On 01/19/2022 I spoke to the complainant. The complainant stated that room A-4 at Holly Elementary School did not have heat on 01/11/2022. The complainant stated that administrative staff told child care staff members not to send the children home. The children remained at the school. On 01/21/2022 I completed an unannounced onsite inspection. I interviewed child care staff member Sarah Rood (School Age Staff Member) and Holly Elementary Custodian Nora Samson. Staff members that worked in room A-4 were not available for interviews. They do not work on Fridays. Both Ms. Rood and Ms. Samson stated that children were never in the classroom when the heat was not working. Both staff stated that children were relocated to room 12 for the day. This room is an approved licensed space. It was determined that the room did not have heat prior to the school day; therefore, the plan was to have children attend programing in room 12. Children went into room 12 as soon as they arrived at Holly Elementary on 01/11/022. Both staff confirmed that children were in room 12 on 01/11/2022. Ms. Samson stated that maintenance was called on the same day the heat went out. It was determined the heat problem was due to a computer control issue. Ms. Samson said this issue has been resolved. I observed room A-4. I took the temperature of the classroom. The reading was 65 degrees Fahrenheit. I observed room 12. The room consisted of at least three learning areas and had sufficient materials to support a classroom of children. The room was clean and organized and free of safety hazards. On 02/23/2022 I interviewed child care staff member Chrissandra Padilla (Assistant Early Childhood Coordinator). She confirmed a heating issue occurred on 01/11/2022, however children were never present in the classroom when it had no heat. They started the school day in Room 12 not Room A-4.
Children were not sent home when temperatures in the building could not to maintained. • On Wednesday, January 12, 2022, a child was sent to tuition preschool and latchkey without a child information card in the building. • There are three children in the tuition preschool classroom that do not have child information records.

The latchkey program is out of ratio (for sure in the morning and unsure about afterschool). There are children as young as 4 years old and only one worker. The child care staff did not provide appropriate care and supervision when Child A removed his clothes, defecated on the carpet, and began to play with it and cover himself in it.

On 01/19/2022 I interviewed the complainant. The complainant stated that Child A, Child B, and Child C do not have child information records on file. The complainant was concerned that staff are not able to verify individuals with permission to pick up children from care if they do not have child information records on site. On 01/21/2022 I completed an announced inspection at Holly Elementary. I interviewed Heather Augustine (Tuition Preschool Lead Caregiver) and Ms. Rood. Both Ms. Augustine and Ms. Rood confirmed that Child Information Records are not on file at the center for all children. Both staff stated that staff members at the Carl Richter Campus (located across the street from Holly Elementary) complete child files at their location and then send them to Holly Elementary School through inter- district mail. Both staff stated they were concerned that they did not have child information records on file for children at the time they started care. Ms. Augustine said Child A, Child B, and Child C are currently enrolled in tuition preschool. She stated that none of these children had child information records at the center on their first day of care. Ms. Augustine said Child A started on 01/04/2022 and a child information record was not received for roughly two weeks. Child B started on 01/10/2022 and a child information record was not received until the following week. Child C started on 01/04/2022. A child information record was not received. On 01/12/2022, Ms. Augustine provided Child C's mother with a child information record to complete. The card was returned on 01/18/2022. On 02/23/2022 I interviewed Ms. Padilla. She stated she was not sure about specific files but understood that files were not reaching their destination in a timely manner due to the inter-district mail only running one time a week. She was aware that center staff should have child information records as soon as the children start. 