On 09/08/2021, I conducted a telephone interview with the complainant. The complainant said Child A passed gas and accidently soiled his pants. The staff at the center was aware of this and they did not do anything about it. Child A remained in the soiled pants until his mother arrived to pick him up. 2 On 09/08/2021, I conducted a telephone interview with Child A and Child B's Mother. She said on 09/03/2021, while at the center, Child A "sharted" in his pants and spent the entire day in "poo pants". Child care staff member, Lathisa Durant and Mylah Tate were aware of the incident and did not call to informer her, nor was she told when she picked up Child A from the center. Child A and Child B's Mother said that Child B (8-year-old female) also attends the center. Child B told her that Ms. Durant and Ms. Tate asked Child A if he pooped his pants and Child A started crying. Ms. Durant asked Ms. Tate if Child A had a change of clothes. Child A did not, therefore, the situation was ignored. The stains were visible on Child A's shorts, when Child A and Child B's Mother arrived to pick him up. Child A had a doctor's appointment that day, therefore, he had to go straight to his appointment. Child A and Child B's Mother had to removed Child A's underwear at the doctor's appointment, so that he did not have to keep on soiled underwear. Child A and Child B's Mother said that Child A had been at the center from 12:00pm to 3:00 pm on that day. On 09/08/2021, I conducted an unannounced inspection at the center. I interviewed program director, Lathisa Durant and child care staff member, Mylah Tate. Ms. Durant and Ms. Tate confirmed that Child A passed gas and accidently soiled his underwear and pants. Ms. Durant said Child A was in the room with Ms. Tate. She overheard Ms. Tate ask Child A what was wrong with him, but Child A did not respond. She went over to ask Child A if he had to go to the bathroom. Child A said he "farted", and he thought he soiled his pants. Ms. Durant said she asked if Child A had a change of clothing and Ms. Tate said no. Five minutes later, Child A and Child B's Mother's boyfriend arrived to pick up the children. Ms. Durant admitted that no one called Child A and Child B's Mother to inform her that Child A soiled his pants. She said she is not sure if Child A and Child B's Mother's boyfriend was told at the time of pick up. Ms. Tate provided the same information as Ms. Durant. She said there was a visible stain on Child A's pants. Ms. Tate denies that Child A wore the soiled pants all day. She said Child A arrived at the center after 12:00 pm. The children took a nap at 12:50 p.m. and woke up around 2:50 p.m. Child A's pants were not soiled before nap time. Ms. Tate said the cook, Erica Lloyd noticed the stain on Child A's pants after his nap. Ms. Tate said she knew Child A and Child B's Mother was coming to the center at 3:00 p.m. because Child A had a doctor's appointment. Therefore, she did not contact her to inform her about the incident. Ms. Tate does not know if Child A and Child B's Mother was informed at the time of pick up because she did not dismiss Child A. She said Child A did not have a change of clothing at the center, even though the parent handbook requires that. On 09/08/2021, I conducted a telephone interview with licensee designee, Shanita Lloyd. Mrs. Lloyd said she is aware of the allegations. She said Erica Lloyd answered the door when Child A and B's Mother's boyfriend arrived at the center. She said does not believe that he was informed about the incident. However, she 3 does agree that staff should have informed Child A and B's Mother that Child A had a bathroom accident. On 09/09/2021, I conducted a telephone interview with Erica Lloyd. She said she is the cook. Ms. E. Lloyd said she was in the kitchen and could see into Child A's room. She noticed a stain on the back of his pants, therefore she told Ms. Tate. Ms. E Lloyd said Ms. Tate and Ms. Duran asked Child A if he had an accident and
Child A (5-year-old male) spent an entire day in soiled pants. Staff was aware that Child A soiled his pants, however, they never informed Child A's Mother, nor did they provide a change of clothing.
