On 02/22/2023 an incident report was received from Wilson Elementary ECC. During quiet time, Child A stated "I'm going to use my mat as a slide" Before any of the staff members could respond, there was a crash and Child A was crying. Child A told the child care staff members that as he went to climb up his mat 2 and it slipped off the bookshelf. Child A's lip was split open. Child A's grandma took him to the emergency room where he received stiches. On 03/01/2023 I interviewed Child A's father via telephone. Child A is a daredevil and is not very coordinated. He has no concerns with the care and supervision that Child A receives at Wilson Elementary ECC. On 03/09/2023 I completed an on-site inspection at Wilson Elementary ECC. I interviewed program director Jessica Konieczny, child care staff member Samantha DeCare and child care staff member Kassie Leaym. They were all present in the classroom on 02/22/2023 when the incident occurred. They confirmed the information stated in the incident report. Child A was in the library area at rest time. Ms. Leaym was sitting at the table near the library area. Ms. Konieczny was sitting at her desk. Ms. DeCare was passing out quiet time activities to the children. It was seconds between the time Child A stated "I'm going to use my mat as a slide" and when he started crying. Child A has never attempted to use his nap mat as a slide in the past. There is another child who has attempted to use his nap mat as a slide and child care staff members redirect the child by telling them that nap mats stay on the floor. On 03/28/2023 I interviewed Child A's mother via telephone. The incident on 02/22/2023 was handled appropriately by staff. She has no concerns with the care and supervision that Child A receives at Wilson Elementary ECC. On 3/28/2023 I interviewed Child B's mother, Child C's mother, Child D's mother and Child E's mother via telephone separately. They have no concerns with the care and supervision children receive at Wilson Elementary ECC.

I received a telephone message on 11/08/2021 to report that Child A got onto the wrong bus. On 11/09/2021 I received a copy of the incident report from licensee designee Ms. Brenda Bradbury. The report detailed Child A left his class group and followed his older brother onto the wrong bus at approximately 4:00pm on 11/08/2021. The report also stated Child A has special needs. On 11/09/2021 I completed an onsite inspection from 3:25pm-4:45pm. I observed 14 children in care with two child care staff members. When I arrived, I observed the child care staff members leading the children out of the school and around the front of the building. They had the children set their backpacks down in an area between the building and the sidewalk. After setting the backpacks down the staff led the children to the fenced in outdoor play area. I interviewed program director Jamie Snyder. There were 15 children the day before with Ms. Snyder and child care staff member Ms. Alisha Treinen. I asked Ms. Snyder to point out Child A which she did. Child A has special needs and has very limited verbal skills. On 11/08/2021 she was at the front of the line, holding Child A's hand along with another child on the way from the outdoor play area to the front of the school to get their backpacks. She instructed the children to get their backpacks on and sit down to wait for the bus. Ms. Snyder turned around to help a child who asked for help. When she turned back around, she noticed Child A was missing. Ms. Snyder asked Ms. Treinen if she saw Child A. Ms. Synder told Ms. Treinen she was going to see if he was in the outdoor play area. Ms. Snyder ran to the play area but did not see Child A. She ran back to the front of the building where Ms. Treinen and the rest of the children were. Child A was still not there so Ms. Snyder ran to the classroom to see if Child A was there. He was not in the classroom. She ran back to the area where Ms. Treinen and the rest of the children were, and an aid waved to her saying one of her children was on the bus with his older brother and wouldn't get off. Ms. Snyder went to the bus and got Child A off the bus. I asked Ms. Snyder why Child A's brother was on that bus and 2 she reported that Child A rode a different bus. The school bus drivers refuse to drive preschool aged children now because they don't want to have to be on their busses with preschoolers while they wait for the all the school age children. The bus drivers like to stand on the sidewalk and socialize until all the children are on the bus. So, the school age children ride the school bus, and the preschool children have to ride a public transportation bus. I observed the group line up to go from the outdoor play area to the front of the building. They got their backpacks on and were instructed to sit down. Several children required a great deal of redirection and supervision during this time. All the school busses were lined up in front of the school. The public transportation bus the preschool children ride was at the back of all the school busses. I asked Ms. Snyder which bus Child A went to the day before. She pointed to the bus right in front of her and the children approximately 3 feet away. I approached the bus, introduced myself to the bus driver and asked if I could ask her about the incident. The bus driver was short with me. She gave me her name but declined to give me her phone number. She reported that Child A got on the bus and was sitting with his brother. When she realized he was a preschooler she told him he had to get off the bus, but he started crying and wouldn't get off. She then had the aid tell the preschool staff he was on the bus. When asked how long he was on the bus she said it was only a couple minutes. During the onsite inspection, I observed the group to have several children that required additional one on one assistance and supervision at various times including Child A.
