On April 1, 2022, I completed an unannounced on-site inspection. I individually interviewed Ramona Borowicz, program director and child care staff members, Lucinda Atalla, Faeza Jariwala and Heidi Foxworthy. Ms. Borowicz stated that on March 30, 2022, after coming out of a meeting she was informed by Coutney Cascarelli, administrative assistant that Child A from Bird Room was briefly left unsupervised on "the hillside" of the playground. Two child care staff members were present on another area of the playground during the time of the incident. There were a total of 12 preschool age children, ages 3-4 years present on the playground. Ms. Borowicz met with child care staff members to review what happened. She stated that she reviewed appropriate child care staff member positioning while on the playground. Ms. Borowicz notified Child A's Mother, Head Start and Child Care Licensing Bureau. Child care staff member Faeza Jariwala stated that she and child care staff member Sydnee Hrachovina were on the playground with 12 preschool age children, ages 3- 4 years, as lead child care staff member Luci Atalla went into the building for a meeting. Child A ran several times to the "hillside" of the playground and Ms. Jariwala ran after her and reminded her to stay on the "climber side" of the playground. She did not notice that Child A ran again to the "hillside" of the playground until she saw child care member Kristin Cook bring her back to the "climber side" of the playground. Ms. Cook observed Child A from her classroom window. Child A was unsupervised on the "climber side" of the playground for approximately one minute. During the time of the incident Ms. Jariwala was talking to a child by the climber and Ms. Hrachovina was talking to another group of children at the sandbox. Both child care staff members were on the "climber side" of the playground with their backs to the "hillside" of the playground. Lead child care staff member Lucinda Atalla stated that she was not on the playground during the time of the incident, as she came inside for a meeting. Child care staff members Faeza Jariwala and Sydnee Hrachovina were on the playground with 12 preschool age children, ages 3-4 years. Ms. Jariwala and Ms. Hrachovina told her that Child A ran to the "hillside" of the playground in the kitchen section. She was unsupervised for approximately one minute. Child care staff member Kristen Cook observed Child A from the window in her classroom and came outside and returned her to the "climber side" of the playground where Ms. Jariwala and Ms. Hrachovina were located. Ms. Atalla stated that she discussed positioning of child care staff members to allow for a global view of the playground. Child care staff member Heidi Foxworthy stated that she works in the classroom with child care staff member Kristin Cook. Ms. Cook was working with a group of children by the window and observed Child A run past the window. Ms. Foxworthy saw Child A stop at the kitchen area outside the window. Ms. Cook asked her to watch the group of children that she was working with and proceeded to run outside to return 611 W. OTTAWA. P.O. BOX 30664. Ms. Foxworthy stated that she opened the window and talked to Child A until Ms. Cook approached her. Child A was unsupervised for approximately one minute. On April 8, 2022, I spoke to child care staff member Sydnee Hrachovina. She stated that on March 30, 2022, she was on the playground with child care staff member Faeza Jariwala and 12 preschool age children, ages 3-4 years. During the time of the incident Ms. Hrachovina was at the sandbox with a group of children and Ms. Jariwala was at the climber with another group of children. On 6/29/21, I reviewed the incident report sent to the department by the facility. On 6/28/21 at approximately 9:45 a.m., Child A opened the classroom door and walked into the hallway. Ms. Courtney Coscarelli, child care staff member, heard and saw the classroom door open and immediately counted children in the classroom. After realizing Child A was not present in the classroom, Ms. Coscarelli called for help and walked out of the classroom into the hallway to look for Child A. Ms. Coscarelli informed Ms. Sarah Barth, program director of the incident and how Child A had walked out of the classroom. Ms. Barth looked in the hallway and found Child A sitting under a hallway bench. Child A was returned to the classroom less than two minutes from the time he exited the classroom. Ms. Barth informed Child A's Mother of the incident at the time she arrived to pick him up from care around 9:50 a.m. Child A's Mother informed Ms. Barth that Child A has been recently hiding under the furniture and even opened the front door of the home and walked outside on his own. On 6/30/21 at approximately 12:00 p.m., I conducted an unannounced onsite inspection to the center. I interviewed Ms. Barth. She confirmed that on 6/28/21 at approximately 9:45 a.m., Child A opened the classroom door by himself and walked out into the hallway of the center. Prior to Child A walking out of the classroom, Ms. Barth said that Ms. Coscarelli had just relieved child care staff member, Ms. Erica Colon. At the time of the incident, child care staff members Ms. Coscarelli and Ms. Jessica Zahm were in the room along with supervised volunteer, Ms. Kenia Rosas. There were eight children ages 3 to 4 years old. Ms. Coscarelli told her that she heard the classroom door make a clicking noise as through it was opening or closing. When Ms. Coscarelli looked up toward the door, there was no one that had entered the classroom. Ms. Coscarelli immediately performed a head count of the children and noticed that Child A was not present. Ms. Coscarelli immediately radioed for assistance in the classroom and informed Ms. Barth Child A was missing. Ms. Barth began looking for Child A in the hallway located just outside of the classroom. Ms. Barth found Child A hiding under a bench located outside of the classroom in the hallway. Ms. Barth explained that this is the same bench the children go to each day at the time of dismissal. Child A was sitting under the bench and in "good spirits." Ms. Barth took Child A by his hand and prepared him for early pick up by Child A's Mother. Both Ms. Barth and Ms. Coscarelli spoke with Child A about the classroom rules and how he cannot open doors without an adult. They explained to Child A how it is important for him to follow the class rules, as the rules are in place to keep him safe. Ms. Barth explained that due to Child A's Mother already being scheduled for an early pick up, Ms. Barth notified her of the incident at the time she arrived at the center approximately five minutes later at around 9:50 am. Child A's Mother informed Ms. Barth that Child A began displaying these same behaviors at 3 home, as he would hide under furniture and on one occasion even snuck out of the home while being babysat, to try and go with his mom. Ms. Barth said Child A has been attending care at the center for approximately nine months. Ms. Barth informed me that Child A is autistic, has a speech delay, and an early childhood education learning delay. Ms. Barth said Child A has been making great strides in preschool and has progressed a lot since first being enrolled in the preschool program. Ms. Barth explained that Child A attends a half day preschool program in the Train Classroom, which accommodates children ages 3 to 4 years old. Child A returned to care on 1/29/21.
