On October 7, 2020, program director, Ms. Jessica Terberg, called and self-reported an incident. Ms. Terberg said on October 7, 2020 at approximately 3:15 P.M., the Firefly Classroom #6 went outside to the playground. There were ten children with two child care staff members, Ms. Lorene Maschke and Ms. Kristy Leach. A child care staff member, Georganna Garner, was in the hallway when she heard the sound of a child crying. This was coming from the Firefly Classroom #6. She went inside the classroom and found Child A (male, 3 years old) standing by the window. He was crying and watching his classmates play outside. Ms. Terberg believes Child A was left unsupervised for approximately 30 minutes. On October 8, 2020, I completed an unannounced on-site inspection. I interviewed program directors, Ms. Terberg and Ms. Jessica Pasienza. Ms. Maschke, Ms. Leach and Ms. Garner were not present at the center during the inspection. Ms. Terberg said Ms. Maschke and Ms. Leach were placed on suspension for two days and Ms. Garner was on lunch break. Ms. Terberg spoke with Child A's Mother after the incident on October 7, 2020 and a follow up phone call this morning. Child A's Mother was appreciative that Child A was safe inside the building and that the center is taking action to prevent future incidents. Ms. Terberg and Ms. Pasienza both said they were in the office when the incident occurred. Ms. Garner came to the office with Child A and informed them that she found him alone in the classroom. Ms. Pasienza stayed in the office while Ms. Terberg brought Child A outside to his class. Ms. Terberg said she went outside to the playground and spoke with Ms. Maschke. Ms. Maschke was unaware that Child A was missing. According to Ms. Terberg, Ms. Maschke told her Child A was in line when they were in the classroom. Ms. Maschke turned around to lead the class out to the hallway and assumes Child A did not follow them. Ms. Terberg said there were two lines, each line had five children with one child care staff member. Ms. Maschke's line was the first group to leave the classroom. Ms. Maschke said she completed a head count before leaving the classroom. Child A was at the end of the line. During the inspection, I received a copy of the incident report. According to the report, " On October 7, 2020 at 3:40 P.M., Ms. Garner walked past the Firefly Classroom and heard a child crying. She entered the room because she knew the class was outside. She found Child A standing at the window by himself. Ms. Garner immediately brought him to the office. 3 • • - • -• • •- • • •• 1 1 . . On October 14, 2020, I called and left a message for Child A's Mother. On October 16, 2020, I called and spoke with Child A's Mother. Child A's Mother said Child A was doing well and does not appear to be traumatized by the experience. Child A was sad that he was inside the classroom while his friends were playing on the playground. Child A's Mother said it was quite fortunate that he was safe inside the building. Child A's Mother said the child care center called and reported the incident when it occurred. She also received a follow up telephone call from Ms. Terberg the next day. She is satisfied with how the child care is responding to the incident. She believes they have implemented a new policy that requires completing additional head counts of the children. On October 16, 2020, I called and spoke with Ms. Leach and Ms. Garner. Ms. Leach said on October 7, 2020 she was with Ms. Maschke and ten children. Ms. Maschke had a group of five children, and she had the other five children. Child A was in Ms. Maschke's group. They were lining up to go outside to the playground. Ms. Leach said she left the classroom and went to the office to get the walkie talkies. When she came back to the classroom, she asked Ms. Maschke if they were ready to go out and Ms. Maschke said, "Yes." Ms. Maschke led her group of children first and then she followed with hers.
