Teacher 1 did not provide appropriate care and supervision when he focused his attention on a project for more than 15 minutes and when he focused his attention on the children in the hallway bathroom and not the children in his classroom who were running around, hitting, eating and throwing toys.

Child A was in attendance on 12/22/2025 from 8:00 a.m. to 4:43 p.m., and only one feeding was documented at 10:37 a.m., during which she consumed 4.5 ounces. Program staff and parents noted inconsistencies in how food intake was documented and communicated.

On 3/8/2023 I spoke to Child A's Father. He stated that there has been a "huge amount of turnover" of staff at the center and that recently five staff had left at the same time. CCSM 1 reported to him that most of the turnover is because of issues with the program director. He noted that CCSM 1 reported that staff can't go to Ms. Green's boss with the issues regarding her leadership because they are personal friends. On 3/9/2023 I completed an unannounced on-site inspection at the center. I interviewed center director Virginia Green. She stated that they have not maintained 611 W. OTTAWA. P.O. BOX 30664. LANSING, MICHIGAN 48909 www.michigan.gov/lara . 517-335-1980 the required staff to child ratio due to so many staff members have recently left the center. Ms. Green explained that three staff walked out yesterday unexpectedly. Today they have had to close classrooms to accommodate the changes. Today the rooms that are open are in ratio, but yesterday they were did not maintain ratio after the walk out. On 3/7/2023 the infant 1 classroom was over ratio by having one staff with eight infants and the two's classroom was out of ratio by having two staff with 11 children. As a result, the infant II classroom, early preschool room, the two's room, and the toddler classroom are closed today. I went around the center to verify that the classrooms that were operating were within the required staff to child ratio. The infant I room had two staff to eight children, the preschool classroom had six children to one staff, the pre-kindergarten classroom had nine children to one staff (children ages four-five), and the school aged room was open due to a half day and they had six children to one staff. Ms. Green indicated that there are no extra staff to give the classroom staff breaks and lunch. She indicated that she was in that role today, in addition to being the cook as that was one of the staff that left the center yesterday. I interviewed CCSM 2. She stated that her classroom is over the staff to child ratio every morning. She reported she is left with up to 11 infants and toddlers by herself. She indicated that it is usually from 7:30 AM- 8:30 AM and they have been over ratio every day since February. She stated that she has been in the toddler classroom by herself with six children before and that she has been in the two's room with up to 10 by herself. I interviewed CCSM 3. She stated that the classrooms are over the required staff to child ratio a lot. She noted she is particularly stressed by this. She does not specifically know when they have been over ratio or by how many children or for how long but noted it happens frequently.

Child A's Father stated the center will provide diapers for the children if the family pays for this service, otherwise the family is responsible for supplying the diapers. CCSM 1 reported to Child A's Father that there were supply issues at the center, 611 W. OTTAWA. P.O. BOX 30664. LANSING, MICHIGAN 48909 www.michigan.gov/lara . 517-335-1980 including diapers. CCSM 1 reported that staff would be yelled at by program director Virginia Green for going through the diapers too fast. Child A's Father stated that Child A had a terrible diaper rash for approximately three weeks, and that over the weekend it would clear up but then it would be back again once Child A came back to the center, leading him to suspect that they were not changing him appropriately due to lack of diaper supplies. On 3/9/2023 I completed an unannounced on-site inspection at the center. I interviewed program director Virginia Green. She stated staff change diapers every two hours or when wet or soiled. She stated that the diapers are currently well stocked. There was one occasion a few months ago where they had to supplement with the center provided diapers by using a different brand of diaper. She explained that their regular online diaper ordering process changed, so they didn't get the supply on time. They ended up having to get diapers at Meijer instead. She stated they only ran out of size 4 diapers and that before they were able to get to Meijer, they used pull ups, which they had in stock instead. Ms. Green indicated that administration has since learned how the new online ordering system works and that they know how to navigate it now and they haven't run out since. I requested to see the diapering supplies. I observed a large number of boxes and packages of diapers, in various sizes, and wet wipes stacked in a storage room. I interviewed CCSM 2. She stated that they change diapers every two hours or before if the child is wet or soiled. The center has run out of diapers and wipes before. CCSM 2 noted that they have been out of all different sizes of diapers from time to time. Recently, in February 2023, they ran out of wipes and Ms. Green had to go to the store to get more. She stated that during this time children may have been changed outside of the two-hour time frame but noted that children did not have to sit in wet or soiled diapers at the time that she can recall. I interviewed CCSM 3. She stated that children are changed at least every 2 hours or immediately if there is a bowel movement.

