program director position. I reviewed the required documents that need to be submitted to the department to have the candidate approved as the program director. Ms. Herbstreit stated that she would submit these documents to Ms. Mitchell once she has made hiring decision.

On 11/17/2021, I walked though all three classrooms (the tuition based, GSRP, and Head Start) to verify the ratios in each room. No classroom was out of ratio, they were as follows: Tuition-based classroom (3 years of age until 4 years of age-requiring a 1:10 ratio) - 2 staff members with 7 children. GSRP classroom (3 years of age until 4 years of age-requiring a 1:10 ratio) - 2 staff members with 8 children. 3 • Head Start classroom (3 years of age until 4 years of age-requiring a 1:10 ratio)- 3 staff members with 9 children. I interviewed Regina Feves (lead caregiver-tuition-based classroom), Louise Kavanaugh (lead caregiver-GSRP), and Danyl Grosser (lead caregiver-Head Start classroom), Interviews were conducted in the classrooms. Ms. Feves stated that she has never seen her classroom out of ratio. Ms. Feves indicated that they have not had any staffing shortages due to Covid-19, or any other reason. Ms. Feves said that if they did not have enough staff members to cover ratio, the classroom would just close until the staff members were able to return to work. Ms. Kavanaugh stated that classroom has not been out of ratio at any time during the current school year but added that the classroom would close in the event they were unable to maintain ratio due to staffing issues. Ms. Grosser stated that the classroom always operates in ratio. Ms. Grosser indicated that the Head Start classroom had to close on two separate occasions due to staff exposure to Covid-19. Ms. Grosser said that the classroom would not be allowed to open if they did not have enough staff members to cover ratio. While on-site I obtained copies of the attendance records for 11/15/2021 and 11/16/2021 for all three classrooms. The attendance records showed the following: • Tuition-based classroom: On 11/15/2021- 2 staff members with 7 children. On 11/16/2021- 2 staff members with 7 children. • GSRP classroom: On 11/15/2021-1 staff member with 8 children.