4 On 3/10/2022 I spoke to Amber Carmichael. Ms. Carmichael is the Early Childhood Education Secretary. She works at the Carl Richter Campus. Ms. Carmichael stated that she is responsible for managing child files for the early childhood programs in the district. She stated as part of the enrollment process she provided paperwork to families to complete and return. This paperwork packet includes a child information record for families to complete. Once the paperwork is submitted by the families, Ms. Carmichael makes copies of child information records, parent permissions, health documents, and other pertinent information and sends this information to the school and classroom in which the child will be enrolled via inter-district mail. Ms. Carmichael informed me that inter-district mail is only operating once a week and has been since January 2022. I asked Ms. Carmichael about child information records for Child A, Child B, and Child C. She stated that Child A's family was told to start tuition preschool by another district staff. The family did not follow the typical enrollment process. She did not recall which staff member told Child A's family that he could start preschool without going through the enrollment process. Child A started class without going through the enrollment process. Once Ms. Carmichael was aware that Child A was attending tuition preschool and latch key, she provided an enrollment packet to Child A's family to complete. When the packet was returned, Ms. Carmichael sent information to Ms. Augustine via inter-district mail. Child B's information card was obtained at enrollment and sent to Ms. Augustine via inter-district mail. Ms. Carmichael stated it took a long time to reach Ms. Augustine because mail is only being delivered once a week. Child C's information card was not obtained at enrollment. Ms. Carmichael stated that Child C just started showing up at preschool. He attended preschool five times in February 2022. On 11/17/2021 I interviewed the complainant via telephone who stated the latchkey program is always out of ratio in the morning. The complainant stated that latchkey is out of ratio two to three times per week. She has two children in the latchkey program and her daughter is four years old. Her children are usually one of the first to arrive in the morning and when she drops her children off there is only one child care staff member present. On 11/18/2021 I conducted an unannounced on-site inspection at Holly Elementary School. I observed 12 children present and three child care staff members. I interviewed Licensee Designee Jodi Sarkella and child care staff member Terri Rodgers. I informed Ms. Sarkella of the allegation and she stated they have had some staffing issues but that she would be present at Holly Elementary everyday until they could hire more staff. I did not observe any four year old children. I interviewed Ms. Rodgers who stated that they have approximately 45 children enrolled in the program and up to 20 children who come in the morning with more children in the afternoon. I asked Ms. Rodgers if they have any four-year-old children in the morning latchkey program which affects their ratio. Ms. Rodgers stated that she does not have any four-year-old children in her room. The younger children are 3 in the preschool latchkey room next door. Ms. Rodgers stated the latchkey program begins at 6:00 am and she is the only child care staff member until the paraprofessional arrives at 7:00 am. Ms. Rodgers works from 6:00 am to 8:30 am, the second child care staff member Jillian Hart arrives at 8:00 am. Ms. Rodgers stated the number of children varies from day to day in the morning latchkey and she has no concerns regarding ratio. I asked Ms. Rodgers what happens if the paraprofessional is absent, and she is alone with children before Ms. Hart arrives to work. Ms. Rodgers stated that if she has too many kids or needs help the preschool teacher next door sometimes comes over to assist her. Ms. Rodgers provided me access to daily sign in sheets and child information records, which I took photos of for verification. On 12/15/2021 I spoke with Ms. Rodgers via telephone and asked her to provide to me additional child information records to verify children's ages. She informed me that Ms. Sarkella is no longer their licensee designee and that the Assistant Superintendent of Curriculum and Instruction will be taking her place in the interim. Ms. Rodgers also informed me that they have hired an additional child care staff member Teresa Jenkins who works from 6:30 am to 8:30 am. On 12/16/2021 I interviewed child are staff member Jillian Hart via telephone. I asked Ms. Hart about their ratio and she stated they have a "normal" number of kids in the morning, approximately 20 children. Ms. Hart stated that they do not have any four-year-old children in the morning, but they do have some kindergarten age children. She did not have any concerns about ratio. Ms. Hart stated that when she comes in to work, she takes the children to breakfast in the cafeteria then the school day begins. Ms. Hart also works in the preschool classroom which begins at 8:30 am. On 12/17/2021 I interviewed Jennifer Herbstreit via telephone. Ms. Herbstreit