During the course of the investigation two different documents of staff attendance were provided. Attendance provided on 10/20/2025 did not include Teacher 4. Attendance provided to Katrice Sweet on 12/23/25 included Teacher 4. PA stated that they were asked to remove Teacher 4, which is inaccurate. PA also stated that the same attendance forms provided to both Jayne Silvers and Katrice Sweet were the same forms.

Teacher 1 left Child A (age 2), whom she was assisting in the bathroom, in order to redirect another child. Child A then shut the door, leaving him unattended. According to interviews, teachers are not provided the support and resources needed to provide appropriate care and supervision of all children at all times.

CCSM 2 was not providing appropriate supervision during the unannounced on-site inspection. He was wearing headphones and watching is phone and did not hear CCSM 1 try to get his attention.

On 10/03/2022, I spoke to the owner of Little Kingdom Warriors Academy, Carrie Esparza the regarding the complaint. Ms. Esparza stated that on 09/22/2022 Child A's Mother picked Child A up early to take her to a medical appointment. When she brought Child A back to the center, she was crying hysterically saying that the doctor was going to call Michigan Department of Health and Human Services (MDHHS) on her because she saw a yellowing bruise on her side. Ms. Esparza explained she was not aware of any bruise, but she immediately went to ask the lead caregiver, Mirjam Kuipers, if she knows if Child A had a mark or bruise or if anything happened to Child A while in care. She informed Child A's Mother that Ms. Kuiper reported that she didn't notice any marks or bruises on Child A and didn't know of anything happening to her. Child A's Mother began to cry again as she left. Ms. Esparza said she told Program Director, Asha Groenhof to go outside to talk to her. On 10/03/2022, I spoke with Child Care Staff Member, Mirjam Kuipers regarding the complaint. She stated that she never saw a bruise on Child A. She said that if she did would have taken a picture of it and entered it into bright wheels. She said that Child A also goes into Infant 1 room. Ms. Angelique Smunty was the lead in that 2 room but she left the child care center for a better job. She said the only thing she can think of is that maybe she rolled over on a toy or maybe one of the older infants could have grabbed her. I spoke with Patrick Arthurs regarding the complaint. He said he has no knowledge of what happened; he just heard about it today. He stated that infant room 2 is not his normal room. He is usually in infant room 1 or Toddler room 2. He said that to his knowledge she was not injured in his classroom or any of the other rooms that he works in. He never heard any crying from her that would suggest that she was hurt. I also spoke with Program Director Asha Groenhof regarding the complaint. Ms. Groenhof stated that on 09/22/22 she talked to Child A's Mother after Ms. Esparza told her that Child A's Mother was in tears because the doctor was going to call MDHHS on her for a small bruise that was barely noticeable. Ms. Groenhof said that the following day on 09/23/22, she observed the bruise on Child A. The bruise was on the left side, and it was barely noticeable; you could have easily missed it. She did not take a picture of it because she could barely see it. She asked staff if they knew anything regarding Child A getting hurt or having a bruise. They said, no. They didn't see anything or do anything to hurt her. Also, no one reported anything to her about Child A being hurt or getting injured or noticing any marks or bruises on her. On 03/03/2023, I spoke with Child A's Mother regarding the complaint. She stated that everything was fine and was wrapped up with MDHHS with no findings. Child A had a inch by inch light yellow bruise on her left side. It was very small. She 1/4 1/4 wasn't really sure how she got it but thinks maybe she got pinched with the car seat. She had no concerns with the child care from the beginning and she told the doctor and MDHHS that. She repeated again that she had no concern that anything had happened to her daughter while in child care. She stated that she had an older daughter age who has been attending the children since she was 12 weeks old, and she is 2 years old now. She has not had any issues regarding her care at this 1/2 facility.

On 05/20/2022 I completed an unannounced visit to the child care center. I interviewed Program Director Asha Nemec. Ms. Nemec stated she occasionally provides care in the infant room. Infants are not placed in bouncy seats. She stated infants are always placed to sleep in cribs. Infants use bouncy seats, but not for sleeping. I interviewed Licensee Designee Carrie Esparza, who stated she occasionally provides care to infants. Ms. Esparza stated infants do not sleep in bouncy seats. She stated children sleep in cribs. I interviewed staff member Jen Hendrick who stated she provides care to infants. Ms. Hendrick stated infants do not sleep in bouncy seats. She stated infants sleep in cribs. I interviewed staff member Angie Smutny, who stated she works in the infant room. She stated infants do not sleep in bouncy seats. She stated infants sleep in cribs. On 05/20/2022 I contacted staff member Marijam Kuipers by telephone. Ms. Kuipers stated she works in the infant room, she explained infants do not sleep in bouncy seats. She stated infants sleep in cribs. On 05/20/2022 I contacted Child A's Mother and Child B's Mother. Both stated they have not seen any infants sleeping in bouncy seats. They have seen infants sleeping in cribs.