On 3/8/2023 I spoke to Child A's Father. He stated that CCSM 1 reported that the center was often running out of cleaning and sanitizing supplies. CCSM 1 also stated that Ms. Green would yell at the staff for going through the cleaning supplies quickly. On 3/9/2023 I completed an unannounced on-site inspection at the center. I interviewed center director Virginia Green. She stated there are no cleaning supply issues. She noted that on occasion, paper towel run out, but that staff just grab rolls from other rooms that have additional stock. I requested to see the cleaning supplies stored at the center. I observed there to be a sufficient amount of extra cleaning supplies stored in a center storage room. I also checked the three-step cleaning/sanitation supply in the classrooms and each room was supplied with the three step spray bottles. I interviewed CCSM 2 and CCSM 3. CCSM 2 stated that they have ran out of paper towel before and that the staff have had to get some from other classrooms. She noted that staff do not get yelled at for running out of supplies. CCSM 3 stated that if they are out of something, Ms. Green may question as to why something is out, but she does get the staff more.

On November 4, 2021, I spoke with Child A and B's Mother on the telephone. She stated that Child A is allergic to gluten and cow's milk. She has asked the center not to give him any foods with gluten or cow's milk. She provides gluten free alternatives for Child A to eat when at the center. The center provides fruit and veggies. She stated that Child A gets bloated and has "explosive diarrhea" if he eats gluten. He will have these symptoms for a few days after eating gluten. On October 29, 2021, she was watching on the center's camara feed available to parents and noticed that Child A was eating pretzels and Goldfish crackers at snack time. She said that this occurred approximately 2:58 and 3:00pm. She said she sent a message via "Bright Wheel", which is the parent/center communication application, letting the child care staff member know that Child A could not have Goldfish crackers. After sending the message, she called the center to let them know that Child A was eating Goldfish crackers. She said she spoke with Christy Galassi and 2 that Ms. Galassi told her she would let the staff member currently in the room know. Child A and B's Mother continued to watch the camera and saw the staff member in the room picking the Goldfish crackers out of the pretzels with her bare hands. She said that the center had a Fall Fest party planned for that day, she had packed gluten free pretzels and a gluten free muffin for Child A to have at snack time. She provided the muffin as an extra treat since it was a party day. Child A and B's Mother said when she arrived at the center to pick up Child A, she informed the staff member in the room that Child A was gluten free. Child A and B's Mother stated the staff member told her that Child A did not eat any Goldfish crackers. Child A and B's Mother said she told the staff member she saw Child A eat Goldfish crackers while watching the classroom feed. She added that she spoke to the staff member in a "stern" manner because she wanted to know why Child A was given the Goldfish crackers and asked her if she was a new staff member. Child A and B's Mother stated she did not know the name of the staff member she spoke with. She said she also spoke with the program director, Jessica Braley, "briefly" on the same day at pick up time. She added that as of November 4, 2021, Ms. Braley had not followed up with her about the incident. Child A and B's Mother said that at enrollment she provided doctor notes and paperwork to the center concerning Child A's gluten and cow's milk allergy. She has noted the allergy on the Child Information Card, and that she communicates with staff on a daily basis about the food for Child A. She added that she is "not sure" if there is a written plan in place. She said, she typically speaks with "Miss Nicole" each day about meals. She added that she "loves" the center and that they have "great staff." She has been very satisfied with the care the center provides for Child A and Child B. Child B (female, age 3) has similar allergies and there has never been an issue with what she eats at the center. On November 4, 2021, I made an unannounced on-site inspection at the child care center. I spoke with the program director Jessica Braley, program manager Christy Galassi, and child care staff members Serrina Stanfill and Nicole Munsey. Ms. Braley stated that on October 29, 2021, she was on a "bus run" until about 3:30pm. When she arrived at the center, Child A and B's Mother approached her and they spoke briefly on the sidewalk in front of the building. She said that Child A and B's Mother informed her that she saw Child A eating Goldfish crackers on the camera feed and then saw the staff member in the room pick out the crackers with her hands. Ms. Braley informed me that after speaking with Child A and B's Mother, she spoke with the program manager, Ms. Galassi and Serrina Stanfill, who was the child care staff member in the room at the time of the incident. Ms. Stanfill did use her bare hands to remove the Goldfish crackers from Child A's snack. Child A did not eat the snack after she began picking out the Goldfish crackers, because Ms. Stanfill removed the whole snack and gave him the pretzels from his lunchbox. She said she had gloves on when she served him the pretzels.
