On 06/19/2023, Child A (17-month-old, male) and Child B (23- month-old, male) both got bitten twice by other children. Child C (24-month- 3 old, male) was also injured and had a bruise. The injuries are from the other children at the center and are not being reported. Licensee designee and child care staff member, Amanda O'Day yells at the children and hands alcohol out to the employees at the end of the day.

The teachers would have to bribe him to get on the bus to go to his mom's because he loved child care so much. However, recently he's been having more outbursts. was asked about having any knowledge of him falling or being accident prone. She reported that he is a typical boy who plays around but is no rougher than any other child. On 04/19/2023, l contacted licensing consultant Mr. Donald Vita in regard to the center and if anything had been reported to him regarding a child being Mr. Vita stated that he had completed a renewal inspection at the end of March, and no violations were cited. Ms. O'Day had contacted Mr. Vita on 04119/2023 and reported that a child did not get injured at the child care center. On 04/20/2023, interviewed the Head Start Regional Coordinator, Ms. Joanna Rouse, regarding A. Ms. Rouse often works with Child A in his classroom at . She stated that she was not aware Pany recent incidents f Child A falling off a bus or being injured. He was doing very well, but recently his behaviors have changed, and she does not know why. He started clearing shelves, yelling and kicking, and escalating so quickly that it's hard to get him to calm down. They cannot avoid him from escalating because they can't predict his behaviors. She had a conversation the other day with him because he was struggling that day. They were talking about his behaviors. She asked him what he does when he is at his parents or at child care, and he just said that he didn't want to go to child care or his dad's. He says he only wants to be with his mom, and he said it's because "they don't listen." He said that the adults don't listen to him. On 04/20/2023, interviewed Child A's Father. On 06/21/2023, I made an unannounced on-site inspection. I explained the allegations to licensee designee/child care staff member Amanda O'Day. Amanda O'Day stated that the center policy is when a child in injured or hurt, the child care staff member takes a picture of the injury and it goes to the parent of the child who was bitten, and the parent of the child who bit is also notified. Everything is documented in the Brightwheel Child Care App. Amanda O'Day reported that she received a text message from Child B's Mother on 06/19/2023 at 4:40 p.m. along with a picture of a bite mark on Child B's forearm. The text message stated, "Hey lady I just noticed this on Child B's arm after I picked him up, do you know if it happened today? I didn't see anything in the app." Amanda O'Day then messaged the child care staff members and asked what had happened to Child B. CCSM 8 responded that she had changed Child B's diaper prior to going outside at 3:40 p.m. and the mark wasn't there. Amanda O'Day suspected Child D (21-month- old, male) had bitten Child B, as he had been having a hard time transitioning into the Red Room and had been biting the other children. Amanda O'Day further reported that child care staff member Kathrine Hill had said that Child B never said anything about being bitten or hurt. Amanda O'Day was unaware of Child C (24- month-old male) having been injured while at the center and stated that nothing had been documented that he had acquired a bruise. She did recall that there was one day when he came in with a bruise on his face and she believed it had something to do with him and an older child on the playground. She further reported that Child C's stumbles and falls a lot, so it is hard to say if he had gotten bruised at the center. On 06/21/2023, I interviewed CCSM 1. She reported that she was working in the Red Room on 06/19/2023; 06/20/2023 and 06/21/2023. She was not aware of Child A being bitten on two occasions. She said there was only one day when he had a bite mark on his arm, right before he left the center around 3:00 p.m. "Red teeth marks.. .it looked old.. .not fresh. I asked him if he was okay, and he really didn't acknowledge it." She confirmed that she did not report the incident. She was not aware of Child C having any type of injury or bruise. She remembered Child B had gotten bitten by Child E (30-month-old, female) when she first started, but was not aware of him having been bitten within the last day or two. CCSM 1 said that she was made aware of reporting incidents when she completed orientation with Amanda O'Day, but since then she feels she is unclear on how and when to document injuries. She said biting has always been kind of an issue in the room but has escalated since Child D has been in attendance. On 06/21/2023, I interviewed CCSM 2. She reported that Child B had been bitten by Child F (27-month-old, female) about a week ago and that she reported it. She did not have any information that Child A or Child C had been bitten, injured, or hurt while attending the center. The Brightwheel Child Care App for the center did not show any recorded incidents for Child B between the dates of 05/11/2023 and 06/21/2023. 4 On 07/10/2023, licensee designee/child care staff member Amanda O'Day provided me with a log of the recorded student incidents as reported in the Brightwheel Child Care App. From 05/11/2023 through 07/07/2023, Child A had two recorded incidents where he was bitten by another child on 05/19/2023 and 06/26/2023. Child B had one recorded incident on 06/01/2023 where other children had thrown sand at him. Child C had two recorded incidents, one on 06/16/2023 where he was bitten by another child and 07/07/23 where he had a toy in his mouth and had fallen on it. On 08/25/2023, a review of the center's parent handbook states, "All accidents and injuries, however minor must be recorded and signed and dated by staff and families. On 06/21/2023, licensee designee/child care staff member Amanda O'Day reported that she is not aware of herself yelling at the children. She said that if she sees something that needs to be addressed immediately, and she cannot get to the situation, she might raise her voice to get the staff's attention. She 5 provided alcohol to her staff on one occasion, and it was after the staff members had signed out for the day. On 05/26/2023, everyone was getting ready to leave for the holiday weekend and she had planned for the whole staff to go out as a group to an establishment, for dinner and drinks. She had to unexpectedly cancel the outing. Since she had already purchased the alcohol, she told the staff members that it was in her car if they wanted any. The alcohol was located outside the center and in her car. The staff members who took some of the alcohol would have signed out and been leaving for the day. On 06/21/2023, CCSM 1, CCSM 2, CCSM 3, CCSM 4, CCSM land CCSM 6 all denied that Amanda O'Day yells at the children. CCSM 1, CCSM 2, CCSM 7 and CCSM 6 all reported they were not aware of Amanda O'Day ever handing out alcohol to the staff members. CCSM 3 stated she was not present on the day Amanda O'Day supposedly passed out alcohol to the staff members. CCSM 5 stated that Amanda O'Day had only given out alcohol on one occasion, "as we were leaving after we had clocked out". On 07/21/2023, Child G's Mother reported that Amanda O'Day has a loud voice, "...but she doesn't yell". On 07/21/2023, Child E's Mother reported that she is only present at the center at the end of the day during pick up time, but she has not observed Amanda O'Day yelling at the children. She further reported that there was one child who used to be in another room, at the center, who no longer attends, who would punch and kick, so Amanda O'Day would sometimes raise her voice, "...but it wasn't excessive or inappropriate".
