    R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    On 2/18/2021 the department received a complaint stating on 2/18/2021 Child A was observed acting strangely during two separate incidents. It was reported that Child A was standing still in an awkward stance, staring blankly. She was unresponsive and would not make eye contact. This happened in the morning and again during lunch service. Parents were notified following the second incident and made Child A a medical appointment with her physician. There were concerns that parents were not informed of the seriousness of the situation and that emergency services should have been called. On 02/23/2021 I completed an unannounced onsite inspection at the facility and spoke to program director Rebecca Ryskamp, curriculum coordinator Jamie Nink and CCSM's Jennifer Norton, Sara Deleon, Alissa Sweeney and Sydnie Avery. Ms. 2 Ryskamp was familiar with the incident and stated she was present at the center when Child A had her first incident. She stated she was walking past the Pedal classroom and saw Child A standing, staring blankly, and pointing at a toy. She asked Ms. Norton, who is the lead in the classroom, what Child A was doing. Ms. Norton went over to Child A and got on her level. Ms. Ryskamp stated about 10 seconds later Child A snapped out of it and was responsive. Ms. Ryskamp thought it was strange behavior, but Child A appeared to be fine. She told Ms. Norton to keep an eye on Child A and to let her know if something like that happened again. Ms. Ryskamp stated she left for the day around 11:30am. She stated around 12:00pm she received a phone call from Ms. Nink informing her that Child A was acting strange again staring and not responding. Ms. Ryskamp stated that Ms. Nink called Child A's parents to notify them of in the incident. They did not answer the first time and a message was left. Eventually they got ahold of Child A's Mother and informed her of the situation. Ms. Ryskamp stated that she was informed that Child A snapped out of it again and she told CCSM's to lay Child A down for nap and to have a staff member sit next to Child A to monitor her. She stated that Child A was picked up around 3:00pm and was brought to her pediatrician. She identified that Child A's parents were going to follow up with a neurologist as soon as they could get an appointment scheduled. Ms. Ryskamp stated no other incidents were reported prior to this or following it. Ms. Ryskamp stated emergency services were not called because Child A was breathing and did not appear to be in any distress. I spoke to curriculum coordinator Jaime Nink. She stated on 2/18/2021 she was called down to the Pedal classroom because Child A was having another episode of staring blankly and was not responsive to verbal cues. Ms. Nink went to the classroom and observed Child A sitting down at the lunch table with her hands clenched, staring blankly and would not respond when spoken to. She stated Child A was breathing but not moving. She stated that CCSM Ms. Deleon was sitting next to Child A. Child A had food on her face and shirt so Ms. Nink assumed Child A had been eating her lunch prior to having this episode. She estimated that Child A was unresponsive for approximately five minutes before she became responsive. Ms. Nink stated that Child A started looking around and appeared confused. Ms. Nink stated she called Child A's Mother and informed her of the situation. She was informed that Child A's Mother made Child A an appointment with her pediatrician at 3:30pm. Child A's Mother was informed that Child A was laying down for a nap and asked that someone sit next to Child A when she slept. Ms. Nink stated a CCSM sat next to Child A as she slept. Child A was picked up at 3:00pm by her mother. Ms. Nink stated she did not call emergency services because Child A was breathing and did not appear to be in pain. She stated Child A seemed medically fine just zoned out. I spoke to CCSM Jennifer Norton. Ms. Norton identified she is the lead in the Pedal classroom. She state