informed me that she will be taking Ms. Sarkella's place as the designee as Ms. Sarkella no longer works for the program. I informed Ms. Herbstreit of the allegations and that I had conducted an on-site inspection and interviewed her staff. On 01/10/2022 I conducted an unannounced on-site inspection. I interviewed child care staff member Sarah Rood. Ms. Rood stated that sometimes she comes over to assist Ms. Rodgers if she was the only child care staff member with the children. I asked her how often that occurs, and she stated whenever she needs help with the children or if the paraprofessional is absent. While speaking with the complainant on 11/17/2021, she informed me of an incident that occurred in the morning that day. The complainant stated Child A who has an Individualized Education Plan took his clothes off and defecated on himself. After Child A defecated, he wiped feces all over himself and the area in the latchkey room. The complainant stated that the other children were not removed from the room and had to wait until someone could come to clean the area. The complainant stated that Child A did not have an assigned paraprofessional and Child A is a runner and has recently begun streaking. During my on-site inspection on 11/18/2021 I asked Ms. Rodgers about Child A's defecating incident. Ms. Rodgers stated that when Child A's mother brought him to latchkey, she informed her that she was alone, and that the child's paraprofessional was not coming in. Child A's mother told her that he could go to his room (a room connected to the latchkey room that has been designated Child A's special room). Child A went to the room, took off his clothes then defecated on the floor. Ms. Rodgers stated that the other children witnessed Child A take his clothes off, defecate, then smear the feces around the room. Ms. Rodgers stated that she had child care staff member Ms. Rood from the preschool room come over to assist her with the other children. The staff used brown construction paper to cover the windows to shield Child A and the feces from view. Ms. Rodgers stated that she called Child A's mother and she arrived approximately 30 minutes later and took Child A home. Ms. Rodgers said Child A is a runner and sometimes runs out of the latchkey room into the hallways of the school. Child A's paraprofessional is employed by the school district and when the paraprofessional is absent, they do not have a substitute who works with Child A. Ms. Rodgers stated that it can be difficult to manage Child A's behavior along with managing the other children. Ms. Rodgers stated that she is not trained to support Child A and is unaware of what his behavior A plan states in his Individualized Education Plan. Ms. Rodgers begins her workday at 6:00 am, Child A's paraprofessional begins his workday at 7:00 am, and child care staff member Ms. Rodgers expressed her discomfort with having Child A in the latchkey program when the paraprofessional is absent but was told that she cannot turn children away. Ms. Hart begins her workday at 8:00 am. Ms. Rodgers stated it can be stressful to manage Child A's behavior when the paraprofessional is absent and she does not have consistent assistance until Ms. Hart arrives. During my onsite inspection on 11/18/2021, I observed Child A's designated room. It is attached to the latchkey room with clear glass surrounding the door. The construction paper was taped to the clear glass windows preventing a full view of the room. The school age latchkey room has an attached kitchenette with a restroom. The preschool latchkey room is adjacent to the school age latchkey room with an open doorway/pass thru. There is an exterior door that parents enter to drop their children off and an interior door leading to the school. While interviewing Ms. Rodgers, Child A had run out of the room and the paraprofessional ran after him. Child A and the paraprofessional did not return to the room by the end of my inspection. I interviewed Ms. Rodgers for approximately 20 minutes. During my conversation with Ms. Rodgers on 12/15/2021, Ms. Rodgers stated that they have hired a new child care staff member who arrives to work at 6:30 am and she is by herself for 30 minutes in the morning. Child A's paraprofessional arrives at 7:00 am and has had no further incidents. They now keep the doors locked to prevent Child A from running out of the room. During my interview with Ms. Hart on 11/16/2021, she stated she was not present during Child A's incident as it occurred before she arrived to work. A child care staff member accepted Child A into the latchkey program on 11/17/2021 knowing the assigned paraprofessional would not be in and she could not provide appropriate care and supervision to Child A. None of the child care staff members had training to handle Child A and provide appropriate care and supervision.
