On 08/31/2023 I received a telephone call from program director Jennifer Klein informing me Child A and Child B were mixed up and fed each other’s bottles. She stated Child A’s parents and Child B’s parents were contacted via telephone after the incident occurred. Jennifer Klein submitted an incident report to the department on 08/31/2023. On 09/07/2023, Jennifer Klein stated on 08/31/2023 CCSM 1 called her at the front desk from the classroom informing her of the mix up of Child A and Child B and their bottles. Child A drank two of Child B’s bottles filled with formula and Child B drank one of Child A’s bottles filled with breast milk. Jennifer Klein went to the classroom and compared pictures of the children on SproutAbout and contacted Child A’s parents and Child B’s Mother. Jennifer Klein said she is working with staff to ensure children are not mixed up in the future. On 09/07/2023, CCSM 1 stated she is primary caregiver for both Child A and Child B. Child A and Child B arrived prior to her arriving at work. She was told by CCSM 2 which child was Child A and which child was Child B. CCSM 1 said Child A and Child B have very similar facial features and are about the same size. CCSM 1 said she is taking full responsibility of mixing up the children because she is their primary caregiver and should have checked the children herself. CSM 1 said she did check the bottles and followed policy regarding checking bottle labels and checking children; however, she had Child A and Child B mixed up. CCSM 3 entered the classroom to provide care to children and discovered Child A and Child B were mixed up. Staff watched Child A and Child B for any gastrointestinal issues and did 3 not notice any problems. Staff have been talking about various ways to ensure there is not a future mix up. On 09/11/2023, Child A’s Mother stated it was disconcerting that there was a mix up with the bottles and children. She was notified promptly of the incident and Child A did not have any gastrointestinal issues. Child A’s Mother discussed the center ensuring there are no further mix ups. On 09/14/2023, Child B’s Mother stated she received a telephone call from Jennifer Klein. Child B seemed fine and did not have any gastrointestinal issues. She has no concerns with the care Child B receives at the center. On 09/14/2023, Child C’s Mother stated she has no concerns regarding the care Child C (male, 5 months) receives at the center. She has an older child that attended the center for five years and is now school age. She has been pleased with care both children have received.

On 08/03/2023, I arrived at the center and informed program director Jacqueline Denney of the allegations. Jacqueline Denney stated the first leak occurred in the Pedal classroom on 07/06/2023. The corporate office hired a 2 plumbing company to fix the leak. While fixing the small leak behind the classroom sink, it blew out the pipe in the toilet room, which is adjacent to the first leak. Pipes that backed up to the Lyric room caused water damage to walls in that room as well. The plumbing company fixed the pipes, dried out the walls, added new dry wall, and painted. All work was done in the evenings when children were not in care, which added time to complete the repairs. During this time children in care were relocated to other classrooms. Parents were notified on 07/11/2023 via SproutAbout of the water leaks. Jacqueline Denney and I walked to both classrooms to observe the completed work. Children and staff were present. On 08/03/2023, I talked with CCSM 1 and CCSM 2 in the Pedal room, and CCSM 3 and CCSM 4 in the Lyric room. All child care staff members expressed no concerns regarding the leaks and length of time it took to repair. On 08/11/2023, I received an email from Jacqueline Denney. Attached was a letter from BELFOR Property Restoration dated 08/10/2023. BELFOR was contacted on 04/17/2023 for mitigation services related to a small plumbing leak at the center. Areas impacted by water were restored and the building was inspected by BELFOR and EMCOR on multiple occasions ensuring all work completed to be safe and satisfactory standards. On 09/11/2023, Child B’s Father stated he did not recall an issue with plumbing problems in the Pedal classroom. He was unable to talk and stated he would call me back. On 09/11/2023, Child C’s Mother stated she has no concerns regarding the leak in the Pedal classroom. She understood it would be a process to repair the leak and was not concerned it would be a hazard for the children. On 09/14/2023, Child A’s Mother stated she was not in town during the leak; however, she did receive notification of the leak which was sent via SproutAbout. Child A’s Mother has no concern regarding Child A’s (male, 2 years) care.