I interviewed Program Director Asha Nemec. Ms. Nemec stated no infants in care are swaddled over 2 months of age. She denied Child C, Child D, and Child E are swaddled. I interviewed Licensee Designee Carrie Esparza. Ms. Esparza stated children are not swaddled over 2 months of age. She denied Child C, Child D, and Child E are swaddled at the center. I interviewed staff members Jen Hendrick and Angie Smuty, Both staff members stated infants over 2 months of age are not swaddled. Both staff members denied Child C, Child D, and Child E are swaddled. I contacted staff member Marijam Kuipers. Ms. Kuipers stated children over 2 months of age are not swaddled. She stated Child C, Child D, and Child E are not swaddled because they are over 2 months of age. I contacted Child A's Mother, B's Mother, and Child C, D, E's Mother. All parents denied witnessing any children being swaddled.

She denied they are used for tummy time because that is not safe. Infants are placed on their tummy for tummy time without being on a soft surface. They are always under direct supervision. I interviewed Licensee Designee Carrie Esparza. Ms. Esparza stated boppy pillows are used for other activities like helping to sit but are not used for tummy time. She stated infants are placed on their tummy for tummy time without being on a soft surface and under supervision. I interviewed staff members Angie Smutny and Jen Hendrick. Both staff members stated boppy pillows are not used for tummy time. Infants do tummy time in the infant room on the floor under supervision. They are on a mat. Ms. Kuipers stated boppy pillows are not used for tummy time. She stated that is a safety issue. Child A's Mother, Child B's Mother, and Child C, D and E's Mother all denied witnessing infants using boppy pillows for tummy time.

On 05/20/2022 I interviewed Program Director Asha Nemec. Ms. Nemec stated staff member to child ratio is maintained in the infant room. When there are children that come, staff might have to move staff around to help cover. She stated staff are good at communication when they are close to being full. Ms. Esparza stated she is not aware of the infant room not maintaining the required staff to child ratio. She stated if staff need help, they will reach out by phone and make movements to help cover areas. Ms. Esparza stated they are constantly looking for help with staff and new employees in the infant rooms because staffing is an issue in that area in general. Ms. Hendrick stated she has been in the infant room when the staff to child ratio was not maintained. This has happened when someone called in and there is not a replacement. She has worked in the room alone with five to six infants for her whole shift. Management try their best to cover, but sometimes they do not have the amount of staff needed to maintain ratio. Ms. Hendrick stated they are trying to hire more people. Ms. Hendrick stated she does not remember what days or times this has happened. Ms. Smutny stated she has been in the infant room when the staff to child ratio was not maintained. If a person calls in, the room will be out of ratio for the day. She has worked in the room by herself with six infants. She stated they are trying to find staff, but they keep losing people. She stated the room is not out of ratio daily, but it has happened from time to time. She does not have specific dates and times for when this has happened. Ms. Kuipers stated the infant room has not maintained the required staff to child ratio recently. People keep calling in and quitting so it makes things hard. There have been times where there is only one person for six or seven infants. She stated they are trying to find more help in the infant room, but it has been hard. Ms. Kuipers stated she does not remember what exact dates and times this has happened. Child A's Mother, Child B's Mother, and Child C, D and E's Mother all denied noticing any concerns regarding the number of staff to children in the rooms during their pick ups and drop offs.

Ms. Nemec stated infants are never fed by bottle propping. She stated infants are held and fed appropriately. Ms. Espara stated there is not bottle propping in the child care center. Infants that need help holding bottles are assisted by holding the bottles while they are being fed. Children that can hold a bottle themselves, are given the opportunity to hold the bottle. This can be a variety of children throughout their age ranges and developmental skills. They are in highchairs, or miniature seats at tables. Jen Hendrick and Angie Smutny both denied infants are fed by bottle propping. Both indicated infants are fed based on how their developmental abilities are. Some are fed in high chairs and hold themselves, some are sitting at the table, and some still need help with feedings. Ms. Kuipers stated they do not bottle prop at the child care center. She stated infants are held and fed. Infants that can hold bottles are encouraged to do so, but they never prop bottles. Child A's Mother, Child B's Mother, and Child C, D and E's Mother all denied seeing bottle propping at the child care center. 7
