On 11/17/20 I received a telephone call from Janet Underwood, program director, reporting Child A, female age 2, had been left on the playground and found by Witness 1. On 11/19/20 I received the incident report documenting this incident. I was also informed that the classroom had been closed due to a COVID-19 positive case. On 11/23/20 I spoke with Child A's Mother. She said she was informed of the incident and felt that it was an accident. Her understanding of the incident was Child A was left on the playground for approximately 3-5 minutes alone, and found by another person coming to the child care facility. She did not have any concerns about Child A's care or the supervision provided to her. On 11/24/20 I spoke with Witness 1 by telephone. She explained she arrived at the child care facility and after picking up her child, she saw Child A crying on the playground. She spoke to Child A and verbally reassured her and then went to the classroom door, for her child, and informed them Child A was left on the playground. A child care staff member came and got Child A from the playground. She said Child A's hand's were red and it was cold out, which left her concerned about how long she had been outside or how long it would have been until it was realized she was not inside with her class. She did speak with Ms. Underwood about the incident, her concerns, and a protocol on checking for children when returning inside. On 12/08/20 I spoke with Adrianna Almazan, child care staff member, by telephone. She explained that on the day of the incident she and Taylor Walters, child care staff member, were outside with Child A and the rest of the class. While they were outside another child care staff member brought Taylor Walters the telephone as there was a call from a parent. Taylor Walters went inside, standing in the doorway to the class, as the classroom doors exit to the playground. Ms. Almazan explained at the same time, she began having the class line up to go inside. She called out to Child A to line up and then had all the children go inside. She told Taylor Walters, as she was entering the classroom, Child A was outside, however she was still on the phone. There was a lot of movement and distractions which led to them both not realizing Child A did not come in with the rest of the class. They realized Child A was not with the class when another child care staff member told them Witness 1 was with her outside on the playground. Adrianna Almazon stated she then went outside and brought her in with the rest of the class. She thought Child A was outside alone for a "few minutes." 3 On 12/08/20 I spoke with Janet Underwood by telephone for an exit conference. I informed her of the recommendation for a 2nd provisional. We discussed procedures for moving children and other steps to implement when there is the potential for increased distraction for staff.
