On 08/11/2022, I spoke with Ms. Lisa Dungey, program director regarding the incident. Ms. Dungey self-reported the incident and explained the facility was on a field trip at United States of America in Lincoln Park. There was a total of 11 children with three child care staff members present. The two child care staff members (CCSMs) were on the skate floor supervising the children throughout the activity. Child A had fallen while roller skating and injured her pinky finger on the left hand. She reported the fall to Ms. Dungey who examined her hand and observed a blister on the tip of her left pinky. Ms. Dungey applied ice to the area and notified Child A's Mother. On 08/09/2022 she contacted Child A's Mother regarding another matter and learned the finger had become painful, swollen and required medical attention from Children's Hospital. On 08/16/2022, I conducted an onsite investigation at the facility and found it to be closed. I contacted Ms. Dungey who stated it was a summer program and ran from 07/11/2022 to 08/12/2022. On 08/12/2022 and 09/15/2022, I contacted Child A's Mother and discussed the incident. The day of the outing she received a call from Ms. Dungey about Child A's injury. She does not recall much about the conversation. Child A's Mother believes "whatever happened it was an accident," She believes the facility always take good care of the children and she has no problems or concerns about anything. Child A has attended the facility for a while and loves it. Child A's Mother also stated, "She is old enough to explain what happened." Child A told her, "She was skating and using a walker when she fell. A skate-mate is often referred to as a walker in roller skating. It is a walker with wheels designed to help maintain balance. Child A does not know if the walker hurt her finger or if another skater ran into it." Child A's Mother observed the blister when she returned home from school and drained the fluid from it. Later in the day, Child A complained of numbness and there was some discoloration. Child A's Mother transported her to Children's Hospital where she was diagnosed with a fractured finger and fitted with a splint to wear for a few days. Child A is doing well and has returned to regular activities without any problem or concerns. On 09/15/2022 and 09/20/2022, I contacted child care staff members Nicole Ray and Christa Wood. They have not returned my call as of this entry. In speaking to Ms. Dungey, she informed me of Ms. Wood only working during school summer breaks. 3
On 08/05/2022, Child A (7-year-old, female) sustained an injury while on a field trip. Staff does not know how it happened.
