On June 9, 2021, the child care staff members did not provide appropriate care and supervision of Child A. None of the child care staff members were aware that Child A left the child care center. Child A left the center 29 seconds after Child A's Mother left the building. Child A's Mother brought Child A back inside the building 66 seconds after Child A left the center.
