Ms. Thomason laid on top of Child A (2-year-old) when she would not lie down on her cot during nap time on 02/27/2023. Child Care Staff Members (CCSM) change the children's diapers on the floor without any form of padding underneath them. Ms. Thomason told Child B (4-year-old) that she was going to send her away and she would never see her mother again. Child A was bitten in the toddler classroom, and Child A, B, and C's Mother did not receive an incident report.

On 03/07/2023, Child A, B, and C's Mother stated in an email that Child C (6-year- old) informed her that Ms. Thomason had laid on top of Child A when she would not lie down on her cot during nap time. When she asked Ms. Thomason about what Child C had stated, Ms. Thomason stated that she covered up Child A with a blanket during naptime and that Child C is an over-exaggerator. Child C was in the toddler classroom with Child A during naptime when the incident occurred. On 03/09/2023, Ms. Thomason stated that she would never lie on a child for any reason, and she did not recall having a conversation with Child A, B, and C's Mother. On 04/19/2023, Ms. Freimark stated that Ms. Thomason would never lie on a child for any reason. Ms. Freimark was not working on 02/27/2023; she was off the full week. She stated that the center does not have cameras. On 04/19/2023, CCSM A stated that she has never worked with Ms. Thomason during nap time, so she has never observed her interactions. CCSM A does not think Ms. Thomason would lie on a child during nap time. Child A was in the toddler classroom, and she did not have a hard time during naptime. Child A would go on her cot and CCSM A would rub her back and she would go right to sleep without any resistance. CCSM A was working on 02/27/2023 in the toddler classroom and CCSM C relieved her during her break. Ms. Thomason did not work in the toddler classroom on 02/27/2023; she worked in the preschool classroom. On 04/25/2023, CCSM B stated in her text message that she has never observed Ms. Thomason lie on a child during naptime. The children will lie on their cots, and the CCSM's will rub their backs when they need extra help to go to sleep. On 04/25/2023, CCSM C stated in her text that she has never witnessed anyone lying on a child. On 05/02/2023, Ms. Thomason sent a copy of the daily attendance for the CCSM's and children that were present on 02/27/2023. According to the daily attendance 3 Child C was not present at the center and Ms. Freimark was not working on 02/27/2023.

On 03/07/2023, Child A, B, and C's Mother stated that she has observed the child's diapers being changed on the floor with no pad underneath them in the toddler classroom. On 03/08/2023, Ms. Freimark stated that the CCSM's have changed the children's diapers on the carpeted area with a diaper changing pad under their bottom. The diaper pad is washed, rinsed, and sanitized (3 step-process) after each diaper change. There is a diaper changing table in the classroom; however, Ms. Freimark stated that the children are too heavy to lift. I informed Ms. Freimark that the children's diapers cannot be changed on the floor, and if a child is too heavy their diaper can be changed while standing in the bathroom area that is located within the classroom. She stated that she would instruct the CCSM's to use the diaper changing table to change the children's diapers. On 03/09/2023, Ms. Thomason stated that children's diapers are changed on the floor when they are too heavy to pick-up and a pad is always placed under the child. On 04/19/2023, CCSM A stated that previously, before my onsite inspection on 03/07/2023, she did change the children's diapers on the floor with a diaper pad under their bottom. Currently, all the children are being changed on the diaper changing table. On 04/25/2023, CCSM B stated that the children in the preschool classroom have always been changed on the tile floor with a pad underneath them and they used the three-step process on the diaper pad after diapers changes. Diaper wipes are used for each diaper change and the wipes are provided by the parents. 4

On 03/07/2023, Child A, B, and C's Mother stated that Child B said that Ms. Thomason told her that she was going to send her away and she would never see her mother again. Child A, B, and C's Mother asked Ms. Thomason and she stated that she had said that to Child B. No reason was provided as to why the statement was made. On 03/09/2023, Ms. Thomason stated that she did not tell Child B that she was going to send her away and she would never see her mother again. Nor would she ever say anything like that to a child. She has no idea why this would be said about her. On 04/19/2023, Ms. Freimark stated that Ms. Thomason would never tell a child that they would not see their parent under any circumstances. On 04/19/2023, CCSM A stated that she has never heard Ms. Thomason say that to a child and she did not believe that Ms. Thomason would ever say that to a child. On 04/25/2023, CCSM B and CCSM C stated that they have never heard Ms. Thomason say that to a child and they do not believe that Ms. Thomason would ever say that to a child.

On 03/07/2023, Child A, B, and C's Mother stated that Child A was bitten by another child last year in the toddler classroom. She does not know the exact date and she did not receive an incident report from the center. Child A, B and C's Mother spoke with Ms. Thomason about her concerns and Ms. Thomason stated that she was not aware that Child A was bitten because she did not cry. On 03/08/2023, Ms. Freimark stated that incident reports were always provided to parents when a child is injured at the center. She was not able to locate the center's incident reporting policy. The center does not have cameras. She was not aware of Child A being bitten at the center and if Child A was bitten, an incident report would have been completed and provided to the parent. On 03/09/2023, Ms. Thomason stated that she was not aware of Child A receiving an injury at the center. She provided the centers incident reporting policy. The incident reporting policy stated that the parents will be informed when they arrive at the center and asked to sign and return the incident report. Depending on the severity of the incident, the parents may be contacted via phone to inform them of the situation. On 04/19/2023, CCSM A stated that she did not have any knowledge of Child A having been bitten in the toddler classroom. Incident reports are provided to the parents when an injury occurs. On 04/25/2023, CCSM B stated that Child A was bitten last year, and Child A's Mother was given an incident report when the incident occurred. CCSM B provided the incident report that was completed 03/15/2022. The incident report stated, "another child was trying to take a toy from Child A and the other child bit Child A on the left arm." The incident report was signed by two CCSM's and Child A's Mother. On 04/25/2023, CCSM C stated that she did not have any knowledge of Child A having been bitten in the toddler classroom. Incident reports are provided to the parents when an injury occurs. 7 On 05/02/2023, Ms. Thomason stated that Child A was bitten last year, and she informed Child A, B, and C's Mother and she was not upset or concerned last year when the incident occurred.