On 04/24/2023 at 9:30 AM I received a telephone call from program director Jacqueline Denney informing me of Child A’s head injury when she sustained a fall on 04/20/2023. On 04/24/2023 at 4:37 PM I received an injury report from Jacqueline Denney which stated the following: “Child A was standing of the floor of the bathroom when she lost her balance and fell backwards, landing on her butt and then tipping all the way back and hitting her head on the floor. She was comforted and checked for any bumps, bruises or red spots, none were present. At 11:30 she was sad and asked for her blanket and paci, and fell asleep for a nap. When she woke up around 2 pm, she threw up. Mom and Dad were notified of the vomiting, and of the fall. Child A was sad while waiting for dad, but was able to stand and talk. Mom emailed around 6 pm on Friday night stating that they took Child A to the emergency room for concussion symptoms. She was not speaking as much, and was loosing her balance when trying to stand. They did a scan, and found no bleeding or swelling in the brain, but were monitoring her for concussion symptoms.” On 07/03/2023, Child A’s Mother stated Child A fell on 04/20/2023 and sustained a head injury while in care at the center. On 04/20/2023, she was contacted at 2:10 PM by Jacqueline Denney that Child A was vomiting. Child A’s Mother said she was able to come and get Child A, so Child A’s Father went to pick up her up. Upon arrival at 2:20 PM, Jacqueline Denny informed him Child A sustained a fall and hit her head in the morning. Child A’s Father brought her home where Child A’s Mother said she was not stable on her feet and continued to vomit. Child A was taken to the emergency room at about 3:30 PM and diagnosed with a severe concussion. Child A 5 hit her head, took a nap before eating lunch, then woke up and vomited on her cot. Jacqueline Denney said she received an email from Child A’s Mother on 04/21/2023 informing her Child A was in the hospital and sustained a concussion. She immediately called Child A’s Mother because her heart could not stand waiting until Monday (04/24/2023) to talk with her. I read BCAL-4605 to Jacqueline Denney and asked why she did not write a thorough and descriptive incident report, informing the department of Child A’s concussion and hospitalization. She stated she does not know why she did not disclose all of the information when we talked on the telephone 04/24/2023 or when she submitted a written, signed, and dated incident report on 04/24/2023. Jacqueline Denney stated she did a staff training on 04/27/2023 and discussed with staff signs and symptoms of concussions. She provided a copy of the staff sign in sheet for the training on 04/27/2023. Jacqueline Denney denied showing favoritism to staff; however, she did confirm the Pedal room, Adabelle room, and Terance room do not have lead caregivers and forgot she could apply for variances. Jacqueline Denney provided staff sign in and out times for 04/20/2023 in the Pedal classroom. Child A was signed in to care at 8:17 AM and signed out of care at 2:45 PM. Jacqueline Denney also provided a copy of Child A’s SproutAbout log for 04/20/2023. On 11/17/2022, Child A's Mother stated she picked Child A up from childcare on Friday, 11/11/2022. Child A was happy as she usually is. She and Child A arrived home and Child A went to the bathroom. Child A's Mother heard Child A scream. Child A's Mother asked Child A what happened, but Child A had difficulty calming down. Child A's Mother called the school and talked with administrator Ms. Jennifer Klein. Child A's Mother was told child care staff had exited the center for the day. Ms. Klein told her she would call the staff and then call her back. Child A calmed down and told her she fell down on the climber and cried. A couple of friends helped her. Ms. Klein called her and said she talked with staff and there were no incidents or accidents to their knowledge. Child A's Mother called the pediatric office and was advised to take Child A to Sparrow Hospital for an evaluation. Child A's Mother took Child A and the physician told Child A's Mother the bruise on the upper thigh and tear on her labia is consistent with a fall. On Monday, 11/14/2022 Child A returned to child care. Child A's Mother said she along with program director, Ms. Jacque Denney and Child A went outside to see the climber. Child A showed Child A's Mother and Ms. Denney where she slipped, which was on the rungs of the climber. Child A's Mother said looking back, the injuries sustained make sense. Child A's Mother said, "Child A is 4 years old going on 40!" Child A is extremely verbal. Child A has been in attendance at Gilden Woods since infancy. She has never had a concern with the care Child A has received. When she arrived on Monday morning, 11/14/2022, Ms. Denney had an ouchie report ready. Child A's Mother said she has always received ouchie reports even for minor things. Child A loves going to child care and wants to be with her friends. Child A never wants to be late, even on 11/14/2022. On 12/29/2022, program director Ms. Jacque Denney stated she was at home when Ms. Jennifer Klein called her to inform her of the incident that occurred with Child A on 11/11/2022. On Monday,11/14/2022, Ms. Denney talked with Child A and Child A's Mother. Child A's Mother told her Child A slipped on the rungs and then went underneath the climber for a few minutes before going about her day. During the conversation with Child A's Mother, she was informed Child A had been taken to the hospital for evaluation. Ms. Denney talked with classroom staff Ms. Eesley and Mr. Foster to ensure proper zoning occurs on the playground as well as in the classroom. Ms. Denney contacted licensing consultant Sharon VanOrd and submitted an incident report on 11/14/2022. While onsite Ms. Denney and I went outside where she showed me the climber. A picture was taken and will be included in the file. A review of the center's file has a playground inspection report dated 05/23/2022 showing compliance for the climber and use for children ages 2-12. On 12/29/2022, child care staff member Mr. Alex Foster stated 11/11/2022 was a typical day in the classroom. Child A acted completely herself. The weather was nice that day, so the classroom spent extra time outside. Child A is reserved, shy, and 3 quiet. She enjoys playing with her friends. Mr. Foster never saw her cry or acting sad. He feels bad that Child A was injured; however, he does understand why she would be a little uncomfortable talking to anyone about the incident because it was in a private area. On 12/29/2022, child care staff member Ms. Ashley Eesler stated on 11/11/2022 she did not see Child A slip on the climber, nor did she see her crying when the classroom played outside. After finishing work for the day, she received a phone call from Ms. Klein asking her if she knew of Child A sustaining an injury while outside. Ms. Eesler said she would have filled out an ouchie report, which is standard practice. Ms. Eesler said she talked with Child A encouraging her to tell a staff member when something happens.
