I reviewed ten child information cards, and nine cards were missing required information such as employer names, employer phone numbers, physician information, release, written parental permission for emergency medical care, and dates on the cards.

I reviewed ten child files, and one child did not have a certificate of immunization on file showing a minimum of 1 dose of each immunizing agent by the health department at the time of initial attendance.

I reviewed five staff files, and no program staff members had completed consent and disclosure forms on file.

One program staff member did not complete the 2025 health and safety update published in MiRegistry within the timeframe indicated in the notice.
